While driving on the highway with Full Self-Driving (Supervised) engaged, the vehicle approached an exit that had been closed with traffic cones and road-closure signage. The vehicle failed to properly respond to the closure, continued through and struck multiple traffic cones, and ultimately collided with the road-closure equipment, causing significant front-end damage. My primary safety concern is that multiple safety systems appeared to provide no effective intervention. I observed no Forward Collision Warning, no Automatic Emergency Braking, and no effective automatic braking or avoidance maneuver, even after the vehicle had already begun striking traffic cones. There were no warning messages or indications of a system malfunction before the collision. This was a serious safety incident involving FSD, Forward Collision Warning, Automatic Emergency Braking, and the vehicle's obstacle-detection/collision-avoidance systems. I am requesting that Tesla immediately preserve and review all available vehicle data, including FSD status, camera/perception data, detected objects, vehicle speed, steering, braking commands, Forward Collision Warning events, Automatic Emergency Braking activity, and driver inputs before and during the collision. At only 27k miles in with no accident history and perfect maintenance my camera system began to fail. It started with a delay of the backup camera and then turned into the cameras no longer working and no longer talking to the satellites. This then became a gps issue. Tesla despite me being 1 month outside warranty and well below the mileage charged me for a $4k new computer without diagnosing the failure as to whether it was heat related or what. It said the FSD computer was failing to make connection with the other computer. I had never even used FSD. This Premature failure of the computer system should’ve never been charged to me.

My 2022 Tesla Model Y does not reliably activate the rear air-conditioning vents when a baby is properly restrained in a rear car seat. When the climate system is set to Auto, the rear vents remain off because the vehicle does not detect the baby/car seat as an occupant. When an adult occupies the same rear seat, the vehicle detects the passenger and the rear vents activate automatically. This creates a significant safety concern because the driver can have the climate system running normally and the front of the cabin can be cool, while very little conditioned air is being directed to the rear where the baby is seated. On multiple occasions, my baby has been visibly hot and sweating in the rear car seat even though the front of the cabin felt cool and the front climate system was actively running. The problem is particularly concerning because a baby cannot communicate that they are overheating, and there is no obvious warning to the driver that the rear vents have remained off. A driver could reasonably assume that setting the vehicle’s climate control to Auto is appropriately cooling the occupied cabin. I have been able to reproduce and isolate the issue. Applying approximately 20 pounds of pressure to a particular area of the rear seat cushion causes the vehicle to recognize the seat as occupied and the rear climate vents to activate. Child entered our vehicle and was UNABLE to exit/open his door (driver's side passenger door) from inside. The child locks were not enabled. The door would also NOT open from the outside. When entering service mode, a VCLEFT_a187latchReleaseFailedR error was present. I looked up on Tesla service how to manual release the door. It states to use a fingernail or flat tool to open the plastic flap in the pocket of the door handle. I was unable to open with a my fingernail and using a small screwdriver took me multiple attempts. It then took me multiple attempts (~5) of pulling the release cable for the latch to unlock. I believe I finally got it to unlock while holding the electronic latch while pulling --this is not mentioned in the service manual (unsure if this step is missing or it just was extremely stuck/luck that it finally unlatched when I did this). Once the latch was released, this error and all history of it was not even logged in the service alerts section. Made a service appointment, however, first available is not for >20 days. I also have a video of us trying to open the door multiple times but I can not upload it here.
While operating a 2022 Tesla Model Y with Full Self-Driving (FSD Supervised) engaged, the vehicle failed to execute a right merge onto the CA-280 North freeway ramp in Redwood City, CA. Instead, FSD initiated an unexpected wrong-way maneuver down a downward off-ramp. As the vehicle headed toward a sharp left curve, it failed to decelerate sufficiently for the turn. The vehicle maintained speed on the downward incline and then accelerated slightly, traveling at around 30 mph immediately prior to impact. Facing an active hazard initiated by the software, I performed an emergency manual override. System steering resistance and abrupt load transfer mid-turn caused an immediate loss of control, driving the vehicle over a mound and into an off-road ditch. Airbags deployed upon impact, and occupants (including a 5-month-old infant) were transported to the emergency room for evaluation. A formal safety complaint is being filed regarding FSD wrong-way navigation failures, failure to decelerate, and severe handling/disengagement instability during emergency takeover scenarios.

FSD (Supervised) was engaged while the vehicle was traveling toward a freeway entrance. The vehicle was expected to turn right, but instead began veering left. The driver applied the brake in an attempt to slow the vehicle and disengage FSD. According to the driver’s recollection, the vehicle did not respond as expected and continued accelerating until it left the roadway and crashed into a ditch. The incident created a serious safety risk to the driver and an infant passenger. Emergency services responded, and the driver and infant passenger were transported to an emergency department for evaluation. The vehicle was subsequently towed from the scene. The suspected malfunction involves FSD/vehicle control behavior, including whether the system properly responded to brake input and whether commanded acceleration continued after the driver attempted to intervene. The vehicle and its electronic systems should currently be available for inspection, and a request has been made to preserve relevant vehicle telemetry, driver-input data, FSD engagement status, accelerator and brake inputs, commanded motor torque, speed, steering inputs, camera footage, Event Data Recorder information, diagnostic logs, and software/FSD version information. A police report has been filed and both Tesla and Tesla insurance have been contacted.

