Defect. Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Risk. An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy. Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Defect. Chrysler (FCA US LLC) is recalling various Dodge, Chrysler, and RAM vehicles equipped with Kidde Plastic-Handle or Push Button 'Pindicator' Fire Extinguishers. A list of the affected trailer models is available at: https://static.nhtsa.gov/odi/rcl/2017/RMISC-17V824-0103.pdf. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. Risk. If the fire extinguisher does not function properly, it can increase the risk of injury in the event of a fire.

Remedy. Chrysler will notify owners instructing them to contact Kidde for a replacement fire extinguisher, free of charge. The recall began February 14, 2018. Defect. Chrysler Group LLC (Chrysler) is recalling certain model year 2014 Chrysler Town and Country and Dodge Grand Caravan vehicles manufactured March 20, 2014, to April 22, 2014, and 2014 Jeep Wrangler vehicles manufactured March 21, 2014, to April 22, 2014. Due to a software error, the Tire Pressure Monitoring System (TPMS) may fail to learn the locations of the individual sensors while the vehicle is being driven. As a result, the low tire pressure warning light will illuminate, despite the the tire pressures being within specification. Should one of the tires lose air pressure, the driver would not be notified of the change in air pressure. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard No. 138, "Tire Pressure Monitoring Systems."

Risk. If the TPMS light illuminates because the sensors cannot be located, it could mask an actual low tire pressure condition, possibly resulting in tire failure, increasing the risk of a crash.

Remedy. Chrysler has notified owners, and dealers will reprogram the TPMS module, free of charge. The recall began on October 23, 2014. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is P63.

Defect. Chrysler Group, LLC (Chrysler) is recalling certain model year 2010-2014 Chrysler Town and Country and Dodge Grand Caravan vehicles manufactured August 25, 2010, through October 31, 2013. The affected vehicles may experience overheating of the vent window switch in the driver's door armrest.

Risk. An overheated switch may result in a vehicle fire.

Remedy. Chrysler will notify owners, and dealers will replace the vent window switch with a newer version, free of charge. The owner notification letter was issued and the remedy campaign launched on on December 31, 2014. Owners may contact Chrysler customer service at 1-800-853-1403. The passenger headrest opened without a traffic incident.

Driving home from work, went over train tracks and heard a pop from the passenger air bag. It did not deploy all the way, just came open. I looked online for a recall and although its very common, there wasn't one. It's a plastic peice that let go and it needs complete replacement.

In August 2025, the front passenger Active Head Restraint (AHR) experienced a sudden mechanical failure and deployed unexpectedly. I immediately contacted my local Chrysler dealership, where a replacement headrest was ordered. The part was placed on an indefinite backorder with no estimated time of arrival (ETA). After several months of receiving no communication, I followed up with the dealer and was given an ETA of May 2, 2026. When that date passed with no word or resolution, I returned to the dealership on June 18, 2026, only to be informed that the parts delivery date had been pushed back yet again to July 3, 2026. In an attempt to make the seat usable, I initially attempted to secure the broken mechanism with heavy-duty zip ties, but the internal spring tension was too strong for them to hold. I was forced to use a ratchet strap to compress it; however, over time, the strap slips under the constant pressure and must be repeatedly retightened. Because Chrysler has failed to provide a critical replacement safety component for nearly a year, I am trapped in a double-sided safety hazard. If I leave the headrest exploded, the seat is physically unusable. Because I am forced to temporarily bind the headrest flat just to utilize the seat, the front passenger now has zero whiplash protection in a rear-end collision. Furthermore, this unstable, makeshift binding poses a continuous, unvetted impact and projectile risk to the middle-row passenger sitting directly behind it. By fai

While parked, the headrest on the driver side exploded causing my head to be forced forward abruptly and painfully. There was no warning before this happened. Had I been driving, I may have crashed and caused injury to myself and others.

2014 Chrysler Town & Country experienced active diagnostic trouble code P2764 — Torque Converter Clutch Pressure Control Circuit Low. Vehicle was non-driveable. Authorized Chrysler dealership technician documented that voltage from TIPM to Valve Body was shorted to ground — confirming a wiring harness electrical defect. The dealership replaced the Valve Body three separate times without resolving the defect. The wiring harness was never permanently repaired despite being identified as the root cause. Manufacturer issued written admission that no repair could ensure resolution after 210 days out of service. The electrical short in the wiring harness affecting the transmission control circuit represents a safety defect that rendered the vehicle inoperable and uncontrollable during acceleration.

