2022 Toyota RAV4

Defect. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Rav4, Rav4 Hybrid, and Rav4 Prime vehicles. The front passenger seat may have been assembled with interference between internal parts that may cause the Occupant Classification System (OCS) sensor to incorrectly detect the occupant. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Risk. Incorrect detection of an occupant may result in improper air bag deployment during a crash, increasing the risk of injury.

Remedy. Dealers will inspect and repair the front passenger seat, as necessary, free of charge. Owner notification letters were mailed September 2, 2022. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 22TA08.

The high-voltage rear traction motor cable floor wire harness on my 2022 Toyota RAV4 Hybrid has experienced severe corrosion, which has severely degraded the high-voltage electrical shielding. This defect has created immediate electromagnetic interference (EMI), causing violent, deafening static noise on the AM radio ONLY when the vehicle is in motion (accelerating/driving). Toyota National Headquarters is well aware of this systemic defect under Customer Support Program (CSP 22TE09) and Limited Service Campaign (25TD01). However, the authorized dealership, Empire Toyota of [XXX], has repeatedly denied warranty coverage using unscientific diagnosis ("FM radio is fine, so the cable is fine") and refused to perform a proper road test or insulation resistance test. Furthermore, Toyota Brand Engagement Center (Corporate Case #XXX) has neglected my safety concerns and failed to enforce corporate policy on the uncooperative dealership INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The contact owns a 2022 Toyota Rav4. The contact stated that the infotainment system continuously rebooted at startup and while driving. Additionally, the contact stated that while reversing, the rear-view camera image was brief and intermittent. The vehicle was taken to the dealer for a diagnostic test; however, the error message "System Access Denied" was retrieved. The dealer determined that the infotainment system needed to be replaced. The manufacturer was notified of the failure by phone and confirmed the assessment of the dealer. The failure mileage was approximately 53,800.

Sunroof shattered after a loud bang noise. No vehicles around.

The contact’s uncle owns a 2022 Toyota Rav4. While the uncle was driving approximately 65 MPH, the vehicle suddenly stalled. During the failure, the instrument panel remained illuminated, but the vehicle failed to restart. The vehicle was later towed to the local mechanic who was unable to determine the cause of the failure. While with the local mechanic the vehicle was restarted and operated normally. The vehicle was immediately driven to the local dealer who was unable to determine the cause of the failure. The failure mileage was 10,000.

The contact owns a 2022 Toyota RAV4. The contact stated that while driving at an undisclosed speed, there was an abnormal sound coming from the front driver's side door. The contact inspected the vehicle and became aware that there was water in the door panel. In addition, the contact later became aware that there was water inside the passenger's side front door panel. The dealer was contacted by phone and made aware of the failure. The vehicle was taken to the dealer, but it was not inspected. The contact was informed that the failure was not covered under warranty.

Idling, received a warning of "engine stopped. steering power low". Trying to restart the car - it would start up then immediately shut down w/ the same error. Letting it sit for 15 minutes allowed it to reset and start again. It was raining at the time, wind shield wipers were on if that matters.

While driving at highway speeds, the moonroof suddenly and spontaneously exploded without being struck by any visible road debris or external objects. After safely pulling over and shutting the vehicle off to assess the damage, the car now refuses to start or crank. The electrical system and interior/dashboard lights still power on normally, but the ignition will not engage to drive the vehicle.

Had parking brake malfunction. The wheels locked up could not move my care either forward or backward this was the second time within 8 month period. the failure was due to the parking break actuator. First time was in January 2026 second time was August 28, 2026 the wheels locked both times and I need to tow the car for service. Each time It cost me arount $1750 to replace the actuator. My concern was that I could not move my car. Found out this is a recurring problem with Toyota. Not only is this a safety problem but an expensive repair. I also found out recently that the car cant be towed unless the rear times are lifted in the air. I am looking for a recall on these system.

