Defect. Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Risk. An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy. Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Defect. Chrysler (FCA US LLC) is recalling various Dodge, Chrysler, and RAM vehicles equipped with Kidde Plastic-Handle or Push Button 'Pindicator' Fire Extinguishers. A list of the affected trailer models is available at: https://static.nhtsa.gov/odi/rcl/2017/RMISC-17V824-0103.pdf. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. Risk. If the fire extinguisher does not function properly, it can increase the risk of injury in the event of a fire.

Remedy. Chrysler will notify owners instructing them to contact Kidde for a replacement fire extinguisher, free of charge. The recall began February 14, 2018. Defect. Chrysler (FCA US LLC) is recalling certain model year 2015-2016 Chrysler Town & Country, Dodge Grand Caravan vehicles manufactured July 31, 2015, to April 18, 2016, 2016 Dodge Journey vehicles manufactured August 17, 2015, to January 29, 2016, and 2016 RAM ProMaster vehicles manufactured August 15, 2016, to April 15, 2016. The transmission pump may seize causing a loss of hydraulic pressure.

Risk. If the transmission loses hydraulic pressure, the vehicle may lose motive power, increasing the risk of a crash.

Remedy. Chrysler will notify owners, and dealers will replace the transmission pump, free of charge. The recall began on August 18, 2016. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is S44.

Defect. FCA US LLC (Chrysler) is recalling certain model year 2015-2016 Chrysler Town and Country, and 2014-2015 Dodge Grand Caravan vehicles manufactured August 16, 2014, to December 5, 2015. The windshield on the affected vehicles may have been installed using expired urethane primer, allowing the windshield to become displaced in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 212, "Windshield Mounting."

Risk. If the windshield dislodges during a crash, there is an increased risk of occupant injury.

Remedy. Chrysler will notify owners, and dealers will replace the windshield, free of charge. The recall began March 4, 2016. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is S02.

Driver side headrest popped out and plastic pieces are broken inside. I believe there is an investigation for this: Active Head Rest Inadvertent Deployment. It randomly happened the other day while vehicle was parked in garage. I’m not sure to get the headrest fixed for safety reasons or leave it broken.

Transmission completely failed and had to be risky rebuilt. I was just driving down the highway and the transmission went into limp mode and I came to a fast slow down as it dropped the gearing. Also have noticed a huge issue with electronics putting me into a limp mode with wipers on headlights wont shut off, lose dash lights and speedometer with no blinkers or hazard lights. This left me in a very unsafe place as I had no way to tell other drivers where I was going or see how fast im going. Could have caused a huge accident 😕. Please look into this as there are many issues of this happening.

This is a safety-defect complaint regarding the Front Seat Active Head Restraint (AHR) on our 2015 Chrysler. In March 2025 the front passenger AHR SPONTANEOUSLY deployed with no collision or impact. The head restraint is now permanently dislodged and dangling from its original position. This creates an immediate safety hazard: it distracts the driver, protrudes into the front passenger seat area, and can cause pain or injury to front-seat passengers. This defeats the very purpose of the AHR and introduces new risks of harm. This is the exact defect NHTSA investigated in PE19-014 (inadvertent AHR deployment without crash). NHTSA closed that investigation in 2026 without a recall, citing FCA’s 10-year / Unlimited Miles warranty extension as sufficient remedy. However, FCA NEVER NOTIFIED US of the extension — even though our vehicle's VIN has been registered in our names at the same home address for ALMOST 5 years. We only discovered the warranty extension days ago through independent research. Our vehicle has extremely low mileage (59,666 miles total as of February 2026 — only 17,265 miles added in the 4.6 years of our ownership). We are the second owners and there is no history of accidents. Because we were never informed of the warranty extension, we could not request inspection or repair even though we were still within the 10-year extension when the headrest deployed. The contact owns a 2015 Chrysler Town and Country. The contact stated that while driving at various speeds, the vehicle shuddered while depressing the accelerator pedal. There was no warning light illuminated. The vehicle was taken to an independent mechanic, where a transmission flush was performed; however, the failure persisted, and the vehicle started stalling after the repair. An independent mechanic was informed about the failure, and the contact was informed that the transmission fluid flush would take a while to take effect. The contact was informed to monitor the issue to see if the failure continued or worsened. The failure persisted, and the vehicle was taken back to the independent mechanic, who diagnosed it with transmission failure. After negotiating with the mechanic about what to do with the vehicle, the mechanic suggested that replacing the torque converter would repair the transmission. The contact agreed to replace the torque converter. Less than a week after the repair, the check engine warning light was displayed on the instrument panel. The vehicle was taken to a local independent mechanic, and DTC: P0868 (Transmission Fluid Pressure Low) was retrieved. The vehicle was taken back to the original mechanic, and the mechanic was informed about the fault code. Plastic oil filter housing leaking oil causing check engine light to come on. P06DD oil pressure code for not changing to high pressure. Common problem with 3.6L engine. Can lead to engine failure. Failed housing needs to be replaced with upgraded aluminum housing. This is located under the intake manifold and is an involved repair that is typically in excess of $1,000.

