Phantom breaking while on autopilot. The car slows down abruptly with no vehicles or obstacles ahead. This is a new issue that started after a recent update. Also since latest update Autopilot has became more sensitive(in a good way) to situations that do require speed reductions. But, the phantom breaking is scary and frustrating.
2023 Tesla Model Y complaints
Owner reports filed with NHTSA about this vehicle, newest first and reproduced as filed. They are unverified accounts: one proves nothing, and a pattern across many is the thing worth reading. Back to the full 2023 record →
Component/System Failure: Brakes spuriously slam on when traveling at freeway speeds. Safety concerns: People behind me possibly slamming into me, in addition, my terrified reactions to an unexpected/unseen concern. Vehicle has not been confirmed by dealer or independent service Vehicle has not been inspected my manufacture, police, or insurance representative there is no warning as to when they may happen. first appeared in March 2023 and has happened about five times throughout the various months since then,
Two times in one week with two different drivers driving the same vehicle the vehicle experienced sudden unintended acceleration while parking. In the first incident this resulted in damage to the bumper into a wired fence which the vehicle did not alert but rather accelerated instead of applied regenerative breaking. Four days later the same incident happened with sudden unintended acceleration occurring while parking the car. When sending the car to Tesla for service due to the safety concern and damage this resulted in they said this was a user error which can’t be the case as this incident happened with two different drivers driving the same vehicle. When reading about this there have been numerous complaints which have shown that there is an electrical “ They show that the sudden unintended acceleration of Tesla cars may have been caused by negative voltage spikes that are logged as a fully depressed pedal, fooling both Tesla and the NHTSA investigators that there has been no hardware or software defect.”
The contact owns a 2023 Tesla Model Y. The contact stated while attempting to reverse the vehicle, the screen failed to display a rearview image, compromising the back over prevention system. After restarting the vehicle, the vehicle returned to normal functionality. No warning lights were illuminated. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 7,500. The VIN was not available.
I received this new car 11 days ago - it has none of its safety assist driving features and Tesla is not responding to my requests for assistance. I can't get a response to my requests for help. The clearly is absent of multiple points of functionality. Thank you
Driving on Highway 46 in California at a speeds ranging from 65 to 75 mph in both adaptive cruise control and autopilot when the car suddenly braked abruptly for no reason. There wasno traffic or obstruction. This occurred several different times on this highway in both directions and on different days during daylight hours with both light and moderate traffic.
Windshield cracked while driving on the highway The vehicle is not available for inspection as we have taken it to Tesla Service center to replace the windshield The cracked appeared in the top center. if it had happened on the driver side it could have resulted in an accident. The manufacturer is denying faulty product. The warning signs were there all along. About a few weeks back we could hear the cracking sound from the windshield. Since we could not pin-point the location we didn't think it was a faulty windshield.
The horn is not working properly. When pressing the left side horn button the horn does not activate. The right side horn button does work properly.
Power steering randomly goes out while driving. Most often happens after reversing, but also happens consistently while in drive. I was essentially unable to turn the car, so I was stuck in one spot and had to get out of the car so that the car could reset. This happened in several low traffic areas but also happened while I was at a red light at an intersection. The system did say “steering assist degraded” and that the cameras were blocked, but not sure if that has to do with the same issue. Never has been inspected, but I did see reports that this has happened to other 2023 Model Y and 3 owners. We have a service appointment with Tesla coming up to address it.
On August 12, 2023, I went to the Plano TX Tesla Delivery Center to pick up my new TeslaY model as scheduled at about 11 am. CST . The vehicle had 3 miles on it. After taking a couple of rounds around the delivery center and becoming comfortable with the default settings, I started driving back home which was 9 miles away. While on the drive, I noticed that the screen was showing “calibration in progress”. When I reached home at around 11:40 am. CST, Tesla showed a total of 12 miles on it, and as the garage door opened, the screen showed STOP as it said the calibration finished. As instructed, I stopped moving the vehicle on seeing HOLD/STOP while entering the garage from the driveway. Suddenly, the car jerked forward forcefully, as if a magnet had attracted it inside with the pedal going down. The car went into the garage and hit the garage wall, damaging the common garage wall and the other side being the master bathroom (by damaging the cabinets, big wall mirror(s), counter top etc), and also damaged the front bumper of the car. I was shocked and terrified for a while. So immediately rushed to the center again and reported the incident at Plano Service Center, who created a ticket on the same day. They said that this case would be sent to the engineering team for review. on Sep 10th, I got a reply from Service saying they have reviewed the Engineering study for the vehicle and the results displayed show that the vehicle operated as intended due to the use of the accelerato
Upon starting the vehicle, the center console displayed a fault code "UI_a020" and a message that steering assistance was disabled. There had been no earlier indication of faults or degraded operation - only minutes earlier, the car was driving fine. The vehicle was now very difficult to maneuver. Rebooting the car did not clear the fault code. I was able to navigate the short distance back home, though it was not a pleasant experience. Upon returning home, I scheduled an appointment for a repair with the local service center. Later, I checked my vehicle again, and found the fault code had cleared and it was operating normally. I still kept the service appointment, and the tech I spoke with mentioned they have had recent reports of faulty steering racks. They replaced the entire steering rack. (I do not have the faulty unit available for inspection.) This vehicle was only a few weeks old at the time of the incident.