A whining noise from a 2022 Toyota RAV4 LE transmission during acceleration often points

The check engine warning light was illuminated, the vehicle shuddered, and then stalled as the contact pulled into a parking lot. The failure had occurred on several different occasions. The contact attempted to restart the vehicle; however, the vehicle failed to return to normal operation. The vehicle was towed to a dealer where it was diagnosed with an inoperable port fuel injector. The vehicle was repaired; however, the failure persisted. The vehicle was taken to another dealer and was diagnosed with inoperable port fuel injectors. The contact was informed that the port fuel injectors needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 50,000.

The car is Rav4 2022 fuel: gas. Dashboard showing Headlight System malfunction, Pre-Collision System malfunction and RSA System Malfunction. The alarm beep and sign of malfunction keep popping up on dashboard. I took it to Toyota dealership to be inspected and they notified me that "The Front Recognition camera Watchdog/Safety MCU Failure." This front camera is connecting to headlight system, Pre-Collision system and RSA system that causing them to be malfunction while I am driving. With all those system malfunction in the car, I am driving with a huge risk. I am certain that not only I have those problems but some owner of RAV4 2022 or perhaps other year that has the same microcomputer for Front Recognition camera for Safety have the same failure that would trickle down to safety of others on the road. The car is only 4 years old. The Microcomputer in the front camera should have been functioning with no problem, unless the factory didn't calibrate well or install malfunction microcomputer for the car. The dealership estimated the price to change the microcomputer $ 2000 and labor $ 400 something. Toyota should have recall and reimburse this service to customers since these problems are factory problems.

The contact inspected the vehicle and became aware that there was smoke coming from the battery, and there was battery fluid leaking from the battery. The vehicle was towed to the dealer and was diagnosed with battery failure. The contact was informed that the battery needed to be replaced. The vehicle was being repaired. The contact stated that in May 2026, the vehicle was serviced for electrical issues with the Start/Stop feature, parking brake error message, and the AC display unit was replaced. The contact stated that other electrical issues occurred intermittently. The manufacturer was made aware of the failure but provided no assistance. The failure mileage was approximately 71,800.

Within the 1st year I purchased it new, I would get a warning that the RSA is malfunctioning. It seems to happen when I park in the sun and the windshield is very hot. The warning in my dash prevents me from seeing my other readings; mph, fuel, and I can't use my other safety features; LDA, PCS or cruise control. I've taken it in but then it does a reset and the technicians say there's nothing wrong. The warnings happen every month, weekly. I just got tired of taking it in to hear they don't see anything wrong!

The dealer was contacted and performed an investigation and determined that the accelerator pedal was depressed during the crash and the vehicle was driving 25 MPH; however, the contact stated that the brake pedal was depressed during the crash. The manufacturer was made aware of the failure. The failure mileage was approximately 62,000.

COMPONENT: ELECTRICAL SYSTEM / TELEMATICS (DCM)The vehicle suffers from a documented manufacturing defect involving the Data Communication Module (DCM/Telematics system, Calibration N01D14B4644). The module enters an infinite transmission loop when the vehicle is turned off, causing a severe parasitic battery drain that repeatedly drops the 12V auxiliary battery below operational voltage.This creates a severe safety hazard, causing total electrical failure and leaving the driver unexpectedly stranded without warning, presenting an immediate safety risk in extreme weather or unsafe locations.An official Toyota Techstream diagnostic scan on 07/08/2026 confirmed 7 active DTC faults in the telematics system. Authorized dealerships are refusing to perform the mandatory firmware updates outlined in manufacturer service bulletins (T-SB-0089-21), opting instead to clear error logs temporarily without repairing the underlying defect. The manufacturer has failed to issue a safety recall despite widespread knowledge of this safety defect.

The vehicle was taken to an independent mechanic, where the infotainment system was replaced. The manufacturer was contacted, but no assistance was provided. The failure mileage was approximately 36,000.

