In reference to recall query RQ24004, the appropriate software was installed under recall O3A for Loss of Motive Power on my 2018 Chrysler Pacifica Hybrid. However, on multiple occasions the vehicle has displayed "Stop Safely - Vehicle Will Shut Off Soon". Once on 6Apr2026 and again on 13Mar2026. The first occasion occurred on a winding mountain road enroute to a ski hill. The road was extremely busy at the time and pulling off was difficult and dangerous with only a small snow covered shoulder. The second occasion occurred on I-15 during rush hour. Again pulling across multiple lanes of traffic and stopping along the shoulder was dangerous. The dealer seems reluctant to do anything other than re-flash i.e. update the software again. A brief internet search demonstrates that often the software is insufficient to fix the problem.

We experience a sticking between 11 and 1 when driving at slow (parking lot) speeds and at highway speeds. It makes it more difficult to drive on the high around long turns, because if it's a gentle turn, holding the right angle of turn is difficult if it's sticking around that area. This can lead to unintentional swerving. This is an ongoing issue.

2018 Pacifia developed a leak in the left bank head gasket. The vehicle only has 53,000 miles. From the forms it sounds like this maybe a common issue. So I am reporting the issue to hopefully help push the company to cover build or design issues. The gasket shouldn't break down a little past 53,000 miles. Thanks,

Chrysler performed the 73B recall and n my van in August of 2025. Ever since performing this recall the van has a continuous stream of check engine lights and error codes. The van has been at the dealership at least 6 times for well over 100 days. The check engine lights and error codes always return. The hybrid battery recently had a failure code, and was replaced. I didn't even make it home before I got a check engine light and error codes. The dealership cleared the check engine light and sent me on my way. The codes returned and the van is back at the dealership. The codes I have been getting are : Service Hybrid System, Parking Brake Temporarily Unavailable, Service Electronic Throttle Control, Service Shifter( I was not able to drive as it wouldn't go into gear), Service Electronic Stability Control, Blind Spot Alert, and Service Airbag System. On the longest service visit (over 80 days), Chrysler ended up just telling the dealership to erase the stored codes and return the van to me. They test drive it 50 miles. The error codes returned. They currently have the van again for the same reason.

The contact owned a 2018 Chrysler Pacifica. The contact stated that while attempting to park the vehicle in a parking lot, the brake pedal was depressed; however, the accelerator was inadvertently depressed, causing the vehicle to unintendedly accelerate. There was no warning light illuminated. The vehicle accelerated and crashed into the wall. After the vehicle crashed into the wall, there was an abnormal sound of air coming from the vehicle. The contact stated that the sound was not coming from the radiator. Additionally, none of the air bags inside the vehicle had deployed during the crash. The contact and a passenger inside the vehicle sustained bruises in the crash from the seat belts. Medical attention was sought following the crash. A Police report was filed. The vehicle was towed from the scene and deemed a total loss by the Insurance Provider. The dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 54,000.

When turning slightly right or left, the lower steering column makes a clicking sound while staying stuck and you have to forcefully move the steering wheel in the opposite direction to get the steering unstuck. A safety hazard of possible over correction could send someone into oncoming traffic.

I am experiencing the exact issues identified in NHTS Campaign Number: PE25009. The steering system on my 2018 Chrysler Pacifica exhibits a sticking sensation during gradual turns, particularly when the steering wheel is positioned near the 11 o'clock or 1 o'clock position. It is most prevalent at speeds over 60MPH. The safety of everyone in the van has been put at risk by requiring increased steering force in the opposite direction that could easily be overcompensated for by the driver. The first time it occurred, I overcompensated which easily could have caused a crash into a guardrail or vehicle. Fortunately, it did not and I was aware of the issue and how to react when it continued during the drive. There were no warning lamps, messages, or other indicators that there's a problem with the van. After parking the vehicle, I saw online that it could be possible to reset the steering sensors by turning the wheel all the way in each direction several times. Not sure if it was a coincidence, but I did not experience the same sticking for the remainder of the drive even with reaching the same speed and taking similar, gradual turns. A service center inspection will come next.