2014 Chrysler Town & Country – VIN [XXX] Both front active head restraints (driver and passenger) have self-deployed without any crash or impact, on two separate occasions about 3–4 years apart. The first occurred around 2019. Each time we were driving normally with occupants seated. We heard a sudden “pop” and the head restraint opened, exposing the internal mechanism. No one was struck or injured, but the unexpected deployment was distracting and occurred close to an occupant. After each event, the head restraint could not be reset or closed and remains deployed. The vehicle now has an airbag/SRS warning light that stays on, suggesting the restraint system may not function properly in a crash. There were no warning lights or symptoms before deployment. The deployed head restraints are still installed and available for inspection. A Chrysler dealer previously acknowledged the issue and ordered a replacement head restraint, but the repair was not completed due to relocation during the COVID period. The contact stated that while driving at approximately 35 MPH, the charging port located between the 2nd and 3rd row seating on the driver’s side caught on fire. No warning light was illuminated before the failure. The contact pulled off a busy roadway and pulled into a parking lot, all while her 3-year-old son was seated in the 2nd row rear driver's side seat and was being surrounded by smoke. The contact stated that it took approximately 10 minutes to pull over and safely exit the vehicle to remove her son from the vehicle. A Fire report was filed however, the Fire report number was not provided. The origin of the fire was the 110-Volt charging port due to an electrical failure. The contact stated that the child seat was also damaged and needed to be replaced due to the plastic being burned. The vehicle was towed to the residence, where it remained for several weeks. However, the vehicle was later towed back to the local independent dealer where the vehicle was purchased and remained at the dealer for further investigation. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 136,000.

We were getting in to the vehicle, the vehicle was parked, and when we were getting in, the side aire bag exploded, hitting my daughter on the face, the car never was in a accident, it wasn’t hit by nothing, the car wasn’t moving, when they exploded, smoke was coming from there

The contact stated that while the vehicle was idling, the contact's wife sat down in the front passenger’s seat, and several minutes later, the front passenger's seat headrest deployed. The headrest struck the contact on the back of the head; however, no injuries were sustained. The vehicle was taken to the dealer, and the headrest was replaced. The manufacturer was made aware of the failure and informed the contact that the warranty on the headrest had expired in September 2023. The approximate failure mileage was approximately 90,000.

Vehicle engine exploded in the middle of the night. Car would remote start on its own as I would lock the doors and walk away prior to the incident. Also the airbag light would come off and on. Notice the day of the incident that the dimmer would disappear and reappear. Very strange I thought. The house could've burned down while we were sleeping. If it wasn't for our neighbors and the quick response of those firefighters it could've possibly burned the house down while we were in there sleeping. The van was picked up by the insurance company two days ago. Not sure what happened as far as it being inspected yet. They have given me the silent treatment. Went to a Chrysler dealership, not where I bought it. This van has had one recall done in 2015. Rear quarter vent window switch.

The driver's side active restraint headrest deployed unexpectedly while parked. The vehicle has never been in an accident. Upon inspection, I can see where there are plastic retaining holes that that had a metal retaining pin in it cracked. The metal pin was still in its clip. The vehicle is drivable and available for inspection. The vehicle was parked and unoccupied when the head restraint went off. No injuries were incurred. However in order to drive the vehicle now, the seat back must be adjusted back making the driving position awkward. Neither the vehicle nor the component has been inspected by anyone at this time. There were no warning lamps or messages presented prior to the issue. I did recently notice that the back plastic cover on the headrest was askew and figured it had been bumped loose by a rear rider at some time. However, it now seems that this was showing the weakening and deformation of the plastic retaining parts that were holding the metal pin.

I was adjusting the headrest and the active headrest Broke Free and released. No excessive pressure just a standard adjustment.

The contact stated while his partner was driving at an undisclosed speed, the driver noticed smoke coming from the rear driver's side sliding door interior and a strong burning electrical wiring odor. The contact related the failure to NHTSA Campaign Number: 14V234000 (Electrical System); however, the VIN was not included. The local dealer and other unknown dealers were contacted and informed the contact that the earliest availability to inspect the vehicle was in two weeks and half. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure and informed the contact that a field investigator would be sent to inspect the vehicle. The failure mileage was approximately 187,000.