My 2022 Toyota RAV4 has experienced failures of the power door lock actuators on both front doors. Neither the driver's side nor the front passenger door consistently locks or unlocks using the key fob, interior power lock switch, or automatic locking system. This creates a significant safety concern because the doors may remain unlocked when the vehicle is parked, creating a security risk, or they may fail to unlock quickly during an emergency. In addition, occupants may believe the doors are secured when they are not. The problem has occurred on both front doors, indicating this is not an isolated component failure. The issue developed under normal use and was not caused by an accident, abuse, or modification to the vehicle. Multiple Toyota owners have reported similar failures involving door lock actuators in the same generation of vehicles, and there is pending litigation alleging Toyota has long been aware of this defect but has not issued a recall. I am submitting this complaint because I believe this defect presents a safety risk and warrants investigation by NHTSA

The automatic locking system stopped working for 2 of my doors. Front passenger and back driver sides no longer lock or unlock. This poses a serious risk considering I live near the ghetto and my car has already been broken into.

Our 3 yrs old + Toyota RAV4 Hybrid SUV with barely over 30,000 miles on it had 2 safety issues within 2 months of each other! First was the driver's side mirror- it was rattling, feeling like it might fall off! Seems common issue, perhaps due to cheap plastic parts inside. We had it fixed- Toyota Brand Engagement Center gave a 'Goodwill' gesture to cover us replacing it at Dealership, though it should never have happened on such a newer vehicle in the first place! Then not even 2 months later, a Warning flashed- "Pre Collision System Malfunction!" Also "Headlights" with same warning- "SEE DEALER NOW!" Toyota Dealer. They said Forward Recognition Camera had 'INTERNAL MALFUNCTION' and needed to be replaced. It was going to be quite expensive, but the camera part is not even available in foreseeable future as there's back order for hundreds and they're still in production! It seems this too is a common safety problem~ We can't understand why there's not a RECALL on this. Toyota took no responsibility for this issue, which is APPALLING as, like the first 'side driver's side mirror failure' was also a safety issue! The whole reason we bought this vehicle was for its safety features! Without the front facing camera that comes with the car in same compartment as rear view mirror resides, we now have NO Pre-Collision Warnings, NO Headlight safety feature, NO RSA (Road Sign Assist System). It's infuriating that Toyota, who obviously is making inferior safety products, does not offer t

After driving for approximately 30-minutes in a heavy rain i pulled over and the automatic engine shutoff feature activated while i was waiting to go inside for an appointment. When attempting to restart the car, the engine would start but then shut off showing a "low power steering" warning message on my dash. The problem persisted for approximately 15-20 minutes. Eventually after about 10 attempts to restart the engine, the car started back up and drove fine the rest of the day. I have heard of this happening to many other rav4s on the Autosafety.org website and it causes me concern, especially if this problems causes the car to stall in high speed traffic in the rain. It is my understanding that this maybe a problem with the Mass Airflow Sensor getting wet in heavy rainfall.

The panoramic moonroof on my 2022 Toyota RAV4 spontaneously shattered and exploded outward at approximately 2:09 PM on June 21, 2026 while my wife was driving and exiting the freeway. There was a loud bang, followed by glass shards ejecting primarily outward onto the roof with a pattern showing the glass failed/bent from the inside out. The outer glass panel broke into multiple large sections with a clean perimeter failure and no visible external impact point or road debris damage. This created an immediate safety hazard: loud distraction while driving at highway speeds, flying glass, and sudden exposure through the roof opening. No warning lights, messages, noises, or prior symptoms appeared. The moonroof had been used normally with no issues. The component (panoramic moonroof glass assembly) is available for inspection. I have a dealer appointment scheduled for tomorrow, June 22, 2026 at 9:45 AM for inspection. The vehicle has not yet been inspected by the dealer, manufacturer, insurance, or police. This matches numerous similar reports of spontaneous panoramic sunroof failures on RAV4 models.

TSB T-SB-0010-24 P0303 code 163,000 miles Toyota denied goodwill assistance Cost: $2,549.65 Check engine light came on with vehicle hesitation on acceleration almost caused to stall with reduced power while driving , the dealership diagnosed the issue as TSB T-SB-0010-24 with cylinder #3 misfire requiring exhaust valve lash adjuster to be replaced. Contacted Toyota corporate to request help with goodwill or discount , was denied , was told they couldn't help no reason given .This issue is a well know Toyota RAV4 defect and even though they claim it only happens when is cold this happened to my SUV at 60*F. Please help.