2018 Chrysler Pacifica PHEV subject to Safety Recall 73B (NHTSA 24V-536) for HV battery fire risk. Following the recall repair at Fremont CDJR (Newark, CA) in January-February 2025, this vehicle has experienced systematic cascading hybrid drivetrain failures across 5 repair visits totaling 57 days out of service: (1) CAN bus star connector failure — 10 modules with fault codes U0103/U0001; (2) Hybrid Control Processor failure — required 14 flash attempts to program; (3) $1,786.67 repair failed within 23 miles — CEL and BRAKE warning light appeared simultaneously; (4) Complete transmission removal required to replace auxiliary transmission fluid pump (DTC P2797-00); (5) Power Inverter Module replacement still pending. Vehicle also disabled itself completely while driving prior to March 2026, requiring 30 minutes rest before it could be driven. All failed components are part of the integrated hybrid drivetrain disturbed during the recall repair. Request investigation into whether Recall 73B repairs are being performed correctly at authorized FCA dealers and whether other owners are experiencing similar post-recall failures.

My 2018 Pacifica Hybrid suffered a catastrophic suspension separation while driving—the exact failure described in Safety Recall V15. The driver fortunately arrived at home safely only seeing smoke coming out form the tire rubbing against the wheel hood. Although my VIN isn't showing as 'active,' this is clearly a safety defect. I would like to open a case for a Goodwill Repair or a 'Special Policy' adjustment because this is a known dangerous failure.

We have had to pull over and luckily have been able to get it to reverse. The seat also will try to stow when I open the side door on the passenger side. This is a huge safety concern to me as death can and has occurred in the past months.

The steering gets stuck trying to return wheel to center after going around a curve or slight turn. It usually happens at 10/11 and 1/2 o'clock positions. It gets stuck and then i have to either slow down and turn the wheel again a little farther, then try to turn it back to center to get it unstuck. This has been replicated by an independent service center. This has been going on for the past 6-7 months. Prior to this, the only thing that was noticed was when reversing and turning at the same time the wheel would shake like something was loose. if on a gravel road it can sometimes feel like the steering is shaky or loose and like there isn't full control. I took this to an independent service center for repair and they explained my whole power steering gear assembly would need to be replaced.

While traveling 350 miles from my home with my family of five, the vehicle experienced a sudden 'Loss of Motive Power' event. The dashboard displayed the message: 'Stop Safely - Vehicle Will Shut Off Soon.' I lost the ability to accelerate and had to navigate to the shoulder in high-speed traffic. The vehicle is currently inoperable and the 12V battery has since died because the Power Inverter Module (PIM) has stopped maintaining system voltage. This vehicle had Recall 03A (NHTSA 23V-010) performed on July 7, 2023, and the more recent Recall Y40 update performed on March 11, 2026. Despite having the 'latest software remedy' installed less than 30 days ago, the vehicle suffered the exact safety failure the recalls were meant to address. This proves that the software-only remedy is insufficient to prevent the hardware failure (internal transmission wiring short). A formal Vehicle Scan Report performed at Lake Norman Chrysler (Dealer 60945) on 3/30/26 explicitly shows 'Distance Travelled with Light ON: 146 miles' (Page 14), documenting a sustained failure of the motive power system. Furthermore, the dealer’s own certified technician (J. Olmstead) provided a written Action Plan (Page 9) recommending a full Transmission and Power Inverter Module (PIM) replacement. Despite the documented 146-mile failure history and the technician's recommendation for hardware replacement under the XE1 Warranty Extension, the dealership service management is refusing to perform the repair, claiming

Vehicle experienced intermittent and sudden loss of power steering assist while driving. Steering becomes extremely heavy without warning, creating a risk of loss of control. This is the same symptom addressed by Recall V53 (Power Steering Assist Loss). The recall was previously marked as completed, but the defect has returned, indicating an ineffective recall remedy. Issue occurs during normal driving and is reproducible. Chrysler claims safety recalls have a warranty and will not repair or reinspect it.

Engine experienced premature valvetrain failure (camshaft/lifter issue) at approximately 145,000. Vehicle developed ticking noise, misfire, and loss of power. Diagnosis confirmed camshaft/lifter damage requiring major repair. This appears to be a known issue with the 3.6L Pentastar engine and is documented in Chrysler STAR case S2309000013. Failure creates potential safety concern due to loss of power while driving. Vehicle is out of warranty, and manufacturer has not provided consistent support for repair despite known pattern failures.