I was traveling on VA 286N/Fairfax County Pkwy, prepared to take a left turn to Franklin Farm Road at 7:54am on October 5, 2023. 1. Left arrow were green for a while 2. When it turned to orange and red, the car infront of me slowed down and stopped 3. This is when I had the false and faulty forward collision warning 4. I stopped without any problem
While driving, the car suddenly lost power steering. We had to struggle to get the car to the side of the road. Had to do a reboot to get the power steering back.
I had three false forward collision warnings on October 3, 2023 - all the false and faulty warning incidents took place on slowed down traffic for the car travelling infront of me: 1. US Route 50, From Fairfax to Arlington direction, at 8:21 AM 2. US Interstate 66, From Fairfax to Arlington direction, at 8:25 AM 3. US Route 50, From Arlington to Fairfax direction, at 2:25 PM Tesla doesn't any resolution for the problems that I shared above, I reported them in the form of an email, as bug reports and as a service request
This faulty and false forward collision warning incident took place on US Route 50 from Arlington to Fairfax direction, in slowed down traffic for a vehicle that is in front of me having plenty safe distance on September 27, 2023 at 2:27 PM. Notice that there is no hard breaking recorded by the car right after this incident.
False and faulty forward warning collision incident took place on July 5, 2023 at Alder Woods Drive. This happened for a legally parked card on the street, the system labeled the car moving car infront as opposed to a parked and stopped car on the side of the street near the curbside. As a result it issued a false and faulty warning.
Right headlight and turn signal stopped working within approximately 110 miles on the odometer. Tesla did not make it a priority to fix the light and turn signal which poses a safety hazard to the driver and other vehicles on the road.
4 times on our 300-mile trip on I-90, where it is a 4-lane divided highway, our Tesla braked unexpectantly (phantom braking) at highway speeds in excess of 65 mph. There were no warning bells or chimes - just violent unexpected braking for no apparent reason. There were no obstructions in the lanes ahead of us and no apparent reason for the sudden braking. Fortunately, there were no cars immediately behind us. But it was extremely unsettling to have the car unexpectedly brake.
Phantom Braking occurs regularly when operating on Adaptive Cruise Control/Auto Steer (Beta). The car slows down to a crawl or comes to a complete stop unexpectedly at high speed on a freeway. Lucking there was no vehicle in the vicinity. There is no warning or notification on the car screen about this. Simply slows down for no reason. I have heard of several class action law suits concerning this as well as numerous users posting similar concerns online. I have not contacted the manufacturer in this regard since they are very difficult to reach.
On Friday, 9/29/23 at 2:24 pm, I was driving on the toll road (freeway) through the Chicago area. I was using my car’s adaptive cruise control. There was no car or other obstacle in front of me, and weather was clear. Abruptly, my car slammed on the brakes, causing an immediate deceleration. If a car had been behind me, I would have been rear-ended at freeway speed. This abrupt deceleration caused my head to snap back, leading to muscle soreness for the past couple days. I called Tesla shortly after, and the guy said he was able to determine there was something wrong with cruise control. He said if I chose to continue using cruise control for the remainder of my trip (Chicago to Minneapolis), I should be extremely careful. I scheduled a service appointment with Tesla, which they just cancelled because they said their remote diagnostics showed no problem with my car. They said the cruise control system is in Beta and therefore it can cause unexpected and unnecessary braking. This is a known issue for Tesla, many owners have complained both to Tesla and to NHTSA. I could have been seriously injured or killed if I had been rear ended at Chicago freeway speeds (i.e., fast).
While driving on the highway, I had set the cruise control to max 65 mph. After some time it suddenly set the cruise control to max 35 mph and suddenly reduces the speed even though all front vehicles were running around 65 mph. It was incredibly dangerous and the vehicle behind me was about to hit.
