The rearview camera supplied in the Rear Video Camera and Prep Kit may not function when installed by the final stage manufacturer (upfitter), resulting in a blank rearview image.
Risk. A camera that does not display a rearview image while reversing can reduce the driver's view of what is behind the vehicle, increasing the risk of a crash.
Remedy. Dealers will update the audio control module software, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Ford customer service at 1-866-436-7332. Defect. Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.
Risk. An unsecured wheelchair can move during transit, increasing the risk of injury.

235/65R16C This vehicle was a rental. During our trip, the right rear/passenger-side rear tire repeatedly lost air pressure, requiring multiple stops to add air. The tire ultimately went completely flat at night while we were traveling on the highway with four young children in the vehicle. We were stranded along the roadway while traffic, including large semi-trucks, passed at highway speeds, creating a serious safety risk to the occupants. After the tire failed, we observed what appeared to be an existing tire plug/repair near the outer shoulder of the tread. Photographs taken during the incident show the location and condition of this area. The tire had been repeatedly losing air before becoming completely flat. We attempted to use the vehicle's spare tire but discovered that the rental vehicle did not contain the tire iron/lug wrench necessary to remove the wheel. Although a spare tire was present, we therefore could not install it and remained stranded. The spare even had a nail and glass in it. During the trip, the vehicle also exhibited abnormal noise/vibration that appeared consistent with a possible wheel or wheel-bearing issue. I cannot independently confirm the mechanical cause. The rental company was notified of these problems. The vehicle was subsequently returned and is no longer in my possession, but it should be available to the rental company for inspection. The failure involves the vehicle’s surround-view (360-degree) camera system, specifically the side camera feed and/or image stitching/display system. When the camera system is activated, one side of the 360-degree view intermittently fails and is replaced by a solid gray box instead of a live image, as shown in the attached photos. This suggests a fault in the side camera, wiring, or software processing. Ford has issued a recall related to the camera system; however, this issue persists in this vehicle despite the action the dealer has taken relating to the recall. The vehicle is available for inspection upon request. This malfunction creates a blind spot on one side of the vehicle during parking or low-speed maneuvering. The system is intended to provide full visibility around the vehicle to detect pedestrians, cyclists, vehicles, curbs, and other obstacles. With one side missing, there is an increased risk of striking objects or people, especially in tight or urban environments, and the system may misleadingly appear functional. This becomes a big issue in the winter when you need to keep the windshield clear of frost on the outside and inside. When looking online I found that I am not the only one having this issue.

Safety Component Affected: BACK OVER PREVENTION: SENSING SYSTEM: CAMERA Description of the Safety Defect: The rearview camera system on my 2022 Ford Transit fails completely, displaying either a distorted image, a completely blank blue screen, or a black screen when the vehicle is shifted into reverse. This total loss of rear visibility creates a severe blind-spot hazard and an immediate risk of a back-over crash, especially given the large commercial dimensions of this van. History of Failed Manufacturer Remedies: This vehicle has been brought to an authorized Ford dealership seven (7) separate times for this exact rearview camera failure. The manufacturer has repeatedly failed to provide a permanent remedy. The dealership attempted repairs under Ford’s active safety campaigns (including Recall 23S48 / 25S38 regarding the Image Processing Module and rear wiring). Despite these recall repair attempts, software flashes, and component replacements, the system fails again immediately after every visit. The camera is currently non-functional. The manufacturer’s authorized dealer network cannot fix this federally mandated safety feature (FMVSS 111), leaving the vehicle inherently unsafe to operate in reverse. I am filing this complaint to document a failed recall remedy and a persistent, unresolvable safety defect.

I’ve been dealing with some intermittent problems with my 2022 transit for a couple years now. Basically I’ll be driving on the highway and all the sudden I’ll lose gas control, and power braking and the dash displays a message that says “shift to neutral to restart vehicle”, battery icon flashes red and I have to coast to the side of the highway. The electronics in the car will still be working when I pull over but the battery is dead. If I shift to park my engine doesn’t turn over or make any clicks at all. The few times it’s happened it eventually will turn back on after 10 min or so after using a jumper pack. I’ve taken it to dealers and they all say the computer isn’t throwing any codes so they can’t help. I’ve nearly been in 2 accidents now on the highway trying to get to a shoulder/lack of shoulder and the fact that I lose my brakes makes this an undrivable vehicle. My vehicle is still under manufacturer warranty but the ford dealers have not been able to help and keep sending me on my way with a very dangerous vehicle. I need help, I’m concerned about being involved in a a dangerous accident that could hurt myself or others when I lose brakes and gas. This has happened 5times now over the last 25k miles. I have a long video of the last incident that I would love to share if a video upload link is made available.

We are reaching out regarding Safety Recall Notice 23S48. This recall pertains to this issue: "when the vehicle is placed in reverse, or when the 360-Degree view is selected, you may intermittently see a rear camera blue or black image on the SYNC® screen." After bringing our vehicle to the Ford dealership, they have replaced the camera as the recall specifies. However, the issue described continues to persist. Ford is telling us than any further work towards fixing this issue would be at our expense.

