Multiple times while driving across I-70 through Kansas, my car suddenly broke and slowed down at least 10 mph for no reason. There was nothing in the road. One time it even identified a mile marker as a motorcycle. I was almost rearended when it hard broke three times in a row and threw myself and my children forward forcefully. I realize this is an issue under investigation, but until people like myself continue to complain and wonderful folks like you force Tesla to make changes, it is going to continue to be an issue. It's not safe. Thankfully I WAS in the middle of Kansas every time and there was no one around (except for the above mentioned incident where the van behind me was gracious enough to pass and could clearly tell I was having difficulty with the car). If this happened in a crowded area, I for sure would be in a high speed collision. After it happened on my outbound trip, I tried to make note of it on my trip home: what was around me, what the camera was "seeing" on the screen, etc. I noted already the motorcycle instead of a sign. It also seemed to have real difficulty when I could see a mirage and reflections (as it was over 100 degrees out) but I genuinely don't know if cameras can see mirages or if that is only human eyes. Thanks for your help in pursuing the issue!

I took delivery of a Tesla Y in February 2022. About a half dozen times since, I've been driving on the highway using cruise control/auto pilot and the car will randomly slam on it's brakes, so hard that loose objects in the car are flying to the front. There was no warning prior or any of these incidents. Thankfully I have not had cars directly behind me when this occurred...if there had been someone behind me they would have absolutely hit me. I have not yet contacted Tesla.

The "advanced autopilot" feature is extremely dangerous in certain conditions especially construction zones, if a bug is on the windshield, while it is raining or night time without street lamps. I purchased this car as a full time rideshare driver for nearly $100,000 after driving the car for less than 1 month the car has terrified myself, my passengers and my pregnant wife and luckily hasn't killed any innocent bystanders, yet. While in autopilot the car will frequently 'phantom brake' when there is literally no vehicles in front of it, quickly reducing speed from 70mph down to 40mph. This most recent time 7/28/2022 I had a garbage truck like vehicle behind me but thank God I have excellent reaction time and canceled the autopilot and smashed the accelerator just in time before causing a pileup on the expressway. The car constantly misreads speed limit and street signs and will confuse the SR46 little sign for a speed limit sign and slam on the brakes from 65mph down to 46mph. Just yesterday the car decided to go in the middle of the two lane highway and slam on the brakes in the middle of the highway, twice, which terrified my wife and I caught on my 3rd party dash cam. I have tried to reach out to 3 Tesla emails and even SpaceX but I receive no response, big surprise. Additionally I'm smarter than the average bear and use a 3rd party GPS miles tracking app on my iPhone called Everlance which when I've cross referenced with my Tesla app about driving range its not even rem

We have owned the car approximately 2 weeks and have had 3 episodes of inappropriate activation of emergency braking. The first two were minor slowing of the vehicle when traveling on interstate highway when the system was activated by dips in the road. The third involved a parked pickup truck on the shoulder of the outside curve of a road - resulting in aggressive braking of our vehicle. Frightening. Thankfully there was no closely trailing vehicle. We have inactivated the system, notified Tesla, and scheduled a service appointment.

While using cruise control (Traffic-Aware Cruise Control) on a recent road trip in our Tesla Model Y, the car unexpectedly and for no apparent reason, abruptly reduced speed. Our car is available for inspection upon request. Our safety and that of others was put at risk as a result of this unexpected reduction in speed while travelling on the highway. An accident could have occurred if cars travelling behind us did not similarly reduce speed to avoid a collision. The problem has occurred on a number of occasions where there was no apparent reason for the car to rapidly reduce speed. It has not been confirmed by a service center. The vehicle has not been inspected by the manufacturer or others. It is a known problem with the 2022 Model Ys based on a published inquiry by the NHTSA (ref: https://electrek.co/2022/06/03/tesla-respond-increase-phantom-braking-complaints/). There were no warning lamps or indicators prior to the unexpected braking event.

