There was a rough idle and tje the check engine light came on, idle smoothed out and the the following week the check engine light started blinking and the van started to miss and then drove rough. Then I took the van to dealer and they said it seemed that I have a cracked head after I received engine code for misfire on cylinder 3... sounds like this is a growing issue for this van and engine type.. Thank You for any help with this situation.
2018 Chrysler Pacifica complaints

Blown head gasket. Had to get it repaired. Steering is catching as well. Probably need a new steering box. It’s been one expensive repair after another.
I bought my 2018 Chrysler Pacifica limited about two years ago. It’s been the worst decision I’ve ever made. I have just about a 3% rate and with the amount of issues these things have I’m so far upside down it’s not even funny. This car with all of its known issues is depreciating faster than I can pay for it. I currently have about 129,000 miles on it. I couldn’t get rid of this thing if I tried. The Chrysler dealership I service it with said they would give me 4000.00 IF they were willing to buy it back. The engine build company my mechanic uses says they won't even build these engines at all because of how many issues they have. So no option other than to buy the exact same engine is has now through Chrysler and know it will do it again. I’m having stalling/accelerating issues (shop can’t figure it out, kids cry when it stalls in traffic.. I've been in for it about 9 times to two different place) Oil cooler housing unit replaced last year (taken it back in a few times for ongoing leak, now need a head gasket) Head gasket Batteries galore Ongoing start stop issues My AC went out the month I bought it (leak in the lines near the rear) 3000k to fix The shop says it needs lifters, cam shafts and rockers (might as well do the engine if I had the extra money after paying off the rest of the repairs) My heated and cooled seats have stopped working numerous times Dash comes on and off when it wants Back up camera stays on the screen while I drive as much as it doesnt My rear wash
74,121 miles head warped on engine. $4300 repair to replace head and head gasket.
[XXX] [XXX] [XXX] National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation 1200 New Jersey Avenue SE Washington, DC 20590 Subject: Complaint Regarding Recurrent Mechanical Issues and Potential Safety Concerns with 2018 Chrysler Pacifica Dear NHTSA Office of Defects Investigation, I am writing to formally report repeated mechanical issues with my 2018 Chrysler Pacifica (VIN [XXX] ), which have raised significant concerns regarding the vehicle’s reliability and safety. In 2023, my vehicle initially experienced a persistent ticking noise in the engine, leading to extensive repairs. These included replacing the tappets, rockers, and timing chain tensioner on the right bank of the engine. Following these repairs, the vehicle exhibited low oil pressure, ultimately requiring a complete long block replacement. These issues left the vehicle out of service for nearly two months, significantly disrupting our daily lives. Despite these major repairs, my Pacifica has recently developed overheating issues, necessitating the replacement of the radiator at an estimated cost of over $2,000. The vehicle has less than 60,000 miles, and I find it deeply concerning that such critical mechanical issues are occurring at such low mileage. These repeated failures suggest a potential underlying defect in the design or manufacturing of this vehicle, which could present a safety risk. An overheating engine, for instance, poses a potential hazard both to the occupants and to
The hybrid battery coolant heater (part 68443445AC) was confirmed a problem, however the dealership wont review ANY recalls on my vehicle until I pay $1700 to replace that part. However, there are MULTIPLE incidents online showing this is a problem with this vehicle and should be a recall as this is obviously defective within the first 150000 miles. My car has roughly 124K miles, making it just outside the warranty coverage. Shortly before this lamp warning (check engine) came on my steering wheel started "sticking" making it hard to steer in a curve, causing me to veer off the road. Within 7 days of this happening, is when my check engine light appeared, throwing code POE15. The heat now also takes much longer to warm up- like 30 minutes longer. The dealership said none of the issues are connected, however after online research I have determined that is not true.
