While driving at highway speed in 6th gear, the vehicle unexpectedly downshifted into what appeared to be 1st gear without warning. Immediately afterward, the vehicle falsely indicated that it was in Reverse while still moving forward, and the backup camera activated during travel. The sudden downshift caused abrupt deceleration and created a significant safety concern due to the risk of loss of vehicle control and the possibility of being rear-ended by surrounding traffic. The issue has occurred multiple times and temporarily reset after shutting the vehicle off and restarting it. During the most recent incident, the vehicle also experienced a delayed restart. The vehicle was inspected by a Ford dealership, which retrieved diagnostic trouble codes P0706, P0707, and P1702 related to the transmission range sensor system. The dealership diagnosed failure of the molded leadframe/internal harness in the transmission main control, causing transmission range sensor faults and erratic shifting behavior. The dealership advised that the repair would cost approximately $4,000 and stated that the repair is not currently covered under the active Ford transmission recall. Prior to the failure, symptoms included intermittent erratic shifting behavior and false gear indication. No collision occurred, but the vehicle behavior created an unsafe driving condition at highway speeds. The failed component should be available for inspection through the servicing Ford dealership.

For the past several weeks, I have experienced an ongoing issue with my truck’s transmission shifting unpredictably. At random times, it becomes stuck in third gear, and while driving it has downshifted from fifth to third and then to first without warning. Even when stopped at a red light, the vehicle will sometimes remain in third gear. These incidents have created multiple safety concerns while driving and stopping. On 05/11/2026, my truck stopped completely in the middle of the road; fortunately, other drivers were able to stop and I was not hit, but I am concerned it is only a matter of time before a serious accident occurs. I also recently received a recall notice, which I was not expecting, and I had assumed this repair would be an out-of-pocket expense. I have contacted Ford regarding the recall repair, but I was advised they do not anticipate a remedy until July. Waiting several more months to drive a vehicle with ongoing drivetrain and downshifting problems feels unreasonable, and I am also concerned the condition may be causing additional damage to the transmission.

I am submitting this complaint regarding Ford Recall 26S28 involving my 2015 Ford F-150. Ford has acknowledged a defect serious enough to issue a formal safety recall; however, there is currently no repair remedy available and no clear timeline for when repairs will be completed. As a result, affected owners are left operating vehicles with an acknowledged defect or are left without reliable transportation for an indefinite period of time. I contacted Ford Motor Company and requested interim transportation assistance or a temporary loaner vehicle while awaiting an approved repair solution. Ford denied this request, stating that my vehicle is outside warranty coverage due to age and mileage. Ford also advised that customers should retain receipts in case a reimbursement program becomes available in the future. My concern is that this situation places consumers in an unreasonable position. Although Ford has acknowledged the defect through a federal recall, owners are expected to absorb transportation costs indefinitely while no remedy currently exists. The issue is directly related to the recall condition, not ordinary warranty coverage or routine maintenance. I believe this situation raises concerns regarding the adequacy and timeliness of Ford’s response to Recall 26S28 and the hardship imposed on affected consumers during the unresolved recall period. I respectfully request that NHTSA review the handling of this recall, including the lack of available remedies, absence of re

The truck often shifts from 6th gear down to 1st or second gear, sometimes at highway speeds and all warning light come on. It sometimes resolved itself upon starting it again, but seems to affect the throttle body. When the electrical system resets, you can hear a click from the throttle body indicating that the valve reset when the computer reset. We also have been affected by intense suffering from the transmission that started about the same time, around 2019 and just over 100,000 miles, after the warranty expired. Both the limp mode and shuddering seem to be results of the TRS signal. I have replaced the battery and throttle body (that repair was first suspected to be the sensor) at Lakeland Ford, then later at Midas in north Lakeland I had work done to the wheel mounts and other axle/wheel relate repairs. In total we have spent approximately $4000 on repairs caused by the improper TRS signal. In addition, we have had at least 4 near accidents caused by the faulty signature. I called Ford with no solution, such as a loaner very, for safety before the recall can be fixed.

