Defect. Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Risk. An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy. Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Defect. Chrysler (FCA US LLC) is recalling various Dodge, Chrysler, and RAM vehicles equipped with Kidde Plastic-Handle or Push Button 'Pindicator' Fire Extinguishers. A list of the affected trailer models is available at: https://static.nhtsa.gov/odi/rcl/2017/RMISC-17V824-0103.pdf. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. Risk. If the fire extinguisher does not function properly, it can increase the risk of injury in the event of a fire.

Remedy. Chrysler will notify owners instructing them to contact Kidde for a replacement fire extinguisher, free of charge. The recall began February 14, 2018. Defect. FCA US LLC (Chrysler) is recalling certain model year 2015-2016 Chrysler Town and Country, and 2014-2015 Dodge Grand Caravan vehicles manufactured August 16, 2014, to December 5, 2015. The windshield on the affected vehicles may have been installed using expired urethane primer, allowing the windshield to become displaced in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 212, "Windshield Mounting."

Risk. If the windshield dislodges during a crash, there is an increased risk of occupant injury.

Remedy. Chrysler will notify owners, and dealers will replace the windshield, free of charge. The recall began March 4, 2016. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is S02.

Defect. Chrysler Group LLC (Chrysler) is recalling certain model year 2014 Chrysler Town and Country and Dodge Grand Caravan vehicles manufactured March 20, 2014, to April 22, 2014, and 2014 Jeep Wrangler vehicles manufactured March 21, 2014, to April 22, 2014. Due to a software error, the Tire Pressure Monitoring System (TPMS) may fail to learn the locations of the individual sensors while the vehicle is being driven. As a result, the low tire pressure warning light will illuminate, despite the the tire pressures being within specification. Should one of the tires lose air pressure, the driver would not be notified of the change in air pressure. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard No. 138, "Tire Pressure Monitoring Systems."

Risk. If the TPMS light illuminates because the sensors cannot be located, it could mask an actual low tire pressure condition, possibly resulting in tire failure, increasing the risk of a crash.

Remedy. Chrysler has notified owners, and dealers will reprogram the TPMS module, free of charge. The recall began on October 23, 2014. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is P63.

Defect. Chrysler Group, LLC (Chrysler) is recalling certain model year 2010-2014 Chrysler Town and Country and Dodge Grand Caravan vehicles manufactured August 25, 2010, through October 31, 2013. The affected vehicles may experience overheating of the vent window switch in the driver's door armrest.

Risk. An overheated switch may result in a vehicle fire.

Remedy. Chrysler will notify owners, and dealers will replace the vent window switch with a newer version, free of charge. The owner notification letter was issued and the remedy campaign launched on on December 31, 2014. Owners may contact Chrysler customer service at 1-800-853-1403. Service Bulletin sent to all dealers to update the firmware in the affected vehicles VMI computer. This allows for more amperage to draw the Powerkneel up allowing the up-limit switch to be tripped consistently. If the up limit switch is not engaged, a vehicle may not come out of park or the conversion may not operate properly next time it is deployed

The contact owns a 2014 Dodge Grand Caravan. The contact stated that while driving 40-45 MPH and depressing the accelerator pedal, the engine revved but failed to accelerate as intended, and the transmission skipped gears, and made an abnormal whining sound. The vehicle was driven to the residence. The vehicle was towed to an independent mechanic, where it was diagnosed, and metal shavings were found inside the transmission. The mechanic determined that the transmission needed to be replaced. The manufacturer was notified of the failure by phone and confirmed that the VIN was not under recall. The failure mileage was approximately 84,900.

The headrest safety feature deployed while the car was parked with the key OUT of the ignition. The internal plastic components have failed and broken and the headrest is not repairable. This incident happened on 9/23/2026. No prior warning lights were displayed and no previous symptoms were visible before failure. Currently, part is available for inspection. A dealer was contacted to check on warranty replacement. Vehicle is out of even the "extended" warranty due to the time frame being from date of service instead of being extended from the end of the initial warranty period. Part was not inspected by the dealer. Due to the potential severe impact these devices could enforce, these headrests should be recalled and replaced with new designed headrests and/or replaced with standard non-safety headrests for the life of the vehicles.