The contact owns a 2018 Chrysler Pacifica. The contact stated that since 2019, the "Auto START/STOP feature unavailable" message was displayed on the instrument panel. Due to the failure, the vehicle intermittently failed to start, the vehicle was jerking and hesitating while depressing the accelerator pedal, and the vehicle also shut-off unintendedly while driving at various speeds. The vehicle had been towed to a dealer on several occasions and the auxiliary and the main batteries replaced; however, the failure returned approximately a year and a half later. To prevent the failure from recurring, the contact disabled the Auto START/STOP feature to prevent the vehicle from stalling. The contact stated that numerous repairs had been performed on the vehicle and that the only other solutions would be to replace the transmission, perform a software update or replace a possible unknown sensor. Additionally, the contact stated that while turning off the vehicle there was an abnormal knocking sound coming from the engine. The manufacturer was notified of the failure and the contact was given a case number. The vehicle was not repaired. The failure mileage was approximately 64,000.
There was a fire of unknown origin within the van. The fire was self extinguished and contained to the inside of the vehicle. No one was in the vehicle at the time of the fire, and it was found about 45 minutes after leaving the vehicle parked.
An engine fault code for P0303 has become active on my 2018 Pacifica. This code was confirmed by an independent service center and confirmed that the code is active. The service center ran a compression test and showed that cylinder three is low on compression. They then ran a hydrocarbon test and confirmed exhaust in the coolant. This diagnosis points to the common issue related to a bad head gasket potentially due to a warped head/block. The car has never overheated and all service maintained.

Vehicle just dies when you brake. This has now happened twice. Once in May 2024 and now Oct 2024. You have no steering and cannot be put into neutral or park. Must pull neutral cord to get pushed out of road. No codes pull up other than the neutral cord used. In May 2024 towed to dealer and they replaced starter but could not find anything else. In Oct 2024, took it to an independent car place and he could not find anything wrong, just re-boot computer. This is very dangerous; this is a handicap van modified for a wheelchair person to be put in through the back of the van. If I get hit in the back, I can't get my son out of the van. Extremely unsafe! I know the 2017 Chrysler Pacifica's were recalled for this and I believe the powertrain control module software was updated or the chip was replaced. This needs to be recalled for the 2018's also.
Engine code misfire cylinder #3 and losing coolant. Took it to the mechanic who diagnosed it has a failed head gasket and coolant leaking into the cylinder. Vehicle had 85,000 miles at the time.
Misfire code on cylinder 1. Pulled the coil and plug and replaced, cleared code. Code returns in a matter of days. Then engine temp hot flashes during a short drive, so I purchase approved OAT coolant and fill it back up. Happens again three weeks later. Then two weeks later. Have local mechanic replace coolant pump and thermostat thinking that might do the trick. It doesn't. Issues return in a week. Mechanic can find no coolant leak and says the only thing that makes sense is a blown head gasket. I browse Chrysler forums and see that this is a common issue. No recall as of yet.
I left work one day and I stopped at Walmart once I was done there, I got into my car to leave when I attempted to start my car all of the dash board lights illuminated although my car turned over it would not engage into gear. I did notice that all the dashboard lights illuminated simultaneously, which was quite alarming. This happened not once, but twice both times when I went to start the car however the second time I immediately drove it to the dealer for service. This left me concerned about what might be causing the issue. Only for the dealer to tell me it is my transmission and it would cost me $10,000.00 to fix it as a blue collar worker I have absolutely know idea what I am going to do. I then began to research the car only to find out that this year, make & model has a recall on the transmission describing the exact symptoms I am having. Please help!!!!!! my lively hood depends on me being able to get to work!
While I was driving the right control arm seperated from the car. I was fortunate that I wasn't on the highway, also that I did not cause a collision with another car/person. With no warning the car felt as if it dropped on to the ground. Soon after I received an error message saying to service the brakes which i assume was also damaged when the control arm fell... I had to have the car towed to a repair shop and received a list of repairs. The car is avaliable for inspection. I had no clue how something like that could even happen. Ive been trying to find answers when i saw a recall on another car by the same maker that perfectly summed up what happened to me. Chrysler's number for this recall is V15.