2015 Chrysler Town and Country lost propulsion power while driving, endangering 2 occupants and surrounding drivers. While driving on a residential road, vehicle came to a stop at intersection. Upon attempting to accelerate from stop and applying gas, vehicle jolted and would not accelerate past 5 miles an hour with transmission engaging and disengaging while shifter was in drive. Vehicle's transmission would intermittently engage and disengage for 1/2 mile with no warning lights or error messages. Vehicle idled as normal, but vehicle would not engage transmission properly when shifter set to "Drive" position. Vehicle was then parked in driveway off of the road.

I was sitting in my parked vehicle in my driveway with vehicle turned off, talking on my cellphone and just adjusted the headrest and the headrest popped out like a bang, since I was leaned over I missed gettting hit. My extended warranty was up in July of this year, and was never told from my Chrysler dealership I get maintenance from was told there was a faulty headrest design.

I was driving at 515 am on the highway about 60 mph, when all of a sudden Drive aside headrest airbag went off. I am thankful that it was early in the morning and not a lot of people on the road cuz I did swerve but did not hit anyone or anything as it startled me as I was driving. I called Chrysler see case number below and they will do nothing about this. When coming to a stop the van will jerk, and when taking off it acts like it does not want to catch the gear, now we are expirancing no take off, it will not go past 20 miles, and when we try the engine will rev up and will not pick up speed.

Passenger side active head rest deployed suddenly while driving down the road on a hot day (90 degrees with heat factor 100+) Called dealer and they stated extended coverage for this expired June 13. This happened on July 28. They want to order parts, approx 170 USD and charge 108 USD labor. This is a common problem because of manufacturing with inferior plastic parts and it is violent when it pops apart unexpectedly.

I purchased the vehicle yesterday. On drive home I noticed that it stop pulling. I took foot off of gas and reapply and the vehicle picked up speed again. I got home and checked recalls for this year vehicle and noticed a safety recall for the transmission. When I called dealership today and gave the VIN # they said there were no recalls for my vehicle. But my vehicle is performing exactly like the safety recall reported. Person I bought it from said she's been the only owner. So now I'm going to have to find a shop to repair the transmission because I refuse to drive a vehicle that's unsafe. Why was my van not included in the recall when the recall list included my year make and model?

The Head Restraint (AHR) on the driver's seat opened, without warning, striking the back of my head with considerable force. I was startled and momentarily dazed, as a result. The vehicle was parked at the time, so no impact or collision was involved. The Head restraint has not been repaired and is available for inspection. I have seen many reports of the same problem with numerous other vehicles. The manufacturers should be responsible for the repairs. Thank you!

The contact stated that while her husband was driving uphill at an undisclosed speed, the transmission failed to properly shift into a higher gear, and the engine was revving abnormally. The contact pulled over the side of the road 1,000 ft away from the residence and added transmission fluid to the vehicle. The contact stated that while following her husband in another vehicle, she observed leaking fluid underneath the vehicle. The contact’s husband stopped the vehicle and the contact noticed that transmission reservoir was empathy. The vehicle was towed to an independent mechanic where it was diagnosed and determined that the transmission fluid was leaking from the bell housing, and the transmission needed to be replaced. The vehicle was not repaired. The vehicle was then towed to another independent transmission mechanic but had not yet been diagnosed. The manufacturer was notified of the failure. The failure mileage was approximately 64,000.

The passenger side active head restraints (AHRs) deployed unexpectedly while the van was unoccupied inside our garage. Upon investigation the metal latch bar was still in it's undeployed location (latched into the head rest) but there were visible plastic parts missing on the other portion of the headrest. It appears that the plastic clamps had broken at some point and caused the AHR to deploy - luckily nobody was in that seat at the time. The AHR was unable to be reset so a replacement headrest has been ordered for installation.

