On October 26, 2026, Tesla FSD Supervised entered an active school zone (20 mph, flashing yellow lights) at ~54 mph and did not slow. I disengaged and braked manually. The same trip, FSD could not position the vehicle in the school car-rider line, forcing another full manual takeover.Failed system: FSD Supervised speed control and path planning. Vehicle/software available for inspection.Safety risk: More than double the posted school-zone speed with flashing lights endangered children, crossing guards, and other road users. Manual intervention was also required in a congested pickup area.Reproduction: Matches public reports and NHTSA defect petition DP26010. Not yet inspected by Tesla, police, or insurance.Warnings: None. No lights, messages, or alerts before the failures.Assessment: FSD does not reliably detect or honor active school zones or common pickup-line geometry. The streak system penalizes these necessary interventions, so the only practical way to maintain a long streak is to avoid real-world driving (school zones, local roads, parent drop-off) and stick to interstate miles—the opposite of the “handles everyday driving” claim. This design forces parents to choose between safety and the score.

No unusual pattern of severe incidents in the complaint record.
Complaint counts track how many cars were sold and how long they have been on the road as much as they track quality. A common model from 2013 has had a decade to accumulate reports; a rare one from last year has not. The signal worth weighing is in the severity columns, in whether the same component keeps appearing, and in whether the manufacturer eventually opened a recall for it.
