The high-beam headlights are randomly turning on while city driving. The auto high-beam function is turned off, so high beams should not turn on. This affects the safety of the other cars around me. I typically drive in the city and using the high beams is not necessary because of dense traffic and available overhead street lights. This issue has not been confirmed by a dealer, service center, manufacturer, police, etc. There is no warning before the high-beams turn on. When they turn on, the blue high-beam indicator on the driver display does illuminate.

After leaving the vehicle plugged in and coming back to it after an extended period of time, The vehicle’s camera is stuck on black screen with spinning circles and gps stuck on last location. Only resolution is to walk away and the vehicle goes into a state of deep sleep. This is a major safety issue because the rear windshield is smaller than a typical standard vehicle, so without the cameras, visibility in the rear is almost non existent especially during inclement weather. Vehicle was brought to service several times to address the issue. Today will be the third attempt to remedy the issue. Issue is ongoing and has been existent since vehicle possession.

Tesla was in "full self-driving” (FSD) mode in a quiet residential area. Vehicle chose to make a left turn on a less than perfectly symmetric T junction. The car was on the thru street at the top of the T. Street had a middle yellow turning zone with yellow lines on the outer side and dotted ones inside. Camera was not filming at the time the crash began. but did start recording midway. Tesla may have other footage. In retrospect, while indicating it was intending to turn, I believe the car never entered the middle yellow zone, but instead proceeded directly to the middle of this unusually wide neighborhood intersection where no other vehicles were close by in any direction, although a few were approaching from a distance. It appeared safe to complete the turn, but instead the car feinted forward several times and hesitated to commit to the turn as cars gradually approached from both directions, but were still a safe distance away. As I waited for it to turn, the vehicle suddenly accelerated at an alarming rate into the turn, so fast that it slid off the road and over the curb into a wall causing extensive damage to the vehicle. There were no other cars involved and I was not injured, although the car suffered extensive damage and is still being repaired more than two months after the accident. My foot was definitely not on the accelerator throughout the accident and everything happened so very quickly that I had no time to brake or take other action. The oncoming car was qui

With auto-pilot engaged the Tesla came to an abrupt stop on an Interstate. Possibly this contributed to the Tesla being rear ended. There was another vehicle stopped ahead of the Tesla so stopping was the correct action but the Tesla auto-pilot braked abruptly. There was adequate distance between the Tesla and the car in front of it when it stopped.

"Full Self Driving" without explanation stopped working while I was using it. I tried to park the car, walk away, lock it; I even rebooted the car by holding both scroll wheels on the steering wheel as I've been directed in the past. Nothing worked. I noticed during my shift that "Auto Pilot" still worked on the highway so I was using it for a couple miles successfully. Then when the time for the exit came the car took the exit normally but then when it switched to "FSD" the car stopped steering and drove me directly towards a drainage ditch next to the off ramp and all steering assist and lane keeping was turned off all at the same time (while traveling at 50mph on I4). I was really scared luckily I never took my hands off the wheel but anyone with lesser reaction time would have crashed and possibly died. I got this on dashcam, pulled over to calm down (McDonalds near MCO), then drove back to prove it happened with the iPhone as Tesla has directed me to do on many occasions to "prove it". So I got it all on video you can see on this link here since NHTSA only allows low quality short videos less than 10mb like its the 1990's. The more I report Tesla and tell my riders about the child slaves in the Congo mining the cobalt in the batteries the more frequently the car tries to kill me. This is a cover up not a coincidence. https://www.dropbox.com/s/ksryabtxo953qz5/Video%20Jan%2024%202023%2C%201%2040%2043%20PM.mov?dl=0