In accelerating of itself, both my own life and that of a potential bystander were in danger. I had no time to react to the sudden acceleration. This has never happened to me in over fifty years of driving and it was very obviously the result of a car malfunction. In addition, the automatic emergency braking function failed. There were no previous messages, symptoms, or signs. The car has been at the shop for three months. The dealer has failed to give us any feedback as to why the accident happened. All they have said is that they don't have the parts that they need to change. It was only after two and half months that they reached out to me. During one of my visits, one of the dealer's service representatives took me aside to inform me that she could not divulge any information about the causes of the accident, and that my case was a “legal one”. I still don't know what any of this means. In addition to all this, the pen drive located inside the glove compartment of the vehicle only has two days of picture and video information, and the file of the day of the accident is empty, which is extremely suspicious. I suffered burns from the airbag, and my cervical region was affected.

Front trunk (hood) flew open while driving at approximately 40mph. The front trunk lid/hood opened completely and smashed into the windshield, completely blocking driver visibility and breaking the glass. It is unclear as to how the front trunk became unlocked, but in the event it was unlocked while the car was in park, the car should not have been able to shift into gear. Tesla Model Y's Owners Manual states: "If the front trunk is not closed, a message displays on the touchscreen that you cannot shift the vehicle out of Park until the front trunk is securely closed (see Park)." Additionally, the Owners Manual states: "Model Y will not shift out of Park if the front trunk is not securely shut" In the event the trunk was open prior to driving, the car should not have been able to shift out of Park. Or, if the trunk opened while driving, there was a latch or electrical problem. Tesla's representative reviewed the incident log from the vehicle and stated that an ignore button was pressed on screen, once the vehicle indicated that the front trunk was open. Quickly pressing a button while driving to a safe place to pull over is reasonable. But, regardless, there should be some sort of fail over that prevents the hood from flying open in the event the front trunk is accidentally opened, or in the event the trunk randomly opens on its own. Perhaps a secondary latch or disabling the vehicle from driving? The hood flying up, smashing the windshield and completely blocking the drivers

While driving on highways /interstates and using the vehicle cruise control or autopilot, the vehicle will suddenly apply the braking system when there is nothing in front of it. I would list specifics, but it happens every time that we take the vehicle on the highway/interstate, usually within the first 15 minutes. This has happened multiple times on trips, at 80mph (South Dakota) or at 65mph. I have noticed that white vehicles tend to trigger it more than others, even if they are just sitting on the side of the road or parked in a driveway off of the highway. Hills also seem to throw it off, when you are at the top and just over the crest, it has applied the brakes multiple times. This has gotten progressively worse since I've owned the vehicle (Dec 2021) which is why I am only now reporting it. I will no longer use it as I firmly believe me or my family would be seriously injured or killed if there is a vehicle behind me, especially at 80 mph that was not expecting the car to brake hard and as a result rear ended me. This is a significant vehicle vision system issue and it really does need to be addressed before lives are lost, if they have not been already. There are no other issues with the vehicle, no warning lights on and absolutely no indication the vehicle will apply the brakes until you are suddenly lurched forward due to rapid deceleration. I understand this is already on NHTSA radar but believe this is a much larger issue than is being reported.

Model Y will repeatedly break for no reason while using Autopilot. This seems to happen most often less busy and smaller highways. The behavior makes Autopilot extremely unsafe to use when traveling at high speed with other vehicles behind you. There’s absolutely no reason for the sudden breaking with no cars ahead on the road. Shadows, dips in the road, cracks seem to be registered by the car as obstacles.