In April of 2022 my dash engine check light went on, as well as my start stop. Van was running rough and coolant missing. Chrysler fixed seals, gaskets. In July of 2024, 3000 before warranty ended, the same thing, Chrysler fixed. As of today October 10, 2024, the same dash lights and coolant missing. Now I am over 100,000 warranty. they just fixed this issue. Looking at Technical Service Bulletins and thousands of Chrysler forum complaints report the same issue. Despite two warranty fixes almost 10,00 in 15 months, the same issue continues. In 2024 this past fix Chrysler said this on the phone, Failed leak and combustion test. Cylinder head gasket has gone. Cylinder itself. Cylinder one. Misfire but not as bad. Checking the cylinder wall. Seems odd. Really inspecting the cylinder wall. Odd it would come back odd a year later. Not sure what he is going to find. They can do it tomorrow unless more. When my husband called today all they said was payment is on us of it is not the same thing but did not day to was we did this 3 months ago. I took my van to auto zone after same dash lights went on and they read the code, misfire cylinder one
2018 Chrysler Pacifica Touring L+ with 59,300 miles on it that we bought new from the dealer with a blown head gasket needing replacement of cylinders 3 and 4. We are outside the powertrain warranty. We got a check engine light [XXX] and found code p-0303 (cylinder 3 misfire). We were fortunate enough to get it to the dealer the very next day. It failed the engine block test and we were informed that the head gasket had blown and needed replacement. They also recommended replacing cylinders 3 and 4. This cannot be the expected lifespan of this engine or car. Searching forums on the internet and unfortunately it is a common problem with pacifica's and other vehicles with the pentastar 3.6L engine. We are averaging <10,000 miles per year, did not drive it aggressively, and got regular maintenance on it. This seems like obvious premature failure of the engine. Chrysler wanted $8200 for the parts and labor to fix it. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Been complaining about his front steering key suspension since 2020 about rattling and squeaking whenever customer turn in the wheel
The contact owns a 2018 Chrysler Pacifica. The contact stated while driving approximately 40 MPH, the vehicle stalled. The contact stated that there was no warning light illuminated. The contact stated that the power steering and braking functionality were inoperable. The contact had the vehicle towed to a dealer. The dealer diagnosed the vehicle and determined that the batteries and the alternator needed to be replaced. The vehicle was repaired. The contact researched online and related the failure to NHTSA Campaign Number: 19V348000 (Electrical System). The manufacturer was not informed of the failure. The failure mileage was 104,417.
I have a 2018 Chrysler Pacifica with 50k miles and it is still covered under the powertrain warranty. 1. I had a check engine light with code as "misfire on cylinder 6" in August of 2024. I had all spark plugs and engine coils replaced. 2. I drove the vehicle for one week before the engine light came back on with the same code, and then the vehicle wouldn't start. It was towed to a dealer. 3. A dealer replaced the starter assembly and valve body. 4. I drove the vehicle for 2 days before the engine light came back on with code as misfire to a different cylinder. 5. I am now being told by the dealer that I need to replace both cylinder heads, both cylinder head gaskets, timing cover gaskets, valve cover gaskets, valve cover gaskets and the coil boots (again) because they were ruined the the failure/leaks in the other parts.
The check engine light recently came on, and there was an issue with starting and idling, so we replaced the battery and the light went off, but would randomly come back on. One day notice the vehicle as overheating, and found the coolant tank to be completely empty. Refilled the tank, and several days later the problem arose again. Checked under the van immediately after refilling, no leaks detected. Took van to the dealership and was told the head gasket was bad, and the coolant was leaking into cylinder 3. Recommended repairs noted are as follows: “HEAD GASKET IS BAD. LEAKING ON CYLINDER 3. NEEDS A NEW HEAD AND GASKET TO FIX ISSUE. WILL ALSO NEED CAMS, LIFTERS, AND ROCKERS DUE TO THE TICKING COMING FROM THE RIGHT SIDE OF THE MOTOR” I’ve seen that apparently, this is a more than common issue specifically with the 2018 Chrysler Pacifica. Others have recommended a total engine replacement, as the damage caused thus far is significant enough that replacing only the head and gasket would be, as quoted, “a very expensive bandaid.” This is the only vehicle we have available for my wife and four small kids, and to see that it’s such a common and, thus far ignored problem is infuriating to say the least. Vehicle was purchased 2 years ago, and has depreciated $21,000 in that short amount of time. Repairs are estimated around $6000. Vehicle value is estimated around negative $12,000, so we’re stuck with a faulty vehicle regardless. With the overwhelming number of complaints regarding
76,115 and my head gasket is blown. After researching, I found that there are more than over 1000 people with the same issue. Dealership says they can do nothing. Chrysler of America says they have no resolution either. This is ridiculous that so many people are having the issues with low miles on their vans. Something has to be done. I'm not sure why there is not a recall already (because Chrysler won't accept the fact that they messed up). People don't have money to replace the head gaskets and/or replace heads or even engines. Please help.