The drivers side front seat belt buckle will not latch. There is no dirt or other debris in the latch to stop it from functioning.

Please see attached complaint letter. Overall, this is related to the 2022 Toyota RAV4 Hybrid electronic hybrid system. This issue related to campaign 25TD01. The issue can cause the car to not start at any time because of the vehicle's start/stop hybrid system. If this happens in traffic, you could cause a safety issue that could lead to an accident and possibly fatalities.

I am submitting this complaint to report a known and widespread safety defect involving the door lock actuators on my Toyota RAV4. My vehicle’s door actuators have failed at approximately 39,000 miles, just outside the 3‑year/36,000‑mile warranty. This failure prevents the doors from locking and unlocking properly, which poses a significant safety hazard while driving, especially in emergency situations. When I brought the issue to Beechmont Toyota, the dealership refused to repair the defect under goodwill or warranty consideration. I spoke with Stacy Meyer, Assistant Service Manager [XXX] ), who stated that the refusal was directed by Rodney Wetenkamp, Service Director. Despite the well‑documented nature of this defect, the dealership declined to assist in any way. This is not an isolated incident. I was driving through pretty heavy rain for about 65 miles. When I got to my destination, I was idling in the car for a few minutes when the car started to shake. I turned the car off. Upon turning it back on the engine would start and the car would shake before getting a message “engine stopped. Power steering low.” After two hours, I was able to turn the car on and move a little bit but the car was lurching and the RPM needle was rapidly bouncing between 1,000 and 2,000.

The steering will drift or pull to the side while driving down the road.

Short in the steering wheel heater mat leads to localized hot spot of greater than or equal to 134° F. Risk of burn injury to customer and risk of electrical fire.

The sunroof shattered while I was driving. There were no other vehicles around me or on the street. I heard a loud bang and glass rained down on me. The glass is pitched upward like it exploded from the inside. There is no other damage to my car. I had no warning other than the loud bang. When I am not out, my car is in a garage it never sits outside.

The contact is an attorney concerned about a crash involving a 2022 Toyota Rav4. The attorney stated that the vehicle had a comma.ai device installed on the vehicle. The attorney stated that while 2 couples were occupying the vehicle and driving at night on an expressway, there was a police vehicle on the side of the road responding to a separate vehicle crash. The driver of the vehicle may have been asleep, along with the front passenger, while relying on the comma.ai device to operate the vehicle. The vehicle failed to engage the brakes in order to avoid crashing into the police vehicle, and as a result, the Toyota Rav4 crashed into the rear of the police vehicle. The vehicle speed was unknown at the time of the crash. The owner of the vehicle rear-ended a Louisiana state trooper. Air bags deployed in the front and on both sides of the vehicle. The vehicle was towed to a tow yard and then to a Progressive Insurance lot. No warning lights were noticed. All occupants were wearing seat belts. The driver of the vehicle sustained concussion injuries, and the front passenger occupant sustained arm and leg injuries. The two rear occupants succumbed to their injuries at the hospital. No other injuries were reported as the police officer was not occupying the vehicle during the impact. The comma.ai device failed to avoid the f vehicle in front. The manufacturer was not notified of the failure. The failure mileage was unknown.

After turning my car on and the car running for several minutes, the engine shut off on its own and it said “engine stopped steering power low”. This is the second time this has happened. The vehicle is available for inspection. My safety was put at risk because I am concerned about this happening while driving, or being stranded somewhere. I have not taken my vehicle in yet, but many other RAV4 owners have this issue from my internet searches. This has happened twice to me in the past two weeks. The vehicle has not been inspected. There were no warning lamps/messages/lights prior to failure.