My 2022 Tesla Y brakes suddenly when I am not expecting it to. This has happened many times on the freeway in traffic at high speed and often with large trucks behind me. I am concerned that I will get rear-ended by a much larger vehicle. This unexpected sudden braking has also occurred in city traffic. My car does not have full auto pilot- it has the standard driver assist that can't be removed from the car. Over the last few weeks I've been on several long road trips. During this time I've experienced "Phantom Breaking" (Breaking that I didn't cause) - abruptly. Very strong breaking at time. It happens around once every 40 to 100 miles of driving. It happens when using Cruise Control or when using Enhanced Autopilot. It seems extremely dangerous at times and has caused at least one care behind us to swerve into a passing lane.

When I was on a road trip I had my 2022 Tesla Model Y in autopilot and with out warning it gave myself and my passengers whip lash because it brakes for no reason. On the course of my road-trip it happened more times than we could count. One horrific time was when a white Cadillac Escalade was tailgating me the Tesla threw on the brakes heavily and almost caused the car behind me to hit me. This is very dangerous and the other driver started threatening me and my family harm. I need Tesla to fix this issue with a great sense of urgency. This is extremely dangerous and in my case we were lucky to not sustain any physical harm or death but the chance was and is still great if they do not fix this issue with autopilot braking randomly not for just myself and my family but countless of other Tesla owners that are affected by this scary unsafe issue. Random braking occurred again on 07/05/22 while on a highway with no traffic. This time consumer's model y was on cruise control. Consumer stated he hardly uses autopilot feature because of his past experience. But continues to do this just on a basic cruise control.

The contact stated while driving approximately 65 MPH, the computer inadvertently shutdown and the steering wheel was difficult to maneuver. The vehicle then drove off the road, and the contact drove over a grassy area and the curb. The TPMS warning light illuminated. The front axle was completely fractured. The police were not initially on the scene; however, the police arrived the following day. A police report was not filed. There were no injuries sustained. The contact attempted to have the vehicle towed several times, but was unsuccessful; however, the vehicle was finally towed to the dealer. The vehicle was not diagnosed or repaired. After the contact consistently discussed the failure; he was informed by the insurance company that the vehicle was deemed a total loss. On July 2nd, 2022 I was driving back from a family visit in Palo Pinto County, TX and had auto pilot engaged for part of the drive. In a span of 40 minutes of that drive on a continuous stretch of Interstate 20 headed east to Dallas the car softly automatically braked once, and twice more strongly braked. In one of those instances I was near another car (near is defined as within 1/8th to 1/4 of a mile) and in both of the other instances I had NO one else within close proximity that could've caused a "reaction". That stretch of i20 is 2 lanes each direction with a median in between, no oncoming vehicles are within at least 75 - 100 feet. In the instance with the car closer, had I not been more reactive and paying attention closely, this WOULD have lead to an accident with another vehicle. In the instances with no traffic IMMEDIATELY near me.. given that rate of speed, it could STILL lead to an accident by over-reaction from others not expecting to see someone slamming on their brakes in a 75MPH zone. Tesla has been extremely unhelpful, their remote diag team says the system is "operating normally" and will not deal with the situation further.

2022 Tesla was in auto-pilot mode, not FSD. Phantom breaking occured on July 6 in Sacramento on Business 80, two incidents about 20 minutes apart. First incident, vehicle suddently slowed from about 65 to 30. Second incident, vehicle suddently slowed from about 65 to 10 (almost stopped) where we almost rearended. The driver was extremely angry as evidenced when he changed lanes to pass us (can't blame him), as Tesla began to reaccelerate. I have heard of phantom breaking incidents, but twice in one short trip where the second had the potential for a serious accident would indicate to me that Tesla needs to take action. I hope you relay this to them.

Braking system applied brakes while on the interstate going 75+ MPH no reason to brake, happens quickly on most instances, I hurry to hit the accelorator to speed back up to speed as to not cause any incidents. First several times it is very uncomfortable to experience, causing concern to be easily hit from the rear in areas with more traffic. The issue is not reproduced as it seems to happen when the car "thinks" it has a reason to brake, different enviromental changes such as cars, in the other lane, or shadows seem to have an affect as well. NO inspections have been completed at this time. No warnings seem to be triggered when this takes place, just rapid unexpected deacceleration.