In approximately 10 separate instances starting around December 12th when I was driving my 2015 Town and Chrysler Mini van, it would lose power and shut off randomly. I would attempt to steer the car to the side or middle of the road (I lost the ability to turn the wheel), shut the car off and turn it back on and the car would start up again no problem. I took the car to the dealership three times for this issue and they were not able to find an issue any of the times I brought it in. It happened once when my daughter was in the car. I was concerned from my safety because I could have been rear ended. I avoid highway driving because of this issue.

Wife was driving vehicle home and loud whining began. Vehicle started to jerk on way home. Got home to driveway and car would not pull itself up the hill of driveway. Vehicle will not shift into any gear now except park.

*The drivers side headrest popped open while I was driving and yes it is available for inspection. Prior to this (several months), the passenger side headrest did the same thing while a passenger was in the seat. *When the headrest deployed/opened, it hit me in the back of the head. *No. *No. No, there were no warnings.

There is a loud ticking noise coming from the engine. A mechanic said it’s a rocket arm failure. He said there are problems with them and were recalled in 2014. After researching the problem, it appears that the problem extends past the 2014 model year. You tube has hundreds of repair videos for this engine. The mechanic said that the problem will only get worse and can cause the engine to stall and miss and it will eventually freeze up causing a possible accident. Please investigate this. I have maintained this vehicle well with oil changes every 3000 miles. Thank you.

The seals in the oil cooler adapter failed. Began leaking oil from the top of the motor down onto the exhaust. This could easily cause a fire when the oil drips onto a hot exhaust. Confirmed by several mechanics and replicated in nearly ALL vehicles with the 3.6-liter Pentastar V-6 engine. There was no warning lamps. The smell of burning oil was the first symptom. Followed shortly by seeing oil puddling under the vehicle.

The contact stated that on several occasions while parked and turned off, the tailgate button was depressed from inside of the vehicle, and the tailgate closed inadvertently. On one occasion while parked with the tailgate opened, the hatchback closed on her head and would release. The contact sustained a head injury. Medical attention was not needed. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 110,000.

OIL HOUSING HAS BEEN REPLACED 4 TIMES. SINCE 64K MILES, NOW AT 112K. THERE NEEDS TO BE A RECALL. THERE ARE SO MANY SIMILAR OCCURANCES. THIS IS A HUGE FIRE HAZZARD.

The engine developed a ticking noise. From what I have found, there are defective rocker arms in the engine. Eventually, they will fail and cause complete engine failure which can occur at any time, even when driving at high speeds. This seems to be a serious safety concern. Went to garage for vehicle to pass routine inspection. However, the OIL FILTER HOUSING was leaking quite a bit. Due to the size of the leak, I had to pay to replace the oil filter housing and the related fluids. This was about a 3 hours repair. I could not drive the vehicle until the part was replaced. This part failing and leaking at about 85,800 miles seems to have been premature. The garage staff told me the vehicle could not pass inspection until the part was replaced. It seems as if the part has had issues and the newer components may be more durable.

Noticed the vehicle was leaking fluid. The vehicle temperature GAUGE was also running HOT. Went to a garage on 9 18 2024 and was told the 2 transmission hose lines were leaking and needed replacing. Was a significant leak. Was not able to use the vehicle due to HOT temperature gauge and only able to drive the vehicle a short distance and leaking fluids. Had to have both transmission hoses replaced at about 84500 miles. (SEE DIFFERENT complaint, that 1 - 2 months later, on 12 5 2024 had to go to a garage and have the OIL FILTER HOUSING replaced due to another leak and replace all related fluids).

Two blocks from home my Pentastar oil filter housing & cooler line broke and there was oil all over and the car stalled just before my driveway. Could not start it and had a towing company take to Meekhof Tire & Service where I have my work done. Safety was ok. No inspected other than Meekhof checking to see what was wrong. There were no warning lights on dash. Chrysler has known about this problem for a long time and never did a recall. So I had to get a used 3.6lV6 engine.

On interstate 95 I was on the on ramp and tried to accelerate and merge onto the highway the rpms went way up and the vehicle slowed down and has no acceleration leaving me in the path of a tractor trailer who honked his horn and barely missed me. I limped to the shoulder got a toe trk to row it to a repair facility. I was told the tranmission pump had gone out and that there was a recall for these transmissions

Minivan was parked in our driveway and we heard a loud bang. The driver side headrest had spontaneously sprung open. Van has never been in an accident.