Safety issue with slider doors not opening. You can hear the sound of the locking mechanism noise continuing after attempting to open the doors. This will happen when it is very hot or cold weather. I think it might be the actuator. I have three toddlers and have had to climb over the front seats to get into the back seats!

The airbag compartment in the drivers seat headrest popped open while driving and I have been unable to pop it back in. I was not injured but it scared me and could have caused an accident.

Tire pressure sensor light keeps coming on I've had the tire pressure sensors replaced with non OEM and now OEM Chrysler parts and vehicle doesn't recognize any of the sensors other than the Left front but all sensors pass when individually scanned vehicle was made with in one week of end date of the safety recall p63 NHTSA 14V-632 mechanic shop found this recall and thinks this vehicle should be included but I can't find a dealership that will honor the recall I have 2 other invoices for this same problem

The passenger's headrest just exploded.

The contact owns a 2014 Dodge Grand Caravan. The contact stated that upon entering the vehicle with other passengers, the passenger's side sliding door failed to latch and close securely. The door ajar warning light was illuminated. The vehicle was not taken to the dealer or to an independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure, a case was opened, and the contact was referred to the NHTSA Hotline for assistance. The failure mileage was 77,000.

The contact owns a 2014 Dodge Grand Caravan. The contact stated that while driving at 30 MPH, the vehicle suddenly came to a stop. The RPM increased to 4,000 and then decreased. The brake pedal was depressed to avoid a crash. The contact stated that the transmission became inoperable. The transmission downshifted unexpectedly, and the vehicle bucked for 1/8th of a mile. There was an abnormal sound coming from the transmission. There were no warning lights illuminated. The vehicle was towed to an independent mechanic who determined that the transmission needed to be replaced. The contact called the local dealer, who confirmed there were no open recalls associated with the VIN. The dealer emailed the VIN to the manufacturer. The manufacturer was contacted and referred the contact to the NHTSA Hotline. The failure mileage was approximately 79,000.

Reported on 09 - 24,25,26 - 2025, after researching on you tube and the net and finding other vehicles with same issue, dodge case was opened with NO assistance from dodge at this time! Out of warranty! Dodge is aware of the problem and extended a warranty on the issue up to 10 years/ unlimited mileage...Problem; While driving down the interstate at highway speed my drivers side SEAT BACK HEAD RESTRAINT EXPLODED, almost causing an accident! No warnings, No check engine lights, No airbag lights, nothing just a loud bang and the head restraint slammed forward into the back of my head.....

I have a 2014 Dodge Caravan.The van only has 48 k miles. The battery went bad so I replaced the battery. Then the problem started. 1st the car will NOT turn off. I remove the key and the engine is still running and won't turn off. I had to remove the ASD relay which was very hot. Once I was able to shut down ,now the dashboard shows the car in reverse but it is actually in park and won't come out of gear. The check engine light is on, the tire is low light is on, and the car with tire tracks is on,(abs). I looked up recalls and they have had a recall on the fuelpump relay .

Passenger headrest airbag went off /exploded open while I was parking the van in my driveway. No one was sitting in the passenger seat so no one was injured. Yes, you can inspect it. I called dodge but they said there were no recalls. No one has inspected it. No warning lights.

First, the back hatch would just open itself even if you were going 30 miles an hour or under. Had it checked out couldn’t afford the diagnostics so I took out the fuse. Second, there is a ticking in my right back corner. It doesn’t happen when you’re going down the road, but it happens while you’re parked or just starting out. Mechanic friend of mine checked into it we would have to trace the electrical from front to back and again, I cannot afford that. So there’s 135,000 miles on my vehicle now and this happened about six months ago. I forgot there was a place to report it. I’m not very car savvy. Also, I had to replace my entire starting my starter, my alternator, a battery actually too and now I’m going to have to replace the switch for the ignition that’s all happened within one year or less. Just thought somebody should report these each time I fixed an ignition issue, the problem was fixed for at least a month or two and then the next one would happen. Very aggravating.

The passenger side headrest just exploded. It turns out that this is a safety feature that deployed on its own as part of the housing that holds the unit together just apparently broke. Had someone been sitting there, this would have blasted them in the back of the head. There were no warnings or messages. Now I'm concerned the driver side headrest will explode too while I'm driving and I have an exploded passenger headrest thats now not safe for passengers.