I was traveling on the highway in Indiana between Bloomington and Indianapolis. I started to experience the car braking for no apparent reason. As I continued on my trip and reached a top speed of 75 mph, the car suddenly and without warning slammed on the brakes! It took me a minute to react and regain control of the car. I absolutely would have been rear-ended if anyone had been nearby or possibly caused a chain reaction accident. I am extremely lucky and now very remiss about purchasing this car. I luckily had a passenger who looked up what was happening. We turned off the emergency braking for the remainder of the trip but it is not something you can turn off permanently. You have to turn it off every single time you start a new trip. I wanted this incident reported so Tesla is forced to deal with this issue immediately.

Several times, after over the air software updates, several components of the electrical systems would stop working, including GPS & cameras, resulting in no cruise control, no back up camera, no navigation, no blind spot warning/heads up collision warning/lane departure warning. Essentially, all safety features, cruise control and navigation fail when GPS and cameras fail. In all cases, the reset options provided such as powering off the carf, powering off the screen etc did not help and instead the 12 V battery had to be disconnected, sometimes more than once, before car would reset to a safe state. this requires removal of a panel, removal of cabin filter, removal of cable to battery. This is a time consuming process and could theoretically void warranties. We are waiting for the next software update to see if replacement of a control board by Tesla this week fixed the issue.

When using cruise control (with or without auto steering to stay within a lane), the car randomly breaks. This does not appear to be associate with signage along the road. It appears to happen more frequently in the right most lane. There are certain locations where this problem always occurs, but we cannot determine what is different about that particular exit than others. The most dangerous situations have been when the breaking occurs with a car closely following while driving on an interstate in the mountains (I-70). A relative driving my car was almost rear ended at those speeds while driving on I-70. The breaking is random and unexpected, which makes it more dangerous because as driver you are not prepared for this to occur. At a speed limit of 65mph, when you are driving in a curve and suddenly the car brakes, especially when using regenerative breaking, it is the same as slamming on the brakes. This could cause you to loose control over your car. The issue has been known to Tesla but has not been fixed and new cars are advertised and sold with adaptive cruise control when in fact they are faulty, rendering the feature useless at best, dangerous in most cases.

On Saturday, 25 June, while driving our 2022 Tesla Model Y north on highway 395 between Lone Pine and Bishop, California, and while cruise control (but not lane assist) was active, I experienced three instances of so-called phantom braking. More precisely, on three occasions within a 10 to 20 minute window, the car automatically braked sharply despite there being no vehicle or other obstacle ahead of me. Each time, the braking was sudden, forceful, and without justification. Had a car been following closely behind me, I could well have been involved in a collision. I stress that my foot was not on the brake pedal when the phantom braking took place. After the 3rd episode, I turned cruise control off and drove another couple of hours (to Mammoth Lakes) without incident. The following day, my wife and I drove the Tesla to our home in the Bay Area. We did not activate cruise control at any point during this approximately 6 hour drive, and experienced no additional incidents. Understandably, we are reluctant to re-attempt the use of cruise control any time soon. I have owned this Tesla Model Y since January of 2022, and these were the first instances of phantom braking.

Turn signal does not always activate. If I hit the turn signal stalk so it goes past the first detent (for three blinks) but not hard enough to get to the second detent (on until cancelled or turn completed), it does not come on at all, either the three blinks or constant blinking. This is dangerous because the turn signal does not activate predictably, especially when changing lanes on the highway. I should be able to tap the turn signal to get the 3 blinks to come on and then switch lanes, but when used this way it does not always come on. I had this inspected by a Tesla Mobile Service technician and he reproduced what I reported but said this is working as intended because it is detecting an "accidental bump" and not activating. My vehicle is available for inspection upon request. There are no warning lights or messages.