I took it to Century Chrysler Jeep Ram in Mt. Airy, MD (Repair Order #XXX). They performed a 5-hour cooling-system pressure test and borescoped all 6 cylinders. Their notes state: “no coolant was found… no leaks… held pressure for entire 5 hours… could not find any leaks.” They replaced the spark plugs and performed a tune-up (total charged $1,606 after discount). Their own paperwork admits it “could be internal engine issue’s” and “will have to wait till gets worse.” They even wrote an estimate for a cylinder leak-down test but never performed it. When encountering a long gradual curve on the highway, the steering sticks and does not return to center unless a conscious effort is made to turn the steering wheel back. If this is not done the vehicle would continue into a state of over-steer. Although this typically happens at highway speeds (55-75 mph), it has occasionally occurred at slower speeds (35-45 mph). This has been reported to two different Chrysler dealers numerous times since I purchased it in February 2018 and neither acknowledged the condition. Each time both said that I am the only owner to report the incident. This has occurred in virtually every state along the I95 corridor from Massachusetts to Florida. 1) While in a gradual turn, with the steering wheel in the 10 or 11 o'clock position, and/or in the 1 or 2 o'clock position, the steering gets stuck and requires to over correct. This happens pretty much all the time and it is very scary at the moment. It feels like you need to yank the wheel so much that you might hit a car next to you. Apparently this is a very known problem with this 2018 Chrysler Pacifica. At the dealership, we have just been brushed off and told they cannot re-create the problem, but yet you can google search and see how many people have this problem. The vehicle is available for inspection. There are no warning lamps or messages when this happens. This has been happening since we bought the car in 2018. 2) Loss of power while in a turn. Quite a few times I have been making a left hand turn at an intersection and during the time I am accelerating, there is a couple second period where it feels as though I have lost power and am just coasting, but then the power kicks back in. I'm not sure if the car is not getting fuel to it, or if its power steering. It just feels dead, but there are no warning lights, messages or symptoms prior or during. I have not had this inspected yet, and it has happened a couple of times in the last couple of months.

I am experiencing a serious brake safety issue involving the brake booster on my Chrysler Pacifica. When braking, the pedal becomes extremely hard, pushes back against my foot, and requires excessive force to stop the vehicle. In some instances, when I am at a complete stop, the brake pedal pushes upward and the vehicle begins to move forward on its own. This issue causes a loss of braking assist and significantly increases stopping distance, creating an unsafe driving condition and risk of collision. The problem occurs intermittently but has worsened over time. Chrysler/Stellantis has acknowledged this brake booster defect by issuing a warranty extension; however, they have not issued a safety recall. I was denied repair coverage solely because my vehicle is now outside the extended warranty period, despite the fact that the defect is known and directly affects braking performance and vehicle control. This is not normal wear and tear. It is a failure of the brake assist system that compromises safe operation of the vehicle, whether driving at speed or stopped in traffic. The condition presents a clear safety risk to occupants and others on the road. I am submitting this complaint so NHTSA can investigate whether this defect warrants a formal safety recall.

My vehicle went in for recall 73B at the end of August 2025. Every since the recall it has been back to the dealer for multiple check engine lights, service hybrid system, abs system, battery heater fail, blind spot warning temporary unavailable, service shifter(would not go into gear), parking brake, stability control light, partial loss of power steering while driving, service electronic throttle control, and service airbag messages. The van has been to the dealership 4 times for these messages. They have replaced components, tried to find loose wires and connectors, replaced the 12V battery, and other fixes. The van will drive normally for a few weeks, and then they come back. On the latest visit (4th since 73B recall) it's been at the dealership for 31 days and counting. The dealership is waiting on Chrysler to figure out a fix for the van. The vehicle has over 50 stored messages. This recall was supposed to monitor the hybrid battery for early indications for a fire, but according to the original recall all batteries are assumed to have the manufacturing defect that leads to spontaneous fires. On June 14, 2021 (only two weeks after purchase), we had to drop it off for intermittent shifting issues with what seemed like skipped gears. On February 7, 2022 we brought the van to Chrysler after taking it in a few times to Carmax to resolve the the disabled "Start/ Stop" System. The light would intermittently come on indicating the system was unavailable, and shifting would be erratic. An inspection and recall work for the PCM was performed. In June of 2023, the UConnect System stopped working intermittently. The gears were still shifting roughly with the shift knob becoming increasingly more difficult to turn. An independent shop found no solution with an unknown timeframe on the delivery of a new transmission. In June of 2023 while driving, the van shut off completely while in motion.