At the end of my shift the "Full Self Driving" started working again, also without explanation. This time its driving me home, gets to the neighborhood I drive in and out of multiple times a day for the entirety of my "FSD Beta" and only address the car has "lived" at. This time it SLAMS on the brakes with someone behind me (scary) then quickly takes the wrong left turn, then another wrong left turn, then starts the ACCELERATE in a neighborhood/school zone directly towards a cul-de-sac with 2 story houses, speeds up more, then abandons all the "Full Self Driving" while I'm flying towards a power transformer box and curb when I SLAM the brakes (I gave her all she's got, she's heavy!) luckily I stopped with just a couple feet to spare. Then I drive back the direction it was supposed to go which also for the record it was showing a different path on the map, the entrance it always is. So the map was showing the blue line for GPS to turn into the second entrance, this time was the first time its ever taken the first entrance and I've literally never driven down the road it almost crashed me on but its been there for 6 months or longer. So just to clarify Tesla has FAILED at least 10 rounds at fixing the car and they cannot offer a solution. This car is a LEMON! Do something, you're the government I shouldn't be having a car try to kill me five times a day for several months without a recall. This clown car even plays the wrong songs and song titles! Driving both east and west on 429 near the Wekiva river results in the car coming to a complete 0mph stop in a 70mph two lane highway with barriers on both sides and no signs or lights. Then the car starts changing lanes on the highway going about 15mph while cars are whipping past me at 70+mph with the horn on. The last 8 or 10 updates have not solved this issue I've been very alarmed and done everything in my power to avoid using the software and have demanded a refund many times, this is simply a deadly scam. The FSD contract is a license to kill and silence your attempts to alert the public like a good Samaritan; it must be shut down until proven safe. Each time they say they fixed it, each time I go out and risk my life and they lie, and lie and lie and now the stories of people dying from this exact scenario are starting to pile up (pun intended). The car is STILL confusing its sea level or altitude positioning on the GPS for the round about with stop signs below the sunpass highway. Fact of the matter is I have over 15,000 GB's of footage and there are hundreds if not thousands of scenarios where the car needs to be corrected. I am a professional driver and have software development experience but we cannot allow a private corporation to use human American citizens as lab rats and throw them aside when their software that is not safe yet kills the very people trying to improve the system. I had engaged Tesla’s “Full Self Driving” feature in a 25 MPH zone. As the speed limit changed to 35 MPH the vehicle correctly adjusted to 35 MPH. However, right after that I entered a school zone marked by a flashing yellow light. I used the brakes to slow down to 18 MPH, which disengaged the “Full Self Driving” feature because the vehicle did not adjust on its own. Once I was at 18 MPH I re-engaged “Full Self Driving” and the vehicle immediately started accelerating to 35 MPH again, in spite of the fact that I was in a school zone still. It seems the ”Full Self Driving” feature cannot detect school zones, even when marked by flashing yellow lights. The onboard visualizations appears to have detected them as stop lights.

While driving on the freeway into a tunnel during bright conditions while on Autopilot, the car unexpectedly braked heavily on a straight road, even though there were no debris or vehicles in front. Had to take immediate action since cars were driving around me. Similar event occurred when driving at night, luckily no cars were around me. During rain, automatic wipers did not activate. Required 2 actions to deploy wipers: pressing button on the turn-signal stock, then finding the menu on the screen to set wiper speed while driving in dangerous conditions. Another option is to use the voice command, however at times it takes 2-4 attempts to activate.

This hasn't resulted in an accident but there has been a close call when a car was following closely. Wanted to report a problem with Tesla's map navigation and adaptive cruise control (what they refer to as "Autopilot). I haven't seen this reported in the media so wanted to report it here. The map navigation system is problematic, when navigating using the map on the freeway, it will reroute a different path because it somehow thinks I am driving on surface streets. It does this consistently. Because of that, the car when on "Autopilot" will stop in the middle of the freeway, which is incredibly dangerous. This system aims to be safe (by stopping) but also causes greater danger to the following cars by having the car brake. This could be the source behind the "phantom braking problem" that is reported in the media.

There were virtually no traffic on I-81. On my right, there was a truck were merging to the lane on my right (not merging to the middle lane where Model Y was). Without any warning, Model Y suddenly reduced the speed to 25-30 miles / hour. I had to disengage FSD. If this happened during traffic hour, it could cause severe accident.