I was driving my car, and the steering wheel locked while driving and pushed me into the oncoming lane. We were lucky there were no other cars coming the other direction. I had to reboot the car to get it to drive again. The steering locked up again while getting the car home and also in the driveway. Tesla towed the car and replaced the entire steering rack of the car. I then saw an article about this exact issue and wanted to add my car to the other complaints. My car is only six months old.
I was driving at a very slow speed of 1 mile per hr as I entered into the Kohls complex in Pleasanton ca. Then as I was pulling the car to a parking spot, all of a sudden I felt the car accelerated on me. My foot was always on the break and I pressed the break but the car still accelerated at a speed I had no control and it went over the parking bump and hit 2 cars that was parked opposite. I felt out of control and scary because the car was not doing as intended, to stop when pressed the break. I then pressed the park button. Then I also reversed because the car was not stopping. There was totally 4 cars damaged including my car. 2 opposite and 1 parked side of me. I could feel the sudden acceleration in the car and out of control feeling. Luckily there were no people in other car so no one was hurt. I was also not hurt. - Not sure what malfunction happened , but the break in that instant of time was not acting as it should for few seconds. Yes, the car is available for inspection - My safety was at risk as the car was no behaving the way it should and I got scared as to what would happened next. Since there were no people in the other car, there was no risk. but if there were they probably would have got hurt. - I reported the issue to Tesla center. They said they will look but never heard back anything from them . - None of them has inspected the vehicle yet. - No warning signs were there. after it hit i could hear the beep
Steering felt odd and was pulling to the extreme right, which caused contact with a curb. After driving about a mile after contact, car lost steering and both front wheels were turned inward. Tesla claimed contact with curb caused the steering gear and rack to break-they did not take into consideration that car steering was not workin correctly before impact. Component is not available for inspection. Safety was a factor since I couldn't steer. Has not been reproduced or confirmed by an independent service center. Component not inspected by anyone other than Tesla service center in San Francisco, Ca. No warnings re:steering or any other issues.
3 day old Tesla y coolant warning light came on, went to the service center and car has been there for 24 hours no update and now they extended to another 24 hours no reason given.
My wife pulled into our driveway at 5 mph. Suddenly our Tesla Y. Auto accelerated, before my wife could realize it ramped into our garage through the door and collided on the wall of our Laundry room and kitchen damaging Foundation of the laundry room.My wife was also injured badly. This incident has shaken my wife. We complained to Tesla and filed the complaint but they are not even following up. Please investigate this issue
Phantom Braking of vehicle when using either TACC or Autopilot on highways. Vehicle will suddenly slow down/apply brakes when traveling at highway speeds. Unsure why vehicle is doing this. Sometimes it looks like the vehicle thinks it see's a person in the highway? There are never any obstructions.
The Tesla Model Y autoaccelerated even with me pressing the brakes. The car was on regenerative breaking and instead of stopping it accelerated lunging into the walkway and almost killing a person. The car hit another car, 2 bike racks and a tree as well as concrete slabs beofr coming to a stop. There have been reports of unintended auto acceleration from Tesla cars. The car components has not been inspected by police, manufacturer or insurance representatives. There were warninngs, especially with the car reporting that sensors were not properly calibrated. I reported this to Tesla agents and they told me not to worry about it. I am injured with a left wrist sprain as well as Neck and back injuries. I need Tesla to take ownership and pay dearly!
The contact owns a 2023 Tesla Model Y. The contact stated that while driving at an undisclosed speed, the steering wheel became stiff and difficult to maneuver. The steering assist warning light illuminated. The contact veered off the roadway. The contact notified a tow truck company, and the vehicle was towed to the dealer where it was diagnosed that the steering rack had failed and needed to be replaced. The vehicle was repaired; however, the contact was concerned for her safety while driving the vehicle. The manufacturer was not notified of the failure. The failure mileage was approximately 1,400.
I lost all power in my steering and was completely unable to steer. I contacted Tesla who advised me to conduct a soft reboot and the issue was resolved only to return about an hour later. I contacted Tesla again for roadside assistance. I was advised to complete a soft reboot and drive the vehicle to the nearest service station. I refused to drive the vehicle, as I did not feel safe to do so. Tesla sent a tow truck to pickup the vehicle and drop off at a Tesla service station. After 3 days, I was informed there was a malfunction in the vehicles steering rack and the entire part will be replaced. I conducted research and found a lot of owners are experiencing the same issue with 2023 Tesla Y, some even having crashes as a result. This is very unsafe and needs to be addressed.
It’s just been 2 weeks in I bought my Tesla Y 2023 model. Was exploring entertainment options in in park mode and then when I had to leave, I see this steering assist reduced and wasn’t able to move my steering at all, I was like what.. a brand new car having such safety issues, thank god didnt happen while I was on road, would have definitely killed me. Tried rebooting or reset but still the same. After multiple attempts, power steering came back on and I was able to get home. After reaching home immediately requested a service through my Tesla app. This is very frustrating to see issues in a brand new car.