The vehicle has stalled several times while driving and while at a stop light leaving the van completely disabled and it will not turn on or start again. My wife and 2 year old were put at risk when the vehicle stalled on them at a major intersection and my wife was unable to put the vehicle into parking or drive since there was no power after it stalled and dashboard lights flicked on and off multiple times.. Stall has also happened at drive thru almost causing collision with vehicle in front. The stall produces dashboard error messages reporting an internal transmission malfunction, auto start/stop error, and or a problem charging the battery. Certified ASE mechanics can’t reproduce the issue or find anything to pin point the issue after multiple diagnostic tests ran and physical thorough inspection. Every time a stall happens loads of error codes (85+) pull up but none point to the root cause of stalling happening. Vehicle has most up to date software update done over a year ago.
The contact owns a 2018 Chrysler Pacifica. The contact stated while driving 25 MPH, the steering wheel seized and was unable to be turned. The contact turned off and restarted the vehicle; however, the steering failure message was displayed, and the power steering function was not available. The vehicle was not taken to an independent mechanic or the dealer to be diagnosed or repaired. The manufacturer was notified of the failure and informed the contact that the VIN was not under recall. The approximate failure mileage was 74,780.
Cylinder head gasket failure: known to be a common failure in the Pentastar 3.6L engine. Repair in thousands, and may not be a solution as the engine block may warp due to failure preventing a proper seal with the cylinder head. The number of cases of this failure is alarming and should really be considered a proper consumer risk.
At just over 100K miles, vehicle had check engine light come on (P303) indicating cylinder #3 mis-fire. After replacing spark plugs did not resolve the issue, further diagnosis from mechanic revealed leaking of coolant into cylinder #3 as the cause of the miss-fire. This is further borne out by the rough starts when cold followed by more smooth operation once the coolant is burned off. Research on-line shows that this failure is common in Chrysler Pacifica rather than a one-off incident. Leaking of coolant into the engine not only puts the engine (and therefore vehicle occupants) at risk but also damages the catalytic converter which can have an impact on the environment.
Purchased vehicle used and have put on miles. But since day 1 issues have been arising. First the Auto stop/start function stopped, 2 years later still unable to find the fix. More recent the gear shift just doesn’t work/turn on randomly or turns off while driving which is a safety concern. Now for the past 2 months the gear shift will not work or while driving the vehicle just loses acceleration and braking and slows to a stall and I can’t do anything until the gear shift turns back. I had 3 children on [XXX] when this started putting all our lives in danger. We were stuck for 2 hours the vehicle ran but was not drivable. Every mechanic says something different and it is a luck of the draw if the vehicle will work when you try it. Batteries all tested good. No obvious other issues other than loss of power steering for a short time. All warning lights flashing on and off randomly but it coincides with the loss of driving capabilities. I just want to be done with this vehicle it’s dangerous and now I’m stuck with it. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Sometimes when I am accelerating it feels like I am being rear-ended. The whole vehicle jumps as if it is being pushed. I have tried to reset the Transmission by watching YouTube videos that show the same problem. But it has not fixed it. It also happens when I am driving. I am worried that it may not shift gears at all. I use it for all my long distance trips and do not want to break down on Interstate 95 in the dark. I have had it back to Auto Lenders in EHT, NJ multiple times from 2019- 2021. I gave up bringing it back because it never got fixed. Now it is happening almost every week.
