Defect. Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Risk. An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy. Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Defect. Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.

Risk. An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy. Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.

Defect. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."

Risk. An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.

Remedy. Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.

Three (3) separate systems failed. 1. Upon seeing cars ahead of us slowing down, my husband pressed the breaks to slow down to avoid collision. When he pressed on the breaks, instead of slowing down or stopping the vehicle, the brakes failed to engage, and caused a surge that compelled the vehicle to lurch forward. The lurch propelled our vehicle into collision with another vehicle. 2. Also, the forward collision warning system failed to alert or engage the breaks. Our vehicle was stopped by the impact of hitting the other vehicle, not by its own breaks. 3. The automatic breaking system also failed to engage at all prior to impact.

The contact owns a 2024 Toyota Sienna Hybrid. The contact stated that while driving at an undisclosed speed, the hatchback independently opened. No warning lights were illuminated. The vehicle was taken to a dealer where it was diagnosed and determined that the hatchback needed to be recalibrated. The vehicle was repaired; however, the failure persisted. The vehicle was taken back to the dealer where it was diagnosed and determined that the hatchback needed to be recalibrated. The vehicle was repaired; however, the failure persisted. The vehicle was taken back to the dealer again and was diagnosed and determined that the hatchback needed to be recalibrated. The vehicle was repaired; however, the failure persisted. The manufacturer was notified of the failure. The failure mileage was approximately 15,000.

I have never been in an accident in my 31 years of driving. However, I am worried that this van is going to cause me to be hit. Several times I have pulled out to make a left or right turn and my gas pedal does NOTHING. I can push it clear to the floor and NOTHING!!! Cars end up slamming on their brakes and honking at me but there's nothing I can do. This is EXTREMELY scary and a HUGE HUGE HUGE manufacturing flaw. There needs to be a recall for this to be fixed before people are injured.

Unexplained acceleration during braking which has caused the vehicle to appear to accelerate further during braking actions causing driver to apple brakes harder

Rear Tail Lights Failed... Brake and Signals work. This was repaired through the dealership. 4 connectors and housing replaced invoice shows part 909890-10942. I am writing to express my concern regarding repeated tail light failures on my black Toyota Sienna that appear to be caused by wiring connection issues. This defect creates a serious safety hazard for my family, other motorists, and pedestrians, particularly in rural areas where there is little to no street lighting. I live in the country, where many roads are unlit and visibility at night is already limited. Because my vehicle is black, it blends into the darkness when the tail lights fail. Drivers approaching from behind may not realize my vehicle is on the road until it is too late to stop safely, significantly increasing the risk of rear-end collisions. This issue is especially concerning because I frequently transport children in my vehicle. Their safety depends on other drivers being able to clearly see my vehicle at night. A tail light failure caused by a wiring defect could place them at unnecessary risk of serious injury or death if another vehicle is unable to see us in time. Moving about 55 miles per hour on right lane of I-95 the Jeep in front of me braked abruptly. I braked hard but as my Sienna was quickly slowing down to stop, the brake pad felt spongy when it was slower and as I pressed even harder, it seemed to have stopped decelerating at that slower speed and that was when my Sienna slammed into the back of the Jeep. Happened today on July 15, 2026 at around 4:24 EST. I will also file this same report to Toyota. My wife and I are at the hospital getting checked, but my Sienna looked undrivable.

The power liftgate on my 2024 Toyota Sienna consistently fails to complete a full close-and-latch cycle. When activated, the liftgate begins its closing sequence normally but then aborts the cycle during the final stages of latching. The system emits an audible beeping signal and the liftgate reverses direction, reopening approximately one-quarter of the way. This failure occurs repeatedly when the vehicle is stationary with no external obstruction present. Multiple repeated attempts to close the liftgate using standard operation (key fob, interior switch, and exterior switch) result in the same failure pattern. The system does not complete a secure latch. Eventually, the liftgate can only be fully closed manually with physical force, bypassing the powered closing function entirely. This condition occurs intermittently at times but has become frequent and repeatable enough to prevent reliable use of the power liftgate system. No obstructions are present during operation. When approaching a stop sign or light and while the brake is already engaged the car will accelerate lunging the car forward from 3-10ft while driver maintains the same pressure on the break pedal.