I went on vacation with my wife and [XXX] son to Canada on this vehicle. On August 27, 2023 around 1700 hrs while I was on highway-40 from Montreal to Quebec city the car screen suddenly displayed “ Car is shutting down , immediately pull off to a safe area, emergency brake disabled”. Battery showed 59 % charge at that time. I managed to pull the car at intersection between highway and an exit before it totally stopped and became non-responsive in less than a minute. I started communicating via text with TESLA help line after attempting to reboot the screen which did not resolve the issue. While this communication was going on vehicle got badly rear ended by a passing car. We were all wearing seat belts and thanks to that we juts had mostly bruises but my son suffered agonizing psychological trauma and kept on crying incessantly for long. He also had an injury on right knee from a piece that shattered from the rear door which also got jammed after the impact. TESLA helpline said since accident has happened they will not be able to assist further, rest of the matter should be sorted out with my insurance. Some good Samaritans stopped the traffic for us and helped us leave the car immediately to the outer border of the exit road fearing that battery may catch fire as a result of impact burning everyone down. The car was later towed to a nearby facility arranged by Police and declared totaled by my insurance. It was some kind of electronic glitch which made car hung up like a co
While driving with autopilot/cruise control, the car started braking heavily with no obvious obstruction on the road. This happened 5 times over 2 days of long distance driving.
The steering wheel locked and stiff, wasn't able to turn. The was very dangerous as my wife and kids could have gotten into a serious accident. We are no longer comfortable with driving this vehicle even if Tesla repairs so we would like a refund
Steering assist reduced error message. All of a sudden I was unable to steer the steering wheel. This was scary and dangerous.
I was driving and the steering wheel completely locked up, I could barely turn the wheel even when using all my strength. The car would only go forward and backward. I was fortunately pointed slightly towards the curb so was able to glide the car to a stop on the side of the road. There were no warnings prior to failure and I immediately lost all steering capability. The car was towed by Tesla to the service center.
During the late afternoon of Saturday, Sep 9 on my way to Jackson, WY from Dallas, I was driving northwest on US Hwy 287 approximately between Vernon, TX and Clarendon, TX and experienced almost a dozen severe phantom braking incidents in the span of 1 hour. My windshield was clean, the weather was sunny and hot. The only thing I could see on the roadway was surface heat reflection/mirage (when the road surface gets hot and you see a mirror-like reflection). I did not experience this issue again until late afternoon on Friday, Sep 15 on US Hwy 87 between Raton, NM and Capulin, NM when I experienced 3 severe phantom braking incidents within a 10-minute timeframe. I recorded on video the 3rd incident and disengaged Autopilot/TACC for the remainder of the day. Video avail upon request.
Tesla Model Y Emergency Braking applied on a freeway while driving with Enhanced Auto Pilot putting me and my family at high risk when there was no emergency whatsoever. My lane and the lanes either side of me were free and there was no traffic.
Vehicle visualizations degrade over a few days following a firmware update and eventually show nothing on screen. Features such as auto wipers, traffic-aware cruise control, auto turn signals and blind spot indicators do not function. There is no warning to the driver in the car that these items are not working. While driving I am unaware if other safety features are not working (emergency braking for example). Manufacturer admits this is a firmware bug and has promised a repair multiple times. The service team has provided a temporary fix to manually reset the self driving computer each time this happens (approximately every 3 days) which requires a recalibration of the cameras rendering these features useless.
The contact owns a 2023 Tesla Model Y. The contact stated while driving 65 MPH with the Autonomous Self Driving feature activated, the contact briefly lost control of the vehicle when the steering wheel suddenly started to wobble, and the vehicle swayed from side to side. No warning lights were illuminated. The vehicle was not taken to the dealer or an independent mechanic to be diagnosed or repaired. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 975.
Power steering went out twice, both from changing gears once was mid drive second time was going from park to reverse.
While Driving the car the steering became unresponsive, this almost created like a death situation for me. Tesla should be held accountable.
I was driving through a busy nyc street very slowly. Probably 10 mph or less. I was on a red light. then i moved forward when the light turned green, my handle felt weird and the car was moving side ways. I first thought it was the tire that blew off. So I immediately stopped the car and had my husband look at the tires. No holes, no pressure was lost. I thought it was so odd, so I started moving forward a little bit, again. Something felt very off. We stopped again and looked throughly and realized the right rear wheel was twisted while the other three remained straight. This is our brand new car that was just picked up 7 days ago. We just simply couldn’t believe our eyes. My whole family including my husband, my two kids (7,10), our dog and myself were in the car and I can’t imagine ourself being in the same situation if we were on a highway at a faster speed. We’d probably dead by now. I didn’t hit anything, i was going 10 mph, there was nothing on the street. And the wheel getting twisted like that just doesn’t justify anything but a defect.