My infotainment screen went black while I was driving at night and it will not come back on. There was no warning. I took it to the dealer and they said that the system had failed. I saw on line that there was a recall for this very issue but my car is a '22 and not included in the recall, but I think it should be. This poses a safety hazard and I don't believe that this should happen under normal circumstances for a car of its age and relatively low mileage. I think this model year should be included in the recall and that Toyota should pay for the repair, which is quite expensive. The receipt on the diagnostic says the radio works, but it does not. It goes on when you push the power and immediately goes off.

We were traveling across country from Michigan to Arizona. We were 50 miles outside Tucson, AZ on [XXX]. Traveling 80 mph in a 75 mph zone, in passing lane and felt like car exploded and all airbags deployed and we came to a quick stop. No major injuries to either of us. One vehicle incident, did not collide with anything or hit any debris. It was a clear day, road conditions good, no noises, lights etc. My husband was able to keep from rolling and we came to a stop with many semi's etc flying by, very scary. When we were able to safely get out and call 911, there was significant drivers side front end damage around tire area and inside near axle. Looked like something broke inside car, sheered off bottom of tire. Police arrived and had to push vehicle on to shoulder of road. He assessed scene and thought it had to be a mechanicla failure as there was no other vehicle or debris on site. We did contact our insurance company and subsequently it was totaled. We also have Toyota investigating what the cause was if it was a faulty part or mechanical failure. We have not heard back but did receive the "recording of time of crash". I was driving home in the rain for about 75 minutes and when I arrived in my driveway I saw steam coming from the front grille area. It wasn't due to a high engine coolant temperature so I decided to carefully open the hood and check for the source. I saw water inside the bottom of the engine bay behind the radiator steaming from the heat of the engine. A few seconds later, the engine stalled and the following message appeared: "Engine Stopped, Steering Power Low" which came on with a check engine light, red battery light, and steering light. I used my scan tool and obtained the code: P0171, System Too Lean. After doing research, I've come up with a conclusion to the potential cause. Starting with the 2022 model year RAV4 non-hybrid (and onwards to the 2025 model year RAV4), the air intake inlet was redesigned from the prior one in the 2019-2021 RAV4 non-hybrid models, as seen in numerous comparison images online. This new design presents a dangerous fault. When driving in rainy conditions, especially at higher speeds with mist kicked up from cars in front, the air intake will take in water. The water will flow to the air filter and then reach the Mass Air Flow sensor (MAF). The MAF sensor monitors the amount of air entering the engine and sends signals to the car's computer to help determine how much fuel to send to the engine to maintain an ideal ratio for optimal efficiency. Driving up a slope of about 20-30 degrees gradient of a straight lane going into a private parking lot. Braked and decided to make a 3-point turn to get back out. There was no traffic in the lane. Put gear on reverse while on the slope with the vehicle pointing in the up direction of the slope. Vehicle suddenly surge backwards, accelerating. Vehicle sped down the slope and crossed a street. Could not stop. Vehicle was stopped only after placing gear into park from reverse, and after jumping curb across the street. Fortunately, there were no cross traffic in either direction on the street, otherwise vehicle could have been T-boned. Tire wall on the rear passenger side busted. AAA was called to replace tire with spare. There were no warnings on the dashboard or any symptoms anywhere else, although the vehicle had all the safety features.. The next morning, vehicle was taken to Toyota of Carlsbad. Left there for diagnosis and tires change. Vehicle is left with Toyota of Carlsbad until this issue is answered/solved. Told by service manager of Toyota of Carlsbad to call Toyota USA at [XXX] to report incident by customer, and request Toyota USA to send expert from Toyota to diagnose issue. I attempted to remote start my 2022 Toyota RAV4 Hybrid from inside my home using the Toyota mobile app. I received a confirmation notification stating that the vehicle had started and would automatically shut off in 10 minutes. Approximately six minutes later, I exited my home and approached the vehicle, which was not running. After unlocking and entering the vehicle, the interior temperature confirmed that the vehicle had not been running. I attempted to start the vehicle by pressing the brake pedal and pushing the start button. No dashboard lights illuminated, no warning messages appeared, and the vehicle was completely unresponsive. I attempted this process two additional times with the same result. After exiting the vehicle and closing the driver’s door, I attempted to lock the vehicle using the touch sensor on the driver-side door handle. The vehicle would not lock. I attempted this a second time and it still would not lock, indicating a complete loss of electrical power. This failure occurred in cold weather conditions and involved the hybrid battery and associated electrical system. The loss of power created a safety risk because the vehicle could not be secured and could have left me stranded in cold weather. Later that day, I contacted my insurance company for roadside assistance. My steering wheel has been peeling/flaking off and it’s only 3 years old and my USB plug in inconsistent and turns off or doesn’t work will cords.