2018 Pacifica with multiple similar issues. August 2018 - Recall - Reprogram Powertrain Control module May 1019 - Recall fixes (Power steering and ball joint). July 2021 - start/stop light came on - took to dealership and they replaced the battery for that system. Feb 2022 - Recall customer satisfaction notification 1 Y65 July 2022 - ISSUES begin - Cylinder misfire - and car would not start and was smoking. Van was towed to dealership. The dealership had the van for 2 months and had a difficult time really diagnosing the issue. They ended up replacing the right Cylinder head - which is a major part of the engine. Just being over the 60,000 mile mark, we had to fight with Chrysler to honor the warranty on this very expensive fix. Spark plugs were also changed at that time. August 2024 - check engine light is back on, as well as the automatic start/stop button. We took to Auto zone and the diagnosis was Cylinder 6 misfire. All along we have felt there should have been a recall on this van. Is it an electrical issue that is failing? Searching online there are a lot of these complaints about Cylinder misfiring issues with this year/model. It also looks like there may have been a class action lawsuit as well. A 4 year old van should not need a major engine repair. Now here we are 2 years later with a 6 year old van and having a similar issue. We are worried it is going to be another costly fix. We have had nothing but issues with this van and it is unacceptable, especially regardi
The contact owns a 2018 Chrysler Pacifica. The contact stated while driving approximately 70 MPH, the message that the "Vehicle Will Shut Down" was displayed and the vehicle stalled. The contact was able to call for help and had the vehicle jumpstarted. The contact stated that the vehicle failure was intermittent but became a recurring failure. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 118,000.
While driving on the highway, the car suddenly had a flash of start/stop system and then car stalled as soon as I moved to the exit lane. Once I stalled I had to stop car. Wait several minutes, and try to restart car. After several attempts and me blocking busy traffic, it restarted. This happened repeatedly even after technician said they cleared codes.
We had a cylinder 6 misfire which caused the engine to stop working. Our car would not start. There were no warning lamps, messages or any other symptom of the problem. We had just had the car serviced and there were no visible issues. We are now told by Chrysler that they have to replace the entire engine. They want to put in the same model of the engine that just randomly stopped working.
The hood paint is showing bubbles
Cylinder 5 misfire confirmed coolant leak and head gasket failure by certified mechanic. 103k miles. Always maintained according to recommended schedule. Never overheated. Possible sudden engine failure is absolutely a safety concern leaving me and my family stranded. $6000 repair needed.
While driving on a 4-lane highway, I received a message on the dashboard "Vehicle will shut off soon". I had just a few seconds before power to the vehicle was limited. The engine stopped working and another message was displayed: "Battery Conditioning Leave Ignition in Run." After that the car would no longer move and I was in the middle of the right lane. Luckily I could get my children out safely. The vehicle was towed to the local Chrysler dealership where it is waiting for repair.
Steering system becomes stuck. You have to give a hard jerk to the steering wheel to free the steering. Happen on an irregular basis but does occur. This can cause an over steering issue. Vehicle is currently at a dealership to be inspected for possible cause. There were no warning lights or messages
At 93500 miles, the check engine light came on. I immediately had it checked by my mechanic and the code was identified as a P301, (Cylinder 1 misfire.) Service was scheduled. At time of service, it was determined that the cylinder head gasket has failed and is leaking coolant into the cylinder, causing the misfire. I have given the shop approval to replace both cylinder head gaskets at an estimated cost of $4500. Loss of coolant can cause catastrophic engine failure, especially while driving. This vehicle is sold as a family vehicle, and can likely be transporting children. This appears to be a known and common engine failure issue and has not been addressed by the Chrysler corporation.
Frequently replacing axillary battery due to a power draw from the sliding drivers side passenger door. The door has a short in the wiring that has killed the battery and drained the main battery no less than 6 times since owning this since brand new in 2018. Vehicle had been stranded on roadways twice, gas station line once, and parking lots three times due to this short in the passenger door. Since removing the fuse for this door we have had no issues, except the sliding power door is now a manual door.
The contact owns a 2018 Chrysler Pacifica. The contact stated the Auto STOP/START light and head gasket light remained illuminated. While starting the vehicle, the vehicle was vibrating abnormally. The vehicle was taken to the dealer who diagnosed a leak in the head gasket. While inspecting the vehicle, the dealer became aware of a hole in the engine, and that the engine and head gasket needed to be replaced. The vehicle had not been repaired. The manufacturer was made aware of the failure and opened a case number. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was 92,000.