- Failed system: Steering wheel was locked up in the middle of driving. Driver was making a right turn at an intersection. Suddenly a "Steering Assist Reduced" UI_a020 warning appeared on screen, and driver was unable to turn the wheel. No autopilot in any level whatsoever was turned on. - Safety risk: Driver hit another car due to the loss of control of the vehicle. Thankfully it happened in a city intersection. If it happened on a highway, much more damages and casualties might have been incurred. - Reproducing the problem: Right after the accident, driver was ordered to pull the car over to the side. But even after a few restarting of the car, the UI_a020 warning reappeared sporadically. From that time we completely stopped driving the car and drove it only occasionally to load/unload it to tow trucks. So we did not have much chance to reproduce the problem. We have some photos and video clips of the UI_a020 warning taken on the date of the accident. - Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others?: Not yet. Currently the vehicle is at a Tesla Approved body shop (collision center) for repair. - Any warning lamps, messages or other symptoms of the problem prior to the failure: There was no UI_a020 warning that we recall prior to the time of the accident. This could be unrelated to the UI_a020 issue, but one day before the accident, I was trying to unlock the car using my cell phone, but the car did not recognize
I bought a brand new tesla, only to find out that it was not emitting any sound slowly driving forward, but only when in reverse. I read through the cars manual and there it says the car should make a sound moving forward under 19mph, to warn pedestrians of the oncoming tesla (my car). If the car does not do that, then to immediately contact Tesla service. I did that and Actually so was there at my Tesla service station in Burbank CA in person and they said to me, I should not worry what their own manual for the car says. The car is not to make a pedestrian warning sound forward, but only when in reverse. To be honest, that seems wrong to me. Please advise. Thank you for your time and consideration. Kind regards, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Steering wheel locked up while driving and making a turn
Driving west on John McCain road (after turning off of I-26 S) in Colleyville, TX. Numerous times, with no oncoming traffic, no cross traffic and clear conditions / visibility PHANTOM BRAKING caused speed to drop (while on cruise control @ ~35 MPH) down to 18-19 MPH. There was NO REASON for it to brake / stop and there was a car behind me, which was forced to stop. I'm sure they thought I was crazy / brake checking them. Clearly the Tesla Adaptive Cruise / Front Sensors (uses only cameras on my model now I believe) caused the braking for now reason. This is a huge issue.
Lost all power steering today on model Y 2023 tesla.
I was turning right from a residential street onto a busy street during rush hour. I heard a sound, which made me think I might have run over something. Immediately following the sound, an alert showed up on the car’s touch screen with the message: “Vehicle May not restart: Service required”. The car was immediately inoperable - the car could not move and it could not shift into neutral to be pushed off the road. The 12V battery of the car continued to function for roughly 10 minutes after the car became stuck, which I knew because I was able to exit my vehicle normally, the touch screen remained illuminated and I was able to turn my hazards on. It did eventually die and I became locked out of the car at that point. I was in the middle of a busy intersection with no hazard lighting at that point and the car could not be moved, so I called emergency services and requested a police officer for traffic redirection while I waited around an hour and a half for a tow. Regarding the initial sound that I heard, I checked under the car as well as in the street around it, and there were no signs of debris or any visible evidence of damage to the undercarriage of the vehicle. I can only assume that the sound that I heard was whatever failure that had occurred internally in the car. At the time of this incident, the car was charged to around 85% of battery, was only a few weeks old, had never been in any type of collision, or required any service. Tesla has inspected the vehicle and iden
The contact owns a 2023 Tesla Model Y. The contact stated that as his wife attempted to park the vehicle in their garage, the vehicle independently accelerated without warning and crashed into a concrete wall. The air bags did not deploy upon impact. The contact stated that his wife suffered from neck pain for a week after the accident but did not seek medical treatment. A police report was filed. The garage was damaged as a result of the accident. The vehicle was towed to an independent body shop where the vehicle was deemed a total loss. The manufacturer had yet to be notified of the failure. The failure mileage was approximately 19.

NHTSA publishes every complaint as the owner wrote it and investigates only a fraction. Spelling, capitalisation and judgement are the filer’s own. What carries weight is repetition: the same component failing the same way across many filings, and whether the manufacturer eventually opened a recall for it.