The driver's side, sliding door will not open manually or (using automatic) from inside of vehicle or outside of vehicle, or with the remote key. Door is stuck closed like it's jammed. I am very careful with my doors, and tell everyone not to slam them hard. I don't trust this doors safety.

Driver side rear sliding door lock doesn't work. The door makes a loud buzzing noise and does not unlock. The door can be unlocked manually with force. There is already a manufacturer recall for this exact same condition and part, but doesn't cover my car. My car was built 72 days prior to the start of the covered equipment, but is showing the exact same issues as those covered.

2015 Chrysler Town and Country. Black "Leather". Right Front Passenger Headrest. I was loading something into the Right Sliding door when my elbow bumped the headrest. It was slight enough that I thought nothing of it, but when my passenger got into the R/F seat, the headrest was thrust forward. We attempted to "reset" it but it would not latch in the back position. Upon assessment, we found the plastic "latches" broken. This is a design or quality problem with safety implications and Chrysler needs to replace this headrest in affected vehicles. There are several reports in the 2014-2015 model years with this issue. It should be a safety RECALL. Some complaints are that the parts are on backorder. This cannot be allowed. Chrysler should be held accountable.

The contact stated while the vehicle was parked, the rear driver’s side and rear passenger’s side doors were inoperable while depressing the unlock button or attempting to exit the vehicle manually. The contact stated that the children seated in the rear of the vehicle had to go over to the front driver’s or front passenger’s side seats to exit the vehicle. No warning light was illuminated. The vehicle was taken to an independent mechanic and the independent mechanic made the contact aware of TSB Number: D21-15; and informed the contact that the vehicle was under warranty. The contact was informed that the door lock actuators on the rear driver’s side and passenger’s side doors were faulty. The vehicle was not repaired. The manufacturer was notified of the failure and informed the contact that the vehicle was not included in the TSB. The failure mileage was approximately 115,000.

Driving on a highway with my teenager sitting in the front passernger seat when all of a sudden we heard a loud pop and the headrest hit the back of my teenagers head. It was like an explosion. Fortunalty my teenager was looking down and said it didn't hurt them. Yes it is available for inspection. Someone could get a concussion given the force that headrest popped so forcefully. I have not taken it to a Chrysler dealer since it sounds like they dont do anything for it. No warnings.

I was driving and all of a sudden my 2015 town and country was shifting erratically then the transmission had no drive all RPMS kept going up but no movement there needs to be a recall for these 62te transmissions there are so many complaints about these transmissions failing and nothing has been done over years and years same issue

The sliding passenger door locks when the other doors are unlocked. But when you lock the car that door is not locked so we have to keep the kiddie lock on and not use it and sometimes we have to open it manually. The kiddie locks automatically comes on when the door closes.

With normal use, no accident has occurred both front driver and passenger active headrest have deployed due to faulty plastic casing. One side, front passenger occured with occupant pulling on headrest to enter vehicle in the middle row. One , front driver side, occured when a shopping bag GENTLY bumped the headrest while being removed from the middle row behind the seat. They will not "click" back into place because of a broken piece. If this deployed while driving, rather than casual contact (not rough contact!) it would cause injury to the neck or confusion in a driver to no longer be alert.

The van began making a squeal noise and the RPMs fluctuating then the check engine light came, I got it scanned the code was for low transmission pressure, This vehicle has less then 100,000 miles on it. The vehicle was inspected by two different repair shops.

The Active Head Rest on the driver seat in my 2015 Chrysler Town and Country. I purchased this vehicle brand new from the dealer in 2015 from the Chrylser Dealer in Springfield, IL. No one was driving the vehicle at the time, we only notice the headrest had activated after entering the vehicle after it was parked in our garage. I have contacted my local dealer (Burlington, IA) to determine the repair/replacement required. No other warning lights are shown on the dashboard. While driving, transmission began to stutter and car began to buck. No check engine lights came on. Transmission then had a whining noise and was overheating.

The driver's side headrest deployed while driving. My husband was driving on [XXX] at about 70-75 mph, when his headrest suddenly and unexpectedly burst open. This is a serious safety hazard. He said it felt like he got punched in the back of the head and was startled. Thankfully he didn't lose control of the vehicle and avoided an accident. Front passenger headrest exploded without being in an accident. Serious injury could have occurred.

While driving the vehicle it loses power and will not shift gears. This happened in the middle of a busy highway with me and my children in the car. When turning the van off and back on it will then go into gear but then quickly loses power and will not move forward. This happened 7/28/2024