On 1/7/2022 I was driving the Tesla vehicle in FSD Beta mode (I don't see this option while reporting the issue). This was on highway101 in Sunnyvale/ Mountain View area. The vehicle was in the right most lane. This lane got converted into an Exit only lane. Tesla FSD failed to detect it (didnt change the lane) and It continue at the same speed in the exiting lane (Approx. 65 Miles/hour).The car further failed to detect the curve of the exit lane and was about to keep driving straight at that speed. I had to immediately intervene and take control of the car or my car would have ended up in the ditch or no man's land around the curve of the exit road. This was near miss scenario and a driver's worst nightmare and I hope such accidents are investigated/tested.

I've reported to Tesla multiple times that on State Road 46 Tesla Auto Pilot and FSD BOTH come to complete stops on the highway while the speed limit is clearly posted 70mph the car is showing 45mph, AP is driving at 50mph then comes to a complete stop 0mph on the highway while traffic is doing 70+ passing me because the car thinks there is a stop sign that doesn't exist. When I spoke to Tesla about it the male tech (British accent) from Tampa service center said that's highly concerning and he would take the car home overnight to make sure they didn't overlook anything but they did nothing other than take away my FSD. Now I'm out $12,000 and Tesla keeps calling me a cheater/liar and I have many examples of the car malfunctioning and we are not talking a little glitch this is fully stopped on a highway the car is trying to drive around a roundabout that isn't there because the car thinks its 30ft below the highway on the street below. There is an outstanding recall 22V702 - automatic window reversal systems and the next course of action is that Tesla will deploy an over-the-air (“OTA”) firmware update to affected vehicles. However, my tesla account doesn't have that software available yet. Registrar of Imported Vehicles has searched the records for the above referenced vehicle and has either detected an outstanding recall, or has been unable to verify the current recall status of this vehicle. So unfortunately, they are unable to release an inspection form until such time as they need confirmation that the recall, if any, has been corrected, or that there are no outstanding recalls for this vehicle. Steps taken by me to eradicate this: 1) I tried contacting the tesla dealer at various locations in Canada and USA. 2) I did reach out to the Tesla manufacturer and they told me that they will try to deploy firmware updated for my vehicle and they told me to wait for 1 business day( this conversation happened like 5 days ago) and some associate from this location will reach out to me and i can visit that service center for the resolution of recall. Tesla dealers along with their service centres are totally unaware of what’s going on and don’t have visibility of when this software will be available. Rented the vehicle from Hertz for a holiday road trip of 3100 miles. Experienced multiple instances of "phantom braking" where the cruise control would slam on the brakes with nothing in front of the car within the range of vision. The car would rapidly slow from the posted speed limit to under 45 MPH and then about 5-10 seconds later resume as if nothing had happened. This problem happened to me several times per driving day, and seemed to be more prevalent when cresting a rise in the highway. A couple of time times, this happened with a car behind me, and the sudden braking could have caused the vehicle behind to rear-end the Tesla. In heavy traffic it is easy to see how this phantom braking could cause a chain reaction accident. At this point, I think that Tesla needs to permanently disable their cruise control in all Model Y vehicles.

