Defect. Ford Motor Company (Ford) is recalling certain 2020-2022 F-Super Duty F250, F350, F450, F550, 2021-2022 F600, and 2022 F750 vehicles previously repaired incorrectly under recall number 24V957. Biodiesel deposits may form on the pump drivetrain roller components, which could lead to failure of the high pressure fuel pump.

Defect. Ford Motor Company (Ford) is recalling certain 2020-2022 F-250 SD, F-350 SD, and F-450 SD vehicles. The 360-degree view camera system may not display a rearview image properly in certain lighting conditions.

Risk. A rearview camera image that does not display properly can reduce the driver’s view behind the vehicle, increasing the risk of a crash.

Remedy. Dealers will update the image processing module software, free of charge.This will be a phased campaign, with the remedy becoming available in different phases based on model years. Interim letters notifying owners of the safety risk were mailed on October 30, 2025. Additional letters will be mailed once the remedy is available on each phase. Ford Motor Company (Ford) is recalling certain 2021-2023 Bronco, Explorer, Lincoln Aviator, F-250 SD, F-350 SD, F-450 SD, F-550 SD, 2021-2022 Lincoln Navigator, Mustang, F-150, and 2022 Expedition vehicles. The low-pressure fuel pump may fail, which can result in an engine stall while driving.

Risk. An engine stall increases the risk of a crash.

Remedy. Dealers will replace the low pressure fuel pump on super duty vehicles and update the powertrain control module (PCM) software on all other affected vehicles, free of charge. Defect. Ford Motor Company (Ford) is recalling certain 2018-2023 Expedition and Lincoln Navigator vehicles equipped with third-row seating; and 2019-2023 F-Super Duty F-250, F-350, F-450, and F-550 SuperCab vehicles. The owner's manual does not include instructions for adjusting or removing certain head restraints. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 202, "Head Restraints."

Risk. Without instructions, the head restraints may be improperly adjusted, increasing the risk of injury in the event of a crash.

Remedy. Ford will mail the Owner's Manual addendums that describe how to adjust and remove the head restraints for the center seating positions, free of charge. Defect. Ford Motor Company (Ford) is recalling certain 2020-2021 F-150, Expedition, 2020-2022 Super Duty F-250, F-350, F-450, F-550, and 2020-2021 Lincoln Navigator vehicles. The windshield wiper arms may break, causing the windshield wipers to fail.

Risk. Inoperative windshield wipers can reduce visibility in certain driving conditions, increasing the risk of a crash.

Remedy. Dealers will replace both front windshield wiper arms, free of charge. Interim owner notification letters, informing owners of the safety risk, were mailed on May 24, 2022. Defect. Ford Motor Company (Ford) is recalling certain 2017-2022 F-250 and F-350 Super Duty vehicles equipped with gasoline engines and aluminum driveshafts. Underbody heat and noise insulators may loosen and contact the aluminum driveshaft, which could damage the driveshaft and cause it to fracture.

Risk. A fractured driveshaft can cause a loss of drive power, or a loss of vehicle control if the driveshaft contacts the ground. Additionally, unintended movement could occur while parked if the parking brake is not engaged. Any of these scenarios can increase the risk of a crash.

Remedy. Dealers will inspect and repair the driveshaft as necessary, and properly attach the underbody insulators, free of charge. Owner notification letters were mailed on April 7, 2022. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 22S09.

Defect. Ford Motor Company (Ford) is recalling certain 2021 Super Duty F-250, F-350, F-450, F-550, F-600, and Medium Duty F-650 and F-750 vehicles equipped with 6.7L diesel engines. A small void may be present in the top-center of the vehicle's underhood secondary fuel filter cap, which may progress to a pinhole over time, allowing fuel to leak from the top of the secondary fuel filter.

Risk. A fuel leak in the presence of an ignition source can increase the risk of a fire or injury.

Remedy. Dealers will replace the primary and secondary fuel filters, free of charge. Owner notification letters were mailed January 28, 2022. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 22S01.

Defect. Ford Motor Company (Ford) is recalling certain 2020-2021 F-350 Super Duty pickup trucks equipped with 6.7L diesel engines and Dana model 275 rear axles. The rear axle housing may not support the load of the pickup truck and deform, which could result in driveshaft separation.