Insurance deemed the car as "totaled". We were reimbursed by insurance so this is not about monetary losses, but a report to protect other drivers and their families. The Tesla database records mentioned user acceleration and error, but this doesn't sound like a reasonable cause. This is a recurring problem and has happened at least a dozen times since I've owned the vehicle. I no longer trust the feature and hesitate activating it. The danger is risk of a rear end collision if another vehicle is close behind when the problem occurs. I have read that this "phantom braking" issue has been reported 700+ times. Auto dimming and bright headlights do not dim for oncoming traffic consistently. Unsafe for other travelers at night. I’ve owned two Tesla’s model 3 and model y over the last 4 years. Both had issues with the Auto Bright/Auto dimming headlights. The automatic feature takes too long to turn on the high beams and takes too long to dim for oncoming traffic. The auto dimming function also reacts to reflective signs and turns off high beams when unnecessary. The auto dimming feature works best when you’re approaching a curve where the travel path is to the right where oncoming traffic crosses the field of cameras. Auto dimming rarely works when turning to the left or straight for on coming traffic since cars are to the left of field for the cameras or head on traffic but Auto dimming feature fails. The bright and dim will go on and off as cars approach further causing issues with people flashing lights at you in their direction of travel. The auto high headlights also react to reflective signs and will go high low high low further causing safety issues when they should remain on.

Purchased a Tesla Model Y approx. 3 weeks ago and within the first day experienced sudden, aggressive braking on the freeway with nothing around and no apparent reason for braking at all. This has happened at least ten times in the three weeks we’ve owned the vehicle. It’s very scary when it happens and we’re very concerned it will cause an accident with my family and small child in the back. It happens when the car is in auto pilot and adaptive cruise control. The car gives no alert and no indication that it’s going to brake hard all of a sudden in the middle of the interstate at 70 mph it will instantly drop speed by as much as 15mph or more. Thankfully nobody has been behind us yet. For us this is an extreme safety concern and we have expressed this to Tesla and I have sent out a maintenance technician today who took it for a simple test drive and said there’s absolutely nothing wrong with the car and that’s the safest car in the world and that what we were experiencing is normal behavior for a Tesla and to be expected. He said when it happens to stereo on the accelerator to get it to stop which is ridiculous. This is a known problem generally referred to as phantom braking and I was assured by the sales rep before accepting the car that this doesn’t happen in auto pilot and nothing to be concerned about. Now here we are stuck with a dangerous vehicle that they can’t fix and won’t take it back. Please help get these cars off the road until they fix this huge safety issue.

I was traveling on a highway when my Tesla Model Y abruptly slammed the brakes for no reason while Autopilot was engaged. The vehicle slowed down very aggressively, this could have caused an accident if there was another vehicle following close behind me. After the first incident, I blamed a dirty windshield/camera lens blocking the systems ability to see what was ahead. But it happened two other times on different days. At this point I was convinced that there is an issue with Tesla's Forward Collision Warning system. On a different occasion, I was driving under an overpass and the shadow on the pavement (overpass shadow) triggered the collision warning and once again aggressively applied the brakes. It is very unfortunate this is happening on this vehicle. I love my Tesla Model Y, but I am very hesitant to use Autopilot due to these issues. I personally like Tesla because they are using advanced technology to make driving a lot safer for everyone on the road. However, it is our responsibility to report these issues so the manufacturer can investigate the root cause. I hope this comes to a solution soon because it is a very dangerous situation that can cause harm to me and my family as well as others on the road. I will point out that I have been fortunate there were no close vehicles in behind me during the three separate times this occurred.

On 5 or 6 occasions while driving on the highway, using adaptive cruise control, the car has braked with no traffic anywhere around us.

NHTSA publishes every complaint as the owner wrote it and investigates only a fraction. Spelling, capitalisation and judgement are the filer’s own. What carries weight is repetition: the same component failing the same way across many filings, and whether the manufacturer eventually opened a recall for it.