You get more seating comfort from a Nissan than a Tesla. 2. Windshield wiper sensitivity lowers when using manually then activating Auto. Sometimes sensitivity goes down with autopilot. Auto wiper sensitivity not constant. 3. If you have more than 4 people in your family, do not buy a model S3Y 4. Bad panels which also affect steering column, with squeaking, why is that not a lemon? 5. Long range below 32F falls to half its mileage per full charge. Proper trip charging should be at 10% to 15% more than the minimum 0% for trip charge. 6. Autopilot forces high beams whenever there is fog. Autopilot will never engage on curves, only straight aways 7. Supercharger long trip navigation will not allow filtering locations with bathrooms or trashcans nearby. Walking far distances to find restrooms and trashcans are a normal thing. Teslas not meant for taking long trips. 8. When this happens, I am forced to hit my brakes to disengage Cruse Control, then take my eyes off the road and find the wiper icon and turn them to the off position. This happens often and has almost caused me to drive off the road a few times. I took the car back to Tesla and was told when I engage Cruse Control the wipers are by default set to "Auto". Also they had seen this before and there was nothing they could do to fix the issue. They also said Tesla was working on the software to improve the wipers. Why am I forced to have my wipers set to "Auto" when I use Cruse? I should be able to set them to "Off" and they only come on when I activate them. If this continues on my car and others there will be accidents because of the distraction trying to turn the wipers off.

The front passenger seat airbag does not properly arm when occupied by an adult female weighing between 120-125 pounds. According to the vehicle's Owners Manual, it should arm at any weight in excess of 100 pounds. There is an indicator on the main display that clearly displays when the passenger airbag is armed and when it is unarmed. This is an intermittent issue, occurring more than 50% of the time the seat is occupied. The best we can do to characterize the behavior is as follows: 1) The passenger airbag is more likely to arm correctly if the passenger gets into the car at the same time as the driver. 2) It is less likely to arm correctly if the passenger gets in after the driver has started the car and/or driven it a bit. Two examples: Driver gets in to set the temp or navigation before a trip and passenger gets in a few minutes later, or driver retrieves car from a parking place and then drives a short distance to pick up the passenger. In both cases, the airbag is unlikely to arm correctly. 3) Even after a successful drive (in which the airbag armed), say to the grocery store, it is unlikely to arm when the driver and passenger get back in together a few minutes later. In all cases above, the longer it is driven in a single trip, the more likely it is to randomly arm/disarm itself multiple times during that trip. By failing to correctly and consistently arm when occupied by a 120-pound adult passenger, it leaves said passenger unprotected in case of an accident. Tesla

First my car was having issues charginging at the charge stations. Then as soon as that issue occurred, I had to replace the car battery. Then I noticed that the windows would randomly retract while I was driving my vehicle. My child had pointed out that his window was down, when he did not roll it down. Then I noticed some leakage from underneath my vehicle. My main concern of fluids leaking from my vehicle was my main concern. The Tesla dispatcher confirmed that concern by stating that fluids should not be leaking from my vehicle. When he started accusing me of having a car accident that caused me to be even more concerned. He told me that the Tesla service center on 1100 Colorado Ave, Santa Monica, Ca. would find out if I did have an accident by running a diagnostics. When I had my car towed to the Tesla service center, their main concern was a hole in my tire and their professional opinion was to just patch up the tire and not replace the damaged product. On a clear day, with a dry road, with no traffic nearby, no shadows in the area, the vehicle suddenly applied the brakes. It was like it detected something in the road and jammed on the brakes. I used the accelerator to compensate, but, it tried to slow down for 20 seconds before resolving. My wife (passenger) has medical conditions that were exacerbated by being frightened. I also was frightened, which is dangerous while driving. If there was a vehicle behind us, there cold have been an accident. There was no warnings before, during, or after the incident. Multiple people online have described their situation. It is exactly the experience that I have had with my vehicle.

On Saturday night between 9:30 to 9:45 PM while driving on 495, Tesla Model Y 2022, Auto pilot was engaged at 70 miles per hour in three lane highway. My car was in middle lane with no vehicle in front in any of lanes. Suddenly without any warning, auto pilot was disengaged and car moved from 70 mph to 30 mph within few seconds shockingly. I was alert to pull accelerator in timely manner and avoided a possible rear-ended accident . It was scary moment with family onboard and luckily cars where behind enough to evade a possible disastrous accident. This must be fixed