My car shuts down while idling at a stop light. Before that happens, the rpm drops down, the car shakes a bit, and then the engine dies. What could be the problem?

Was driving down the highway coming back from Orlando when my air bag light went off. Heard cracking in my head test and all of a sudden it expired and deployed and hit me in the back of the head and almost knocked me out. Made me dizzy and I had my kids in the car. This should not have happend if I was not in a car crash and dodge refuses to fix it. It was faulty without any warning. There are many people this has happend to and the had a extension back in 2024 and to repair them and I never received a letter

The seals in the oil cooler/filter assembly are leaking and causing oil to run down the motor and along the exhaust system underneath potentially causing a serious fire risk. The leak could also cause the engine to seize and pose a risk while driving at highway speeds or in corners. When searching for recalls on the internet it says there isn’t enough vehicles with the problem to trigger a recall. It don’t take long searching to see this is a wide spread well know issue with most all the 3.6 and 3.2l engines. It was a terrible plastic design places in one of the hottest areas of the engine. This issue was so common dodge eventually changed to design of the cooler to correct the safety issue and it was so prevalent on all these engines. The fix is fine but it has left all these other vehicles on the road with the unsafe design flaw. I really can’t understand why an issue as well known, common and serious has been allowed to remain without a recall, especially given they have an updated replacement part available to correct the issue. I can’t help but wonder if this issue was on an import it would have had a recall years ago but because it’s a US manufacturer is getting swept under the rug. Please do the right thing and look into the issue, especially as common as it is. I can’t even get an inspection sticker on the vehicle because is unsafe and doesn’t meet the standard for a Maine inspection, so there is no way one can argue it isn’t a safety issue.

While driving in the parking lot of my kids school to drop them off in the morning, I heard a loud noise and felt something hit me in the back of my head with some force. It was not painful, but was actually very uncomfortable. I stopped in the parking lot to see what it was. I noticed that my driver side headrest was out of place (the fabric portion was popped forward) away from the plastic backing. I could see the the springs inside the headrest. My husband used a zip tie to pull the headrest back together. My vehicle was not in any accidents to cause this to happen. I did not take it to a dealer to inspect, as it just occurred. There were no warning signs/lights. It was very unexpected and abrupt. No one was injured, but my head was actually uncomfortable and I very glad that I did not have my son driving at the time, as he could have been injured due to the spring loaded force.

The contact owns a 2014 Dodge Grand Caravan. The contact stated while operating the vehicle, the passenger’s side electronic sliding door locking mechanism malfunctioned and erroneously unlatched. Additionally, on several occasions, the sliding door failed to unlatch, and the door could not be opened. The contact stated that occasionally while driving at undisclosed speeds, the message that the door was not latched securely was displayed. During the failure, the interior roof light remained illuminated, and the vehicle would not lock as intended. The battery was disconnected and reconnected to correct the failure. The vehicle was taken to the local mechanic who diagnosed that the siding door locking mechanism was faulty and needed to be replaced. The vehicle was not yet repaired. The local dealer was notified of the failure. The manufacturer was not yet notified of the failure. The failure mileage was 235,000.

The contact's mother owns a 2014 Dodge Grand Caravan. The contact stated that after shifting into drive(D), the vehicle failed to accelerate upon depression of the accelerator pedal. The contact stated that the check engine and traction control warning lights were displayed on the instrument panel. Due to the failure, the vehicle was towed to an independent mechanic where the contact was informed that the fuse box in the engine had melted. The vehicle was not repaired and remained in the possession of the mechanic. The manufacturer was not notified of the failure. The failure mileage was unknown.

I have a problem with fuel pump circuit .it stays open and drain the battery after I shut the car off ,the problem with the fuel pump rely witch s integritated in the TIPM, to solve the problem I have to replace whole TIPM wich is cost $1070. All Dodge Grand Caravan model 2010 to model 2020 have same problem . Please liste this problem as a recall. Thanks for understanding.