We purchased our 2018 Chrysler Pacifica in October 2023. It had 72,000 miles at the time of purchase. My wife was driving with our kids when the misfiring happened. This is clearly a safety issue as my children and wife don't feel safe in it. About two weeks ago, the check engine light came on and I took it to AutoZone to get the code checked out and then went to a mechanic. Upon inspection and diagnostics. He said the head gasket was blown and that a new engine was needed. We are currently sending the oil out to get tested for coolant. There is currently less than 85k miles on the minivan, and on our first oil change a couple of months ago, we started using a full high mileage synthetic motor oil. We are incredibly disappointed that our minivan has such a huge problem that apparently is fairly common with this year and model.
Check engine light came on. Pulled code, cylinder 1 misfire. Service for spark plugs was already scheduled for 2 days later. Van parked awaiting service. Service performed, CEL off. Van runs as normal for about 1 day. CEL comes back on. Cylinder 1 misfire. Multiple messages display on instrument cluster. Service shifter. Start/Stop unavailable. Some inerior lights turning off then back on later. Similar to a previous issue with a failing battery. Replaced both batteries. Cleared codes. Van drives as normal for 1 afternoon. Next day hard start and rough idle. CEL on. Cylinder 1 misfire. Van operates normally once warm. Schedule follow up service for diagnostic. Next day, CEL off. Van operates as normal in the morning. Hard start in the afternoon with rough idle and CEL back on. Van runs rough tries to stall. Service shifter message returns. Start/Stop unavailable message returns. Parked van at home until service appointment. Day of service battery completely flat. multiple attempts at jumpstart finally get the van started. White smoke coming from exhaust mixed with smell of burning coolant. Notice coolant level has dropped in resevoir. CEL light is on, cylinder 1 misfire. Service messages. Tested battery voltage and alternator appears to be charging. Dropped van off with service. 2 days later report from dealer service department indicates leaking head gasket, and scoring of walls in cylinder 1 seen through endoscope. Recommendation is new motor. Van again requires jump start
Van currently has a P0303 code for cylinder 3. Have done research and found that this is a common issues with this vehicle. Also the stop/start needs to be service. Which is also know to happen a lot in this type of vehicle.
Having trouble with steering. At times steering wheel makes noises and locks up while turning. I was told a recall was already done and that I need a new computer that will cost 5k for part and install. I just had issues with panel lights going on and off and had to replace secondary battery due to stop/start button which Ive had trouble with consistently. I think this issue with computer may be due to recall issue and I believe this issue should be fixed and paid for thru recall process. It is an ongoing issue with pacifica 2018 with start/stop and push button starts. I have taken it to 2 dealers. They said recall done. So I need new part
I keep getting a check engine light with my Pacifica hybrid. The scanner says P1B11, soft code is P0506, Hybrid battery or wiring harness. The codes erase after driving for a long while, however, I notice that I’m not able to maintain a full hybrid battery charge for longer than a few miles. After about 5 miles of driving, I’m a 80% of battery life left. MPG averages about 30+/- something’s not right. It’s happened twice now, both times with excessive outside temperatures. 1st time while idling, 2nd time while stopped for fueling.
While driving on the highway at 70 mph with children in the car, my screen system shut off quickly and then rebooted. Was concerned something worse could happen. Randomly, my car will start fine and will drive to my location, when leaving and trying to start my push/start button it will not start. I attempt many times to start the vehicle until the battery has no power. It doesn't take much to jump the battery. I've been stranded at grocery stores, work, or other places with my children. I was driving and stopped at a stop light, and my car shut off with no noticed. Had to get jumped at an intersection. When driving my car will stall when accelerating and continue driving Both batteries have recently been changed out. I believe when reading about this its the powertrain control module. This has not been confirmed by the dealership, no engine light is on, the van drives great but I am afraid that it will not start after I've turned it off or will lock up and shut off when I am driving with my children in the vehicle. The no starts were farther in between with issues but has recently become more frequent. There are no prior issues to the vehicle. My safety and my children's safety is put at risk , hot weather when car won't start having to wait, stopping at an intersection and the van shuts off with my kids in the car , driving on highway and electronics on vehicle shuts off, possibility of losing control of vehicle. Last occurance was at a grocery store tried to turn on vehicl
I had had multiple incidents when the Van would get stuck in a gear or would lose the ability to accelerate. The passenger doors, specifically the right side passenger door would get suck and continuously open because it could not close. The rear view cameras would get stuck and after reverting to D it would stay on the screen for at least 5 min before switching back. The screen would also go blank. The van would also get stuck between gears or would shut off at a stop light. On this particular day I was driving and was starting to slowdown for the light signal. The van suddenly jolted and stopped completely. I tried to turn it on and it would not start. I was very lucky that the car behind me noticed me slowing down and did not hit the van. I tried to turn on the van and the electrical function would turn on but not the engine. Then after continuously trying to start it the gear function would not move. I also want to add that at the time of purchase the van was missing a air duct and I was not made aware of that. I was told by the dealer that it had active recalls but they were not able to fix the recalls because the engine would not start. It had an active recall for the flash software and for the start stop. Dealer tried to tell me it overheated, however this is false, there was no white smoke and no other indications that the van had overheated. I wasn't able to add that the electrical in the vehicle is a problem. The radio and screen would just go blank at times.