I am writing to file a safety concern with vehicles manufactured by Tesla Motor Company Inc (“TESLA”). The complaint relates to safety failures with the automatic retracting window system. This complaint is different from the NHTSA previous issued recall for pinch points (issued on or around September 19th, 2022). During the vehicle software update process, owners can enter the vehicle and close the door (there is no engineered prevention of opening the doors). However, upon attempting to exit the vehicle the window does not automatically retract. When the owner closes the door after exit the windows are prone to break or shatter (see attached photo) which creates a safety hazard. The specific hazard is with broken glass spread in the cabin of the vehicle. Tesla charges customers to replace the window in spite of the known defect (see attached paid invoice, which documents how the damage occurred). Tesla states "it is a known documented issue and is therefore the customers responsibility". In lieu of correcting the acknowledged known issue or providing a proper engineered control to avoid the safety hazard created by the defect; Tesla has apparently accepted the defect and passed the risk of the defect to customers who are not formally made aware of the problem and are expected to understand the problem based on documentation in third party forums. The issue poses a cutting hazard to the driver and occupants of the vehicle and also a foreseeable egress concern in an emergency

While using cruise control the vehicle brakes hard when no obstacle is any where near the car in front or sides. Some times hard enough to engage abs and you can feel the car on the edge of losing traction/control. This has happened multiple times on multiple trips.

Car did not stop after I parked and continued to go forward and hit another parked a car Even with my foot on the break. No warning light of any sor. This incident happened at 5:35 PM Pacific time on Monday, September 12. I made an appointment with the service department and it took the car there on September 23, 2022, . They only asked me about the time this happened and they said you’re going to send it to engineers and will get back to you. They did even Care to look at the car or inspect it. Since then, no one ever contacted me. I was lucky that this truck was in front of my car otherwise if there was anyone walking they would have been killed.

All safety features - backup, turn signal and blind spot camera detections stopped working on November 22, 2022. Tesla gave me appointment to bring in the car on 11/28/22 then emailed me informing appointment pushed back to 12/8/22 and today emailed informing appointment pushed back again to 12/12/22. The car is not drivable without these safety features as it poses the risk of accident that could harm driver and other on the road. Tesla is not responding at all for providing a replacement vehicle, while this car is still under warranty.

Sudden Phantom braking happened while driving north on garden state parkway. I was in the 2 lane express northbound highway, and there was heavy traffic and flashing police lights on the separated 3 lane local northbound highway. I was using adaptive cruise control when Tesla suddenly braked, reducing speed 20-30 mph in a matter of seconds, despite no cars in front of me. I took back control and disengaged cruise control

When using Autopilot on interstate, car did phantom braking when approaching new concrete sections of highway (I-4 between Orlando and Daytona Beach) on a primarily black asphalt highway during sunny afternoon conditions. This happened on a couple of occasions during drive.

Steering

I was driving with my wife and two kids at around 8:30pm (it was dark outside already) on November 29, 2022. The street was pretty empty and it had 2 lanes in each direction. I was driving the left lane with the speed at about 40 mph and had the autopilot (the one that keeps lane, not FSD) turned on. When the car was approaching the intersection with the green light on, it suddenly decided to turn left (even though there was a dedicated left turning pocket on the left hand side). It looks like it was confused and after trying to change the direction it pointed the car right to the road sign. Hopefully I had my hands on a wheel and was watching the situation, so I immediately turned the wheel right to get back to our lane. If I wasted a fraction of second we could end up smashing into the road sign at about 40mph.

The wipers "swipe" when passing interstate streetlight when in the "Auto" mode. Additionally, the "Auto" mode is automatically enabled when the adaptive cruise control is set. It is not possible to turn off the wipers while using the cruise control, making it basically useless at night. I have been advised on two occasions by Tesla Service that this is a firmware issue and there is no fix. The complaint was last verified by Tesla Service on Dec. 1, 2022.

NHTSA publishes every complaint as the owner wrote it and investigates only a fraction. Spelling, capitalisation and judgement are the filer’s own. What carries weight is repetition: the same component failing the same way across many filings, and whether the manufacturer eventually opened a recall for it.