Risk. Driveshaft separation may result in a loss of drive power, or unintended vehicle movement if the vehicle is in park without the parking brake applied, increasing the risk of a crash.

Remedy. Dealers will inspect the axle housing for deformities, and repair or replace the housing, as necessary, free of charge. Owner notification letters were mailed on September 3, 2021. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall 21S31.

Defect. Ford Motor Company (Ford) is recalling certain 2021 F-350, F-450, F-550, and F-600 vehicles equipped with dual rear wheels and front wheel hub extenders. The front wheel hub extenders may be improperly attached to the front rotor hub assemblies, which could result in separation of the wheels and wheel extenders from the vehicle.

Risk. A detached wheel and wheel extender can increase the risk of a crash.

Remedy. Owners are instructed not to drive their vehicles until the torque of the wheel extender nuts can be verified. Dealers will inspect and tighten the front wheel hub extender nuts, free of charge. Owner notification letters were mailed on June 18, 2021. Defect. Ford Motor Company (Ford) is recalling certain 2021 Super Duty F-350 and F-450 vehicles equipped with a fifth wheel left-hand mounting bracket that may be cracked.

Risk. A cracked mounting bracket could result in the fifth wheel detaching from the vehicle while towing, increasing the risk of a crash.

Defect. Ford Motor Company (Ford) is recalling certain 2021 F-150 and 2020-2021 F-250, F-350, F-450, and F-550 Super Duty vehicles. During manufacturing, the windshield may not have been properly bonded to the vehicle, allowing it to detach in a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 212, "Windshield Mounting."

Risk. In the event of a crash, if the windshield separates from the vehicle, it can increase the risk of injury.

Remedy. Ford will notify owners, and dealers will remove and reinstall the windshield, free of charge. The recall began April 16, 2021. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 21C06.

Defect. Icon Vehicle Dynamics, LLC (Icon) is recalling all 05-UP FSD 2.5 Centerline Stabilizer Kits with part number 65050. The kits were sold as an accessory for certain 2005-2021 Ford F-250 4X4 and F-350 4X4 pickup trucks. The foam ring inside the stabilizer "through shaft" may block the piston ports, possibly causing the steering to lock up.

Risk. If the steering locks, it can increase the risk of a crash.

Remedy. Icon will notify owners, and dealers will install a new shock assembly with a revised foam ring and steel support disc, free of charge. The recall began December 21, 2020. Owners may contact Icon customer service at 1-951-689-4266.

Failed Component & Availability for Inspection: The Bosch CP4 high-pressure fuel pump (HPFP) failed internally, shedding metallic debris throughout the entire fuel system. This matches the exact defect in NHTSA Recall 24V-957 and Ford Safety Recalls 24S78 and 26S17. The failing CP4 pump and eight injectors were replaced on 03/14/2026 (Eastpointe Invoice #5997) due to contamination. The truck is located at Bob Maxey Ford in Detroit, MI, and is available for inspection alongside work orders from an authorized Ford dealership and two independent facilities. Safety Risk: The failure caused an abrupt, total loss of motive power on an active highway off-ramp, followed by an immediate no-start stall. Coasting without motive power across traffic lanes created an imminent collision hazard before the truck was towed by Troy’s Towing on 06/05/2026 (Invoice #34647). Problem Confirmation: •03/14/2026: Eastpointe replaced the failed CP4 pump and eight injectors (Inv #5997). •04/07/2026: Bill Brown Ford pulled DTC P228F, found debris on the regulator screen, and advised full fuel system replacement (RO #80302). •04/29/2026: Eastpointe confirmed metal contamination upon teardown (Inv #6342). •05/14/2026: Diesel’s Place found metal shavings on the regulator valve (Inv #2118). •Current: Bob Maxey Ford scanned DTC P0087 but reported no debris, contradicting prior documentation. Inspections: Bill Brown Ford inspected the vehicle on 04/07/2026 and documented debris. Bob Maxey Ford retrieved DTC P