On 4 separate occasions the vehicle while in motion will flash an alert that states “vehicle shutting down, stop safely” and the van will proceed to completely shut down- all electrical components do not work, including the sliding doors. After 30-45 minutes, the can will start back up without issue. Has been evaluated by a Chrysler dealership mechanic, as well as 2 private mechanics. They have been unable to replicate the issue. The main and auxillary batteries have been replaced twice each. Each incident we were able to pull over safely and there have not been any injuries or crashes, but we are concerned if it happens and we can’t pull over, we could easily crash.
The contact owns a 2018 Chrysler Pacifica. The contact stated that while driving at slow speeds, the vehicle transmission shifted to park with the START/STOP system warning light illuminated. The contact took the vehicle to an independent mechanic where an unknown repair was performed; however, the failure persisted. The contact then took the vehicle to the dealer and was informed that the gear shifter module needed to be replaced. About a year later, the contact stated that the START/STOP warning light and the check engine warning light were illuminated. The vehicle was taken back to the dealer where a misfire in cylinder #6 was found. The dealer flushed the engine, replaced the ignition coils, and replaced the spark plug in cylinder#6. Despite the repairs, the contact became aware of a slight engine rattle at start up. The contact continued to drive the vehicle however, the check engine and START/STOP warning lights returned. The vehicle was taken back to the dealer where the contact was informed that the failure might be linked to a blown head gasket but could not determine the failure without an official diagnostic test. While the contact's husband was driving from the dealer, the engine started to overheat. The vehicle was driven to the residence and then towed to an independent mechanic and the vehicle was diagnosed with head gasket failure. The manufacturer was notified of the failure and advised the contact to have the vehicle towed back to the dealer to be diagnosed.
At low speed 12-18 mph the transmission hard shifts. It slams so hard that you feel like someone rearended you.
I am experiencing premature transmission failure. It is available for inspection upon request. The safety of myself and my 3 small children have been put at risk because of the likely potential of transmission failure during our frequent trips on the interstate, that could lead to a deadly accident. This problem as been confirmed by an independent licensed automotive mechanic. The vehicle has not yet been inspected by any other entity, but is available for inspection. The symptoms of the problem started around March of this year, as a sweet burning smell, that lead to a smell of burning rubber. The Chrysler dealership I brought it to would not check the transmission fluid itself due to the "forever system" design of the 2018 Chrysler Pacifica, that does not allow for easy access to the transmission fluid, rather is designed to tell you if it is full or not. So the issue was missed during inspection, and was labeled as being in good condition. After I noticed the vehicle beginning to skip between 1st and 2nd gear recently (end of June), I brought it for inspection to an independent mechanic who was able to inspect the fluid which was extremely burned, as well has hook the vehicle up to their reader machine and access that it was in fact in the process of transmission failure. The transmission has not yet completely failed, because I have stopped driving the vehicle for fear of the safety of my children.

NHTSA publishes every complaint as the owner wrote it and investigates only a fraction. Spelling, capitalisation and judgement are the filer’s own. What carries weight is repetition: the same component failing the same way across many filings, and whether the manufacturer eventually opened a recall for it.