On 8/22/2026, my 2022 Tesla Model Y Long Range with approximately 81,000 miles developed an Electronic Power Assisted Steering (EPAS) fault immediately after I felt a brief jerk while reversing. The vehicle displayed “Steering assist reduced / Steering may require increased effort.” Manual steering currently feels normal, but Full Self-Driving/Autosteer and Lane Departure/Lane Keeping functions became unavailable. Tesla Service Mode shows repeated faults EPAS3P_a072_motFeedFwdError, EPAS3P_a049_piCtrlError, and UI_a020_SteeringReduced. For EPAS3P_a072, Tesla’s diagnostic screen describes a “Motor control feed-forward correlation error,” lists the potential impact as “Reduced steering assist and EAC inhibited,” and specifically lists “Replace EPAS” as the clear condition. The faults return after vehicle power-off and reboot. The vehicle was updated to software 2026.21.6 / FSD v14 Lite approximately two weeks before this occurred. There were no previous steering warnings. I am not claiming the update caused the failure, but I believe the timing warrants investigation. NHTSA Complaint 11749826 describes a 2021 Model Y with the same EPAS3P_a072 and UI_a020 faults, including Tesla Service Mode stating “Replace EPAS.” Complaint 11758026 describes another 2021 Model Y steering gear failure and requests expansion of Recall 25V-092 to earlier Model Ys. The contact owns a 2022 Tesla Model Y equipped with Michelin Tires, Tire Line: Michelin Pilot Sport, Tire Size: 255/35/ZR21, DOT Number: (N/A). The contact stated that while driving 65 MPH, the rear driver’s side tire experienced a blowout. The contact was able to safely pull off the roadway and wait for towing assistance. The local dealer was contacted. The vehicle was towed to American Depot Tires. The contact stated that all four tires were replaced, and the vehicle was repaired. The contact stated that the inner tread of the front tires were prematurely worn, and the vehicle required an alignment every three months instead of every six months. The manufacturer was notified of the failure and informed the contact to take the vehicle to the dealer and have the vehicle repaired out of pocket. The failure mileage was approximately 99,482.

On Aug. 9, 2026, my 2022 Tesla Model Y experienced a sudden low-voltage electrical failure that completely immobilized the vehicle. Earlier that morning, the Tesla app sent a push notification indicating low-voltage battery service was needed. I tapped it, but it provided no actionable warning. I immediately checked Maintenance. At 9:06 AM, the app displayed “0 Recommended” and “No maintenance is recommended for your vehicle at this time.” Less than an hour later, the vehicle completely failed. Nothing functioned. We could not open the doors or trunk, access the vehicle normally, or operate vehicle functions. The vehicle could not be put into a condition for normal towing; it would have had to be dragged/pulled onto the tow truck. Tesla later documented “Electrical system is unable to support all features” and is replacing the 12V battery. This may be relevant to NHTSA PE25010.

--torn bushings on both the lateral and compliance links --my understand is that torn bushings, if they completely failed, could cause a wheel to lose its rigid control causing the control arm to shift wildly, causing severe steering wander, loud clunking noises over bumps, unstable braking, and rapid, uneven tire wear. In worst-case scenarios, it can lead to a sudden loss of vehicle control and at worst cause an accident --also, this can also cause alignment issues and excessive tire wear--in my case, the torn bushings caused the front tires to chunk --three dealerships, including Tesla, confirmed the issue, including that it likely impacted the alignment and tires --the earliest symptoms were uneven tire wear: 1) all four tires from my first set were chunking and had to be replaced; 2) the front two exhibited the same chunking and have yet to be replaced
The Autopilot/FSD computer (driver-assistance ECU) on my 2021 Model Y (HW3) began failing intermittently on August 2, 2026, days after installing a software update -FSD v14 "Lite" branch. The component is in the vehicle and available for inspection. While driving at highway speed in high ambient temperatures (112–125°F), the system repeatedly disengaged with "Cruise control unavailable" (alert DI_a175) and "System error — take over immediately" warnings, and later alert APP_w46u on FSD engagement. Failures included: FSD and Autosteer aborting within seconds of engagement or disappearing from the menu entirely; complete loss of driving visualizations (no lane lines or vehicle overlays); rear camera blacking out; loss of speed-limit recognition and automatic turn-signal cancel. Both FSD and Autosteer Beta failed identically, indicating the fault is in the shared Autopilot computer/camera pipeline, not one feature. Because the same camera/compute stack supports automatic emergency braking, forward-collision warning, and blind-spot warning, the status of those safety systems during the failures is UNKNOWN — I had to complete long desert highway stretches fully manually, with repeated unrequested hand-backs of control creating a sudden-takeover risk. The fault is thermal-load related and intermittent: it recurred at high ambient temperatures, sometimes recovered after a rest or power-off reset, but on August 4 one full power-off reset at 88°F ambient failed to restore the system (

I am reporting a serious safety issue involving severe, difficult-to-detect inner-edge tire wear on my Tesla that resulted in a tire failure on Highway 101. The tire failed while I was driving on the highway, creating a dangerous situation and significant traffic disruption. After the failure, I discovered severe wear concentrated on the INSIDE edge of the tire. The concerning part is that the normally visible tread appeared serviceable. An owner can therefore believe the tire is safe while the difficult-to-see inner portion may be dangerously worn. I am concerned that Tesla’s suspension/alignment geometry, including camber/toe settings, vehicle weight, or related components may contribute to excessive inner-edge tire wear. NHTSA should investigate whether this is occurring on other Tesla vehicles and whether owners are being adequately warned to inspect the entire tire, particularly the hidden inner shoulder. I also experienced a very concerning mileage discrepancy. My vehicle showed approximately 36,000 miles before traveling to Tesla for service. During the trip to the dealership, the mileage associated with my vehicle appeared to increase to approximately 56,000 miles. Tesla subsequently referenced the higher mileage in connection with service/maintenance coverage.