Sliding door actuator failed causing the door to remain in the locked position and no way to unlock. The actuator arm broke off due to inaffective connection and therefor without cutting a hole in the door it cannot be opened

Hello,my concern is safety and All of the recalls,have never been sent to my mail/honored.There seems to be safety issues with the 2014 Dodge Grand Caravan engines,transmissions,tp sensors,door bend agitator.Theres more but Id rather have a safe healthy outcome.Also my vehicle accelerates at a un safe speed of over 80 mp when at 30 mpg without my foot even being on the pedal??? Oh my not safe,and police have to help stop vehicle on freeway with 5 lanes of ongoing traffoc very unsafe and scarry.Please help get these issues fixae asap .Thank you

The vehicle suddenly shuts down while driving creating a dangerous situation. The dealership diagnosed it as a faulty ignition switch and said it is common and that there was the same issue on previous generation of caravan that was recalled. I have it scheduled for replacement and will ask for the old part back.

The head rest popped out for no reason. I noticed it when I got into the car.

As I was driving down the road, doing about 40-45 mph, the airbag in my driver's seat headrest deployed. I was jolted forward and suffered neck and back pain, as well as a headache. Due to the neck and back pain, I had to visit the ER and have a CT scan done. There were no lights on the dash at the time of this happening to indicate it was going to malfunction. This has not been investigated yet, as it happened over the weekend but I will be reporting to the manufacturer as soon as they're open as well.

Vehicle started on fire while parked. It was a total loss. The auto shop reported that the fire started due to faulty wiring under the driver side seat. Specifically, related to the harness wiring. Thankfully this didn't start while I was driving. I have seen other reports on this website for similar fires on the Dodge Grand Caravan AND an actual recall for the Crysler Town and Country for similar fires related to faulty wiring. That vehicle is almost identical to the dodge grand caravan. Something need to be done before a loss of life incident...I reached out to Dodge to notify them, and an investigator sent me a same day email that Dodge is not responsible. I only had 65,000 miles on this 2014 DGC and took great care of it.

I bought this van the previous owner told me that the headrest deployed at an unknown time while the vehicle was parked and no one was in it. Since owning it a few months ago the other headrest deployed. This also happened while the van was parked in my driveway and no one was in it.

Both sliding door lock actuators buzzed and stopped functioning properly, causing them to need to be replaced. This has been difficult to have the doors close and get the kids in and out. Tpms failing, pressures correct on newer tires light keeps coming on.

On 7/1/2023 my vehicle began to malfunction while driving - power was flickering with EMC drive error flashing. When this happened, I was at a complete stop in traffic, so I put it in park, then restarted. Everything seemed to be fine, but I was cautious. The same thing happened again, so I pulled into a parking lot and went inside a store for a few minutes. I got back in the car, started the vehicle, then before pulling out into traffic, the center console reported a flashing error again, so I put it in park and restarted again, put my hazard lights on, then pulled away. I was consciously trying to stay a few car lengths behind vehicles ahead because I was nervous about what was going on and just wanted to keep away from other vehicles. I made it to an intersection where there was a red light with 2 cars ahead of me. I began braking and decelerated to around 5 MPH and within 30 feet of the vehicle in front of me when the vehicle lost power to the brake. I don’t recall if the steering also went out. I hit the bumper of the car in front of me, and her car was pushed into the car in front of her. Fortunately, I did not sustain any injuries. My airbags did not go off. My confidence in this equipment is seriously affected. How will I know this will not happen again? I could have died or taken the lives of other people. I requested that the company MobilityWorks not make any repairs to the vehicle until they explain the origin of the problem. Instead they replaced parts and stated

Driver's seat headrest suddenly deployed while driving. I contacted Dodge they haven't replied yet. From what I'm reading there are numerous vehicles with same issue. I was very startled, distracted from surprise and have a sore neck.

Symptoms and Unknown: There are numerous reports of the oil filter housing that is prone to cracking which causes the engines in 3.6 Pentastar V6 engines to leak oil. This engine issue affects the 2011 and newer Grand Caravans with the said engine. My sister has had to replace this part twice now (Every other year - She replaced the last one two years ago) in her Grand Caravan SE. The symptoms which leads us to know that this is the problem is the vehicle develops a really bad oil leak and the oil level ends up being a quart low. The same symptoms happened when the first part was replaced prior to the part that needs to be replaced now. My sister was informed that the part she put in last time is not a definite fix as the said part could malfunction and crack again. This is a defect of the design of this engine that needs to be recalled and replaced with the new part free of charge that is also an updated part that is different than older parts that will not break. This is a SAFETY HAZARD as my sister has two small children and they could wind up on the road or interstate broken down. RECALL or TSB (TECHNICAL SERVICE BULLETIN) is needed from Chrysler.