The infotainment screen and its buttons do not work. All I see when parked or driving is an unresponsive black screen. There was no warning prior to the screen not functioning.

The contact stated that while his daughter was driving at an undisclosed speed, the brake pedal was depressed, and the front driver's and rear passenger's seat belt buckles and the other rear seat belt buckle, where a child was seated, failed to latch securely, causing his grandchild's rear passenger's side car seat to become loose. No warning light was illuminated. The dealer was made aware of the failure and informed the contact that a diagnostic test was needed. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The failure mileage was unknown.

The Panoramic Sunroof Shattered on its Own out of nowhere. It just made loud noise. we thought it was tire, but car did not make move, looking above we saw the panoramic section of sunroof shattered. I went to dealer, they said there is a point of contact (means something has hit the roof) and they told me i have to file with insurance company. Insurance company is asking a million questions. It seems they are both passing it to each other. TO my opinion it is manufacturing flaw. very unsafe!

It was a rainy day and I had come home to pick up someone, didn’t turn my car off but had it in park. I went to reverse and pressed on gas, all of a sudden my engine started to aggressively shake/vibrate while showing on the dash that power steering was off, I couldn’t move my car anymore. I turned it off and same thing. Finally after 15 minutes, it started back up like normal. Scary to think others have been experiencing this with or without rain involved, and also while driving. Toyota is suppose to be reliable. Taking it in to Toyota service department tomorrow, but scared to drive it and just be told nothing is wrong like others have been told.

At the car wash while slowly pulling (1mph) into a parking space to vacuum the vehicle the car unexpectedly accelerated. The brakes failed and the car continued to accelerate until forced into park and turning off the vehicle. During the acceleration there was a grinding noise. This acceleration caused the car to hit the front curb and cement pilings with poles supporting the roof and vacuum. There were no warnings or lights being presented. If not for the curb and being off the road anyone in the car would at the very least been injured and possibly hitting another auto. The dealer is claiming no none issues as described. The auto was towed to the body shop to inspect and repair. We were told by the body repair shop the front suspension failed. The auto was great prior to this incident. Afraid to drive for fear of a repeat especially due to others on the internet claiming the same issue has happened and often continued to have a repeat acceleration incident. No one has found resolution from Toyota. Approximately $7,000.00 worth of damage and our insurance has now gone up.

Electric Power Steering Low indicator displayed while driving. It re-appeared several times over the past days. Called dealer and awaiting their response.

After driving for 35 minutes (in the rain), I pulled up to work and let someone out. While car was sitting there in park, it bucked in the back, I then put it into park and almost immediately, it shut off with this on the display "Engine Off, Steering Power Low, Do Not Drive". My lights and windshield wipers remained on. I tried to turn it back and and it shut off again. I waited a few minutes, turned it on and went to pull out onto main road and the car shut off again with the same message. Thankfully I was turning onto the main road so close to shoulder and able to keep my car pretty close to side of the road. I called the Toyota dealer who told me not to drive it and it would need to be towed. I towed it to the closest dealer and when they checked the codes (the next day), they said there were no codes and they could not replicate the problem. As a note, the rain was off and on, nothing that was worse than anything I have driven in previously. I drive main roads and did not go through any large puddles or anything similar. This is a major safety issue and I am terrified that it could happen while driving on a highway where I may not have time to get out of the way of other traffic (car literally stopped with no option of continuing to coast). I am also shocked that there were NO codes triggered as there was obviously some event and that the car is only one month outside of warranty.