The contact owns a 2014 Chrysler Town and Country. The contact stated that while driving at 45 MPH, she crashed into the rear of a stopped Tesla, which crashed into another unknown vehicle. The floor air bag deployed and struck her knees. The driver's side air bag deployed. The passenger-side airbag deployed backward and struck the front windshield. No warning lights illuminated. The driver's side seat belt fractured. The passenger side seat belt was locked in its original position. The contact was able to continue to drive. A police report was filed. There were no reported fires. The contact sustained bruises to her chest and knees, but medical attention was not provided. The local dealer was not contacted, but the vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was approximately 84,135.

The transmission failed. There were no lights on on the dash, vehicle started making a whining noise and within 12 hrs of the onset the transmission started to fail. The van would lose the ability to accelerate and put my family and other in danger because it happened on the highway and our vehicle was stopped in traffic. The check engine light did not come on until a few hours AFTER the transmission failed, but there were no light indicators prior to this happening. The transmission has been replaced after the damaged transmission was inspected by our mechanic and he stated that with less than 100k miles on the vehicle, based on the amount of metal and debris in the transmission pan, there must have been an issue with it during manufacturing. The original transmission will be returned shortly for a core refund, so will not be available for additional inspection after that. The van was test driven by our mechanic and the issue was replicated while he was driving, ie, it would not accelerate after about 5 minutes of driving.

Wife was driving down the road and side airbags deployed without cause.

I was driving in normal conditions and as i went over railroad tracks, which were not overly bumpy, the driverside headrest hit me in the back of my head while driving. It appereared to be popped out. Took it to our mechanic and the plastic that holds the retainer pin is broken, which caused the headrest to spring forward.

Having to have a new oil cooling housing replaced at 94,102 miles. Also had to change out front struts around 87,000 miles

The passenger sliding door won't open

Failure of engine oil filter housing - engine leaking oil If the engine fails, the vehicle could stop in traffic thereby putting the driver and passengers at risk of injury from other traffic. Independent repair service fixed and stated they average 1 a week. The place that diagnosed the problem said they do quite a few repairs on this engine monthly. Nothing reported but this seems to be a common problem with this engine. Large oil drip only indication of problem.

The active headrest on the driver's seat malfunctioned and deployed while the car was parked in my driveway. I have seen reports where there has happened while driving and has caused concussions. The local Chrysler dealer is in the processes of fixing the failure.

Vehicle was built in April 2014 and now has a transmission pump failure. Vehicles built between January and April 2014 were recalled. Subsequently 2015 through 2016 Vehicles were recalled for the same reasons.The Vehicle exhibit the exact symptoms as the recalled Vehicles. I also have obd data if needed for evidence.

2014 Chrysler Town and Country: While in motion, headrest and back of seat sounded like they "exploded". Headrest had broken and snapped forward thrusting head downward towards steering wheel. Back of seat lunged forward at the same time. Now air bag light is on in vehicle. This caused headache, neck pain, back pain, and pain sensitivity to hearing.

The steering wheel controls stopped working. Had a mechanic look at it and said it was the clock spring contacts were compromised.

Left Cylinder failure /misfire on 2014 Town and Country. Most likely 2013 Engine. ) Bought minivan brand new in April 2014. Have had several Chrysler minivans never had issues. Mechanic say this minivan complete failure, too expensive to fix. Impacted Transmission and it is bad too now. No recalls on this and should be. Such a great van and was disappointed I did not get more years and miles out of it.

The cylinder 4 misfires. A leak down test was done by a mechanic and it failed. The entire cylinder head will have to be replaced. There are loads of people with this issue, and I know there was a recall at one point for certain years but the year 2014 was not included, neither was cylinder 4. There were warning signs such as shaking in the car when coming to a stop.

Cylinder 2 codes out. Check engine light on. The check engine light, came on 2 weeks ago and has not gone out. Van shakes a little bit, when in low RPM and sitting at lights. It has been confirmed by independent mechanic.

Headrest on driver side exploded on 10/12/2024. I was reclining in the seat when it happened. Forced my head forward.

Seat release for the middle right seat does not work when the seat release lever is pushed up. Failed more than 3 years ago. Prior owner did not disclose the problem.

Items of concern range from a sliding door that will intermittently refuse to respond to the key fob command, or it will randomly close on passengers. Additionally, the rear hatch unexpectantly closes on members of my family as they remove groceries. Lately, the time display and rear camera have ceased to function. At times, the dash screen lights up with a gray display that illuminates the cab and makes night-driving very difficult. When this happens, I cannot get it to go dark.