The contact owns a 2021 Ford F-350 SD. The contact stated that while driving 50 MPH, the accelerator pedal was depressed; however, the vehicle failed to accelerate as intended. The engine seized, and the vehicle was pulled over to the shoulder of the roadway. The contact stated that while attempting to start the vehicle, the vehicle failed to start. The vehicle was towed to the dealer, where it was diagnosed with high-pressure fuel pump failure. The contact was informed that the high-pressure fuel pump needed to be replaced. In addition, the contact was informed that a repair request had been submitted to the manufacturer; however, the request was denied. The contact was informed that the failure codes displayed were not associated with an unrepaired recall. The contact received notification of NHTSA Campaign Number: 24V957000 (Fuel System, Diesel). The manufacturer was contacted by phone, and a case was filed. The failure mileage was approximately 89,545.

The high pressure fuel pump failed sending metal shavings through out the entire fuel system. I had no heads up and the failure left me stranded on top of a bridge. I called 911 twice and finally the cops showed up and wrecker towed me off the bridge. Ford is aware of this known issue with this make and model, and even put a recall partial recall / quick fix to increase the flow around the pump to keep it cooler. I had the quick fix recall completed but it did not prevent the failure and me being stranded on a bridge.

The contact owns a 2021 Ford F-350 SD. The contact received notification of NHTSA Campaign Number: 24V957000 (Fuel System, Diesel). The contact stated that while driving 20 MPH, the message "Reduced Power" was displayed, and the check engine warning light was illuminated. The vehicle stalled and failed to restart. The vehicle was towed to a dealer and was diagnosed with failures of the CP4 fuel pump, cycloid filters, and contamination of the entire fuel system. The vehicle was not yet repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 30,000.

Vehicle went into limp mode and we towed it to the dealership. They ran diagnostics and found that the fuel filter was causing a fault for water in the fuel filter. They replaced the fuel filter and ran diagnostics again and it flagged failure for Low pressure fuel pump. They replaced the low pressure fuel pump and reran diagnostics. When they reran diagnostics it flagged for High pressure fuel pump failure and now they are in the process of tearing it down to the point of failure to find out the cause. My vehicles vin was not included in the original recall and believe that it should have been included.

I was driving my truck home when it completely shut off, I lost all power to everything, including the power steering brakes. When I got home, I did some research on the Internet to see possible causes. Apparently there is a service bulletin for the fuse box having water getting into it from the cowl of the hood, which states it can do exactly what happened to my truck? You can also keep your truck running and not shutting off and just to hold host of other safety issues with the service bulletin. . So I brought my truck in to get diagnosed by Ford and they did indeed say it was the fuse box under the hood that got water into it and shorted it out. Ford would not cover this part even though I have a factory extended warranty, annoying this is a common issue for 2020 to 2022 Ford trucks they issued a recall on expeditions in a couple other vehicles, but not Ford trucks specifically the super duties. How is this? OK that they don’t tell a customer who purchased a vehicle that this could potentially happen and then when it happens, they refuse to cover anything even though it’s their fault why it happened.

Vehicle Safety Complaint – 2021 Ford F-350 I am submitting this complaint regarding a safety-related failure involving my 2021 Ford F-350. While driving under normal conditions, the vehicle suddenly entered “limp mode,” resulting in a significant and immediate loss of power. Approximately 20 miles later, the vehicle completely shut down, leaving me stranded and requiring the truck to be towed to Gene Messer Ford for diagnosis and repair. The failure created a potentially dangerous situation, as the loss of power limited my ability to maintain safe speeds and respond to surrounding traffic conditions. After the vehicle was delivered to the dealership, it took approximately one week for inspection and an additional two weeks to complete repairs. During the repair process, I was informed that newly installed components—including the fuel rail and two fuel injectors—were defective and required replacement again. This raises concerns about whether the root cause of the failure was properly identified and whether contamination or systemic failure within the fuel system was present. I am also aware of widely reported issues involving high-pressure fuel pump (HPFP) failures in Ford diesel engines, which can result in catastrophic damage to the entire fuel system, including injectors, fuel rails, and lines. Given the nature of my vehicle’s failure and the components involved, I am concerned that this incident may be related to a broader, known defect. This type of failure—sudden loss

Repair remedy has not been addressed. Leaving the vehicle owners footing the bill when a fuel system problem arises. It's been since 2024 and FMC has not done anything about it. The contact owns a 2021 Ford F-350. The contact stated that while operating the vehicle, the check engine warning light remained illuminated. The vehicle was taken to the local dealer who diagnosed that the DEF Pump, DEF Pump Lines needed, and DEF heater element needed to be replaced. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 92,567.