In the Fall of 2024 we started having trouble with our F150 Ford Truck's transmission. It would drop gears, even from 6th gear to 1st gear, as the recall has stated, so we took it to someone who specifically deals with transmissions, and he stated that the truck was throwing a code that shows that the transmission shift module or sensor was bad. He told us to check with Ford to see if there were any recalls and Ford told us that there were not. We had the sensor replaced, which cost us over $900, back in December of 2024. Well, the transmission continued to slip from 3rd to 4th gears constantly and would only occasionally drop gears, but is continuing to do so on a regular basis. We took the truck back to the transmission garage and he stated it would cost $5000 to rebuild the transmission. We have tried to call the Ford recall line, but just hangs up after giving us the same information the recall letter told us. So we have no one to report that we are having the issues we are having. So are we to continue to drive a truck that possibly could cause an accident on the highway or is there another solution to this problem other than waiting on Ford to come up with a fix possibly in July? I haven’t been in an incident yet with this recall. But I contacted ford today to let them know my truck shifted down to 3rd gear from 5th gear. I informed them that when I entered my vin on the ford web site that my truck came up for a recall and that I contacted a local ford dealer and they said they couldn’t do anything for me because the remedy wouldn’t be out till July 1. My truck is showing a wrench symbol after it shifted down and then it disappears. I told for this is my only transportation vehicle and that I can’t just go buy a new one. They said they aren’t releasing anymore information at the time and they aren’t offering rental vehicles to people with this problem but they aren’t telling people to not drive their vehicles either. I said what happens if I continue to drive my vehicle and it causes me to wreck. They said they couldn’t give me any information.

While accelerating on a green light, vehicle shifted down to a crawl. Truck had an abrupt reduction in speed and was difficult to get vehicle to a safe place to stop. Traffic was heavy and dangerous, trying to get truck off roadway was very difficult. I had to park in a no parking zone to allow traffic to continue. The wrench warning lamp symbol did come on, and warning hill climbing is not possible. The roadway was flat, straight with no incline at all, and clear for easy traffic movement. It was sunny afternoon. I shut truck off and checked underneath vehicle. After 10 minutes, I restarted vehicle and warning lamp was off and truck operated properly.

While driving on the highway at approximately 70 mph, the vehicle unexpectedly downshifted without any prior warning or indication of an issue. This sudden loss of speed created a hazardous situation, as the vehicle travelling behind me was forced to swerve quickly to avoid a rear-end collision. Immediately following this, the check engine light illuminated. From that point forward, the transmission would not shift below 3rd gear for the remainder of my commute. This significantly impacted the drivability of the vehicle and made continued driving concerning, particularly in traffic conditions. After arriving at my work, I retrieved the diagnostic trouble code, which was identified as P0721 (Output Shaft Speed Sensor). Notably, there were no prior warning signs, drivability issues, or alerts indicating a potentail failure before this incident occurred. Given the sudden and unpredictable nature of this event, especially at highway speeds, I am concerned about the safety implications of this issue. A failure of this kind, without warning, creates a substantial risk not only to the driver but also to surrounding traffic.

Engine
Filed Mar 1, 2026FL

The vehicle’s original engine began producing excessive white smoke during startup. The vehicle was diagnosed by AutoNation Ford, and it was determined that the passenger-side cylinder head was cracked near the exhaust port. This defect allowed coolant to intrude into the passenger-side turbocharger. Due to the extent of the damage, the engine required full replacement. The engine was replaced with a used motor under a third-party warranty on August 21, 2024. The replacement engine is now experiencing the exact same failure symptoms. Specifically, the vehicle produces white smoke at startup consistent with coolant intrusion. Bozard Ford performed a diagnostic evaluation, completed on February 27, 2026. Their findings determined that the replacement engine has the identical defect: a cracked passenger-side cylinder head near the exhaust port allowing coolant intrusion into the turbocharger. This repeated failure raises significant safety concerns. Coolant intrusion into the turbocharger and combustion chamber may lead to engine misfire, loss of power, potential engine damage while driving, and possible sudden engine failure. Additionally, burning coolant may damage emissions components and create hazardous driving conditions due to reduced visibility from exhaust smoke. The fact that two separate engines in the same vehicle have experienced the exact same cylinder head failure suggests a potential design or manufacturing defect specific to the 2.7L EcoBoost engine. My vehicle, a 2015 Ford F150. XLT 3.5 Ecoboost 4x4 SuperCrew has been downshifting intermittently while under load. This has almost caused me to wreck my truck as it has gone from 5th gear at 65 mph to suddenly 1st gear causing the rear drive axle to lock up momentarily on the freeway. This has also happened while accelerating onto the freeway where it will go from 3rd or 4th gear under hard acceleration to suddenly slam back into 1st gear. After this happens it will almost always go back to 3rd gear and will not shift up or down. The only solution I’ve found is to pull over, turn the truck off, remove the key and restart the truck. It will then drive normal for some time until it happens again. I was told by Ford that” oh yeah, it’s your transmission lead frame/ output speed sensor”. If Ford knows about this safety issue, why are they not fixing it on their dime. This is obviously a known issue, but I guess some people will have to lose their lives before Ford will fix this.