No check engine warning, unpredictable no start, no crank. Physical well being of grandchild, others and self at risk due to high temperatures. This also puts stress on mental well being, how to fix, can I fix, the cost and constant worry of being stranded. Have not confirmed with dealer. Did have a shop review. Battery, alternator good, starter is new. Issue is sporadic and hard to duplicate, but happening daily. Research on internet shows this is common occurrence without a definitive solution. Who has to have a breakdown, not just mechanically speaking to find a solutuon?

I was driving down the road and when rpms were between 1 and 2 and I applied brakes my van stalled out at the lights. My battery is new so I know it's not that. There is a large number of people that are saying there is a safety concern. I am scared to drive it in case it happens again. It was on a busy road in front of a mall. [XXX] . Please look into the number of people reporting this same issue. Also when slowing down the vehicle starts to stall like its going to die. Then I had to put myself into park and restart the vehicle and it ran fine again. I was able to get home. I have a check engine light that is related to some kind of pressure system according to o reillys auto store scanner. The safety of mysef and my child and others behind and around me put me at risk and it looks like it is a serious problem that isn't easily diagnosed. I just spend 2000 on different repairs and I explained this issue. It wasn't fixed. On that thread they are saying it could be a ecg valve gone bad. If that's the case it should be a recall. Drivers Headrest Airbag went off unexpectedly causing minor whiplash to myself, the driver and causing uncomfortable head placement and high repair costs.

Bought the car new in 2014. after approximately a year, the passenger rear sliding door began to close randomly after opening, on several occasions pinning the passenger who was entering or leaving the vehicle between the door and the frame. the car was under warranty so i had it "repaired" at the dealer. after several months, the same random closings began to occur but less frequently. Most recently, the same door could not be locked leaving the car open to burglary. The dealer says its the door latch that needs replacing and is charging me $1,600 to "fix" it. Are these doors being investigated by your agency?

The front passenger headrest activated with no cause. It exploded out luckily no one was in the seat. It is a known problem with chrysler. When I contacted the company as for as the safety warranty goes, they said I missed it by two weeks. It exploded due to faulty equipment. It is literally a time bomb waiting to go off. The driver side has not gone off, but I am afraid it will deploy when driving down the road and hit me in the back of the head. I followed up with the manufacturer but never head anything back. I purchased part out of pocket to replace.

The vehicle was able to be restarted each time. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 74,000.

Dear evaporator coil going out. This is a very common issue on these vehicles and needs to be addressed.

The oil cooler that is located under the intake manifold has a very bad issue and they crack and almost everyone of the engines like this I called about 9 diff shops and everyone of them told me it was a very common issue the one shop even keeps 5 in stock I already have changed this one but it causes them to leak oil down onto the exhaust and transmission and for some reason or another when ours started to leak badly the transmission acted up and we had to have that rebuilt so on top of the 250 dollar part it also cost almost 5k for a Transmission at same time I feel there needs to be something done about this as it a very bad common issue on all the engines like ours

Rear passenger door is stuck in the locked position and cannot be unlocked manually or with the fob. This is dangerous if children in the rear seats would need to exit the vehicle.

The passenger seat headrest deployed without warning. I slowly pulled out of a parking lot and proceeded down the road. I heard a loud bang and looked over at my daughter to ask what was that. She said ouch my head. I looked at her seat and saw the headrest deployed. I took it to the dealership and they said there wasn’t anything they could do but it was equivalent to an airbag deploying. I called Chrysler and they say my van is out of warranty and they can’t help. This is a known safety issue and yet no recalls have been made. I fear my drivers side headrest will do the same. Someone needs to fix this issue. My daughter has whiplash.

The passenger sliding door locked is stuck locked and the door will not open.

I was driving on I95 and the car completely shut down. The RPM went to 0. It started up again after about twenty minutes. I had the car repaired (throttle body replaced) and since that incident I have had this same thing happen four more times. The car seems to “resets” itself. I have had the throttle body replaced twice, the ignition control switch twice, the relay harness and then the relay redone. The van only does this periodically. The dealership had my van for six weeks and “couldn’t duplicate the problem “. This usually happens when driving for over an hour and speeds over 65mph. Last week I was driving to work, eleven miles from home and it happened again. The van restarted after sitting about thirty minutes.