Cold side intercooler boost tube cracked due to constant vibration impact from coolant overflow hose routed directly above and on top of it. Part and vehicle is available for inspection. The split in the boost hose caused me to loose power while attempting to merge onto a road, almost being hit by another vehicle. After research there are multiple other instances of complaints of the same thing happening to others and multiple aftermarket manufacturers producing replacement and upgraded parts with references and warnings of the same thing happening for the last 10 years. No warning lamps or symptoms prior to failure. After failure no warning lamps but could hear an audible hiss. The vehicle has not yet been inspected by the dealer or police. I was told by their parts people it is a regular failure and the part costs typically around $200 and they only sold it as a full part and not just the rubber replacement hose althought they are separate parts connected with hose clamps.

Driving and abrupt transmission shifting issues.

On 11/19/25 , my 2021 Ford f350 burned to the ground due to engine malfunction. From what the fire department said.

The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact stated that while driving at an undisclosed speed with the cruise control activated, the contact heard an abnormally loud popping and clicking sound. The check engine warning light was illuminated. The message "Decreased Engine Power" was displayed, and the vehicle experienced a loss of drive power. The contact coasted off the road and came to a complete stop. The message "Shift to Neutral" was displayed. The contact turned off the vehicle and let it cool down for five minutes. The vehicle failed to restart. The vehicle was towed to an independent mechanic, who determined that the high-pressure fuel pump had failed. The dealer was made aware of the failure but confirmed that parts were not yet available. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 132,900.

Back up camera screen goes blue when put in reverse. Then after 20 seconds says camera unavailable see dealer. I had dealer repair with a software reset. Worked for about 2 weeks then same symptoms. Noticed Ford has a recall for the same symptoms on other vehicles but for some reason F-350s are not a part of it. Would suspect the truck uses the same camera system as the other ford vehilces under the recall. Ford needs to step up and cover the F-350s

At this point, nothing has happened. This is a fuel pump, bio-diesel issue that can cause a lack of power which is a safety risk. This recall has been active for 7 months with no remedy listed, but according to Ford Motor Company will require a software update? The remedy is replacing the fuel pump with a DCR valve. Waiting for the truck to breakdown and potentially leading to loss of life should not be acceptable. They know about the issue, they say no remedy, yet they are continuing to manufacture these vehicles. Are they manufacturing new vehicles with a known safety risk or did they fix the problem on newer model vehicles and just stonewalling current owners?

There is an active recall on my truck and it is affecting my ability to use my truck for work purposes. I am requesting: A full inspection and evaluation of the fuel systems under the scope of Recall 24S78 once the software becomes available. While operating my vehicle on a busy road the power failed and vehicle shut down. Saftey of myself and others were at risk as this occurred while actively driving. Upon restart attempt vehicle failed to start. Vehicle was towed to and inspected by an independent service center. Mechanic advised, based on codes from check engine, the high pressure fuel pump failed and metal shavings were in my fuel system. Mechanic replaced pump, flushed system. Truck is unable to be driven due to constant power failures. Advised to tow truck and get a rental, both at my cost, until a fix was discovered which could be upwards of 1 year, keep receipts as I MAY be able to submit for reimbursement but it was too soon to know if those reimbursement services would be covered under a fix for the recall 24S78.

Ford knew there was an issue back in 2021 as they so filed with your agency. Yet waited. Then warranty claims continued. Now, four years later they file as a safety issue, BUT have told me twice in phone calls that I can drive the truck, despite it could shut down on it's own while driving. I tow donated RV trailers to the North Carolina disaster, now I cannot as my dealership advised I not go on long trips. Ford refuses a like loaner, or any loaner at all. This is a Ford issue, not mine nor the dealership and Ford needs to either replace all suspected parts, have a resolution now or proved no charge loaners of like kind to all owners.