I am the original owner of a 2024 Toyota Sienna Hybrid (VIN: [XXX]). The vehicle has required two significant engine repairs within the first 57,000 miles of ownership. Both repairs were performed at authorized Toyota dealerships under warranty. INCIDENT 1 — ENGINE REPLACEMENT (10,000 MILES): At approximately 10,000 miles, the engine developed severe knocking. The dealership determined the engine was defective and performed a complete engine replacement under warranty. INCIDENT 2 — OIL LEAK (57,000 MILES): At approximately 57,000 miles, the engine developed an oil leak between the oil pan and the timing belt cover. The dealership repaired the leak under warranty. The dealership informed me that this repair carries only a one-year warranty on the work performed. The fact that the engine required complete replacement at only 10,000 miles indicates a manufacturing defect. The second engine issue at 57,000 miles suggests a pattern of powertrain reliability problems. I contacted the Toyota Brand Engagement Center (Case #XXX) and spoke with both an advocate and a supervisor. Toyota declined to offer any extended powertrain coverage and would only agree to review future issues on a "case-by-case basis." What component or system failed or malfunctioned, and is it available for inspection upon request? -The engine malfunctioned. It is available for inspection. How was your safety or the safety of others put at risk? -The engine went out at 77k miles in the middle of driving with no warning. Could have caused a serious accident. We have always done proper oil changes. The rods separated from the pistons and punched a hole in the engine block. Has the problem been reproduced or confirmed by a dealer or independent service center? The dealer diagnosed it and our mechanic said this has happened with Toyotas too recently Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? -no Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? -no warnings or messages before the failure of the engine.

While applying the brake at the stop light. (25 to30 mile speed limit zone) The Toyota hybrid sienna start to accelerate for split 5 or more seconds. I have to apply the brake harder even though I was already applying the brake to slow down because the light was red and the are several cars in front of me. For the first time I was scared thinking about rear ending some one in front of me or the Toyota Lexus incident where it accelerated and everyone died flashing through my mind. It was not a good feeling.

I notice the driver seat had a clip that popped off unto my carpet, when inspected it came off the seat rail. While moving my seat back using the electronic seat lever a white clip with two metal bearings was sticking out, I notice the ball bearings were not in place and had came off the white clip. Yes, it is available for inspection. This pose a safety concern as the seat rails will not function correctly when moving seat or using the memory seat buttons for each driver. I have not brought the vehicle to a dealer to inspect as there is a estimate fee involved. I am currently out of my 2 year toyota care warranty. I am not seeing any warning lamps or messages besides the seat making a clicking noise when I move the seat back and forth.

Stepping on brakes - Car slows then accelerates a few seconds and lunges forward a couple of feet. Extreme safety issue. This occurred yesterday with my sister in the car and we almost rear ended the vehicle stopped in front of us. I have complained to the Cumberland Toyota dealership in the past and was told if they cant recreate the issue and it doesn't happen all the time there is no way to fix the issue. Yesterday I was almost in an accident because of this faulty design/anomaly associated with my car. This lunging forward is not suppose to happen and I want it fixed Toyota needs to come up with a resolution before it kills me. The driver's side mirror vibrates when the vehicle is at speed. I've been to two dealers who say that it's normal, and everything is intact and installed correctly. When driving, the mirror vibrates a lot, which is a safety issue when trying to use the side mirror during changes, etc.

I am reporting a possible safety defect in my 2024 Toyota Sienna Hybrid AWD, VIN [XXX] . The vehicle intermittently surges, lurches, or creeps forward while I am pressing the brake pedal and attempting to slow or stop. It feels as though the vehicle briefly adds power or braking force momentarily decreases even though my foot remains on the brake. The sensation is not normal creep alone; it feels like a brief unintended forward push while braking. The problem is intermittent, which makes it difficult to predict, but it creates a safety concern because it can happen when slowing, stopping, or maneuvering at low speed. I am concerned this could increase stopping distance or cause the vehicle to move forward unexpectedly. I am requesting investigation of possible defects involving the braking system, hybrid regenerative braking transition, brake actuator, pedal sensing, or related control software. It happens daily. While driving on highway, on 3/11/26, car powered down. I had to coast to side of highway. I was able to turn vehicle on after several minutes, but SEVERAL system malfunction error messages (hybrid, accelerator, pre-collision, tpms, airbag, engine…) appeared. The vehicle would not accelerate beyond 10mph. Once I moved it to a safe location, I had the vehicle towed to the dealership. The dealership technician was able to replicate the issue during test drive. The technician apparently had the vehicle towed back to the dealership last night. The car has had other related issues since June 2024 when I bought it. The TPMS system, the accelerator system, and the airbag system have all malfunctioned independently. The accelerator system malfunctioned in Jan. I had to pay for a replacement fuse despite the warranty. The vehicle is modified with Braunability equipment for wheelchair access. These modifications are approved by Toyota and were completed by MobilityWorks. Aside from the vehicle itself being unsafe to drive, the passenger sliding door has had numerous problems. On 3/07/26, the door mysteriously slammed shut after opening and would not open again. These are all dangerous situations that could have resulted in fatal accidents.

My 2024 toyota sienna suddenly stopped for 2-3 seconds while driving in the city roads. I was caught off-guard with that behavior. I kept pushing on my gas pedal and after few hard pumps the power came back. This happened couple of times so far. Not sure what the main problem is.