Since I purchased this vehicle, it has experienced severe safety-related failures in the electrical system, battery, vacuum pump, powertrain, and other components. All warning lights would activate on the dashboard, messages appeared such as “Service Immediately” and “Power Reduced,” the vehicle became limited to about 10 mph, and on several occasions it shut off completely while in use. Six days before taking it to Midway Ford, Metro Ford replaced the battery, but within days the vehicle became inoperable again. Midway Ford kept the vehicle for weeks and eventually claimed the battery was defective, but attempted to charge me over $2,000 for sensors and a fuse box despite my Ford 5-year/8-year warranties and an extended service contract. I opened formal complaints with Ford, Metro Ford, and Midway Ford. Ford acknowledged factory defects, and the vehicle has more than six prior repairs since 2022, consistent with known issues in similar models (vacuum pump failures). During active state and federal investigations—and without my authorization—Midway Ford moved the vehicle off their property. I have evidence from the Ford app showing it was relocated to an apartment complex, then a private house, and finally to an unauthorized tow yard. When I returned to the country, police accompanied me to the location where I found the vehicle hidden, vandalized, with deflated tires, scratches, and surrounded by stripped and crashed cars. My 2022 Ford E-Transit has experienced repeated electrical, propulsion, and powertrain failures that cause the vehicle to become completely inoperable. On September 8, the van displayed multiple critical alerts including powertrain malfunction, brake system failure, vacuum pump failure, and full electrical system fault, and it would not start. The next day, the same failure occurred again and the van had to be towed. The dealer replaced the 12V battery without proper diagnosis and returned the vehicle with all warning lights still on. A technician privately stated that the issue was more complex than the battery and that the system had only been reset, not repaired. No written authorization for the repair was requested, and the underlying electrical/propulsion failure was never addressed. Five days later, the vehicle became fully inoperable again with the exact same failures. These repeated shutdowns and inability to start happen without warning and create a serious safety hazard because the van can lose all function while parked or potentially while driving. The system faults affect essential components related to braking, propulsion, and overall vehicle control. The pattern indicates a significant defect in the electrical and propulsion systems of the vehicle, and the problem persists despite attempted service. From the moment I purchased the vehicle, HGreg Nissan withheld essential information and misled me about its condition and warranty status. They told me the vehicle had a “bumper-to-bumper” warranty valid until October 2025 because it was a 2022 model, but they never informed me about the actual manufacturer warranties (5-year and 8-year EV warranties), which remain active and override any extended warranty they sold me. To justify selling me a six-year extended warranty through NESNA, HGreg Nissan hid the fact that the manufacturer warranties were still in effect. They also added three additional warranties I explicitly said I did not want. All documents were signed on a tablet, and I was never given physical copies. The USB they provided did not include any warranty information. I repeatedly requested the warranty documents and received no response. NESNA later confirmed that the extended warranty could not be activated because the manufacturer warranty was still active. HGreg Nissan also failed to disclose that the vehicle had been part of a commercial fleet in California, was auctioned, retitled, and then sold to me as if it were a clean personal-use vehicle. Since day one, the vehicle has experienced serious and repeated failures: electrical system malfunctions, powertrain warnings, brake system faults, vacuum pump failures, sudden loss of power, reduced speed mode, and spontaneous shutdowns. My 2022 Ford E-Transit 350 has had repeated safety and performance failures. The vehicle has an open recall on the rearview camera, which caused a police incident when I hit another car because the camera was not working properly. On several occasions, the vehicle has also completely shut down and would not start at all, leaving me stranded. The van often loses acceleration, gets stuck at 20 mph, and becomes unsafe to drive. The brakes, electrical system, and propulsion system have all malfunctioned. Despite multiple visits to Ford dealers, the problems remain. I have had to rent vehicles at my own expense since no loaner was provided. These failures put my safety and the safety of others at serious risk. I have police reports, medical certificates (including 12 days of medical leave), dealer invoices, and photos fully documenting everything.

I went to dealer to dealer and they told me they could not fix it because there is no remedy not yet available. They camera is also working fine, no issue at all. I use this vehicle for a business and BECAUSE of the recall, I am not allowed to use this vehicle for a commercial use. The engine 'start-stop' feature is permanently 'ON' . The ability to temporarily disable it has been removed by a Ford software update. Case #00715259 was opened with Ford, vehicle taken to Capitol Ford, Carson City NV, for verification. Dealer verifies that although the start stop feature works correctly the option to temporarily disable it was removed by a Ford software update in August 2023 with no plans to restore it. In mountain driving not being able to disengage this feature is dangerous. Attempting to enter an intersection with a loaded vehicle and having the engine stop just before accelerating will eventually cause an accident. The option to temporarily disable 'start stop' must be restored before someone loses their life at a busy intersection.

This is a fleet vehicle operated by Walmart and managed by Wheels Inc Fleet Management. Vehicle experienced loss of brake effectiveness due to brake vacuum pump failure. As operator described: "CUSTOMER STATES BRAKE LIGHT ERROR POPPED UP ON DASH AND BRAKES BECAME SPONGY AND VEHICLE FELT UNSAFE TO DRIVE... VEHICLE WAS TOWED IN." There is a recall 23S48 to replace the rear camera. My 2022 Ford Transit has a 360 camera and only the rear camera was replaced with the recall. My side camera is also having intermittent issues where it shows a grey/blue screen when the vehicle is placed in reverse. Ford needs to replace all three cameras for the 360 system and not just the rear. This is still a safety issue when you cannot see vehicles on your side when reversing.