Passenger side front headrest deployed for no reason while vehicle was parked.

While driving with my three daughters in the car, the driver side headrest activated and hit me in the back of the head. This was random and no warning was given. As a result the headrest deployed and it cannot be fixed/put back. I currently have a strong headache and am still feeling a little dizzy after the incident.

Driving down highway. Rear spoiler flew off.

Like the other hundreds of thousands 3.6 Litre V6 engines with the notorious oil filter housing/oil cooler oil leak, mine just started at 120,000 miles. I'm the only person to ever change the engine oil and oil filter in this vehicle. I have always torqued the oil filter to spec (18 ft. lbs). There should be a recall on this faulty part.

My family and I were on a hike in the mountains, and when we got back to our car, the drivers headrest had popped out of itself. We tried to see if we could fix it, but it busted out with such force that it broke parts. When I looked this up, it appears that this is a pretty common occurrence with our make and model, and others have actually had it happen when they are in the seat and been injured! Thank God we weren’t driving through the mountains when it happened! This is a serious issue and I couldn’t believe that nothing had been done to create a recall, especially after finding a report about the same issue with the same vehicle online from October 2019 (almost FOUR years ago) by [XXX] , stating that the government was researching this issue after several people were injured. Apparently this is a safety feature that is built to prevent whiplash in an accident, but is activating without provocation in many vehicles, if it’s not offsetting whiplash, could this not CAUSE whiplash?! This needs to be addressed as a recall and fixed for others before it causes a serious car accident (if it hasn’t already) or kills someone. This is completely unacceptable. The stall forced the vehicle to lose power steering and it had to coast over to the shoulder to get out of traffic, this could’ve resulted in a crash or blocking traffic. This happened twice, once during a very hot day. The van was able to restart after about 30 minutes and finished the short distance to the destination. The second time was at night when temps had cooled 20 degrees and it stalled at approximately the same distance it had traveled the first time. That time it was not able to be restarted and had to be towed to a repair shop, where they tested and replaced the ignition module. No warning lights or indicators went off either time prior to the engine stall. The check engine light did not come on until the second time when it would not start again.

Airbag light came on passenger side on door trying to see if it was recalled at all. No accident in this van is it supposed to do that. Was being getting the van worked on.

The driver's side headrest popped forward. OK one day, then when I got in the car the next it was popped forward pushing it against the back of my neck (thinking it was 4/14/23). Looks like the plastic hooks, or plastic parts just broke. I'm 75 years old, 6"3" tall. Very uncomfortable. I am very happy that the thing didn't explode when I was driving the car. I thought my 2014 Grand Caravan was a safe car. Have less than 128,000 miles on the car. Took it to the local Dodge dealer in Whiteville today (4/21/2023), and was told by the service manager that I was responsible for fixing it (at minimum part would cost $135 + tax, then installation - OR MORE). Told it was a common occurrance. BUT IT WAS ON ME. I have not had any wrecks, speeding tickets, or any bump ups. Now I no longer feel safe in this car. NO WARNING LIGHTS CAME ON. I would like responsible parties from Chrysler Motorrs to inspect my car. Planning on zip tying the other headrests for fear of it blowing up too.

In October 2022 I was driving down a residential road and slowed to turn into a parking lot and all power was lost. Was able to safely maneuver car and stop in parking lot. Put car in park, turned off the ignition. Restarted car, seemed ok. Mechanic said nothing was wrong. Now it’s happened twice more (April 14th and 17th, 2023). Once when trying to accelerate and merge onto a highway, once when slowing down to turn into a driveway. Both times the van just seemed to shut down and lose power. Steering became very difficult. Both times van restarted and continued driving after the incident. Waiting on word from mechanic to see what may be wrong.

The vehicle has shut itself off multiple times. It is seemingly random and happens without warning. It has happened when driving at slow speeds but it has also happened when driving at speeds of up to 60 mph and the vehicle loses power. We need to maneuver through traffic while coasting to the side of the road with no power steering available until the car is restarted. The dash goes blank and the car is rolled to a stop. The key is turned off and back on and the car drives as if nothing is wrong. It has been brought to a mechanic and dealer. No one has successfully duplicated the issue and there is no code issued after it happens.
