On September 9, 2023, we were driving from Winter Park, CO to Omaha, NE. Three or four hours into our trip, we experienced what is described on the internet as "phantom breaking" four or five times in an hour. We were in a remote area on I-80 with no cars ahead of us or behind us and no debris in the road. We have occasionally experienced this hard automatic breaking for no obvious reason before in our almost four years of driving this Tesla, but we never thought much of it. This time since it happened so many times in such a short amount of time, we researched online if there was anything we could do. With that information we turned off the "Automatic Emergency Breaking", which seemed to do the trick. When we contacted the Tesla Service Center in Council Bluffs, IA for an appointment to see if they could fix the problem, they asked for more information. They said the following: "We performed a remote diagnosis on the Autopilot System in the vehicle and its affiliated hardware and all checks came back in good standing with no faults." They then suggested I read the Owners manual for factors that may cause the vehicle to brake when not required or expected. I read it and told them none of those conditions were present. They said that was all they could do and suggested if it happens again I should file a "bug report" (which was a new concept to me!). The phantom breaking happened, but Tesla either doesn't seem to believe me or does not have the ability to properly diagnose or

While driving on the highway, the car suddenly alerted me that the steering assist was affected. The steering wheel became extremely stiff. To turn, it took two hands and quite a lot of strength. I pulled off onto an exit, turned the car off, then started it back. The steering worked properly the rest of the drive. The next day, steering worked fine on my 50-mile drive to work. On the way home, however, it froze up the same way again. Again, I pulled over, turned the car off, then back on and the issue was resolved. The steering wheel was so stiff that I could barely turn it, however, and it is incredibly unsafe. The soonest they said they could check the car is October 2nd. It seems to be a software issue since turning the car off and then back on resolves it. Tesla, however, is impossible to get in touch with other than sending a text through the app and then I'm lucky if I ever get a response.
On 9/3/23 we were traveling from Moab UT to Green River UT and the cruise control was engaged at highway speeds. We were not using the "Autopilot" functionality. Without warning, the car suddenly decelerated under what felt like hard braking. There was no warning immediately before the hard braking started and the car had no warning indicators of any kind either before or after the incidents. The hard deceleration happened at least four times. One of those times a raven flew up from the road, but the other times there was nothing in the road. At least three of the incidents occurred on highway 6 traveling north. The last time it occurred was on I-70 westbound after we stopped the cars and cleaned all the cameras and ensured there was no debris on the front bumper which could cause issues with the forward-looking radar. We lost confidence in the cruise control feature and did not enable it for the rest of the trip. This is a safety issue because the car does not just reduce speed, it feels like it is braking hard or perhaps full regeneration of the battery. Either way, on these 2-lane highways, there are often situations when other drivers are driving too close to the rear bumper and a sudden, inexplicable hard braking is very likely to cause collision. A collision at these speeds could push the car into oncoming traffic, leading to fatalities. We asked Tesla service to look into the issue, and said there is likely footage of at least one of the events (because I honked the ho
Spontaneous shattering of side window, rear driver's side. Car was parked in driveway at the time.
I have had multiple incidents of sudden speed reduction on major hi ways. (us 287 and I 25). Usually these incidents occur when I am traveling north (increasing elevation and later in the day), but that is not always the case. The events appear to be totally random, but result in a speed reduction of 70mph to 40mph rapidly enough that loose items (including pets) fly. on occasion I have had to be very quick to accelerate to avoid being run over by a tailgater. I scheduled a service appointment, but was discouraged from doing so by Tesla Service people who said “the car is still learning”. The fault has not improved. If anything it happens more frequently. At this point, I would say we travel “on edge”.
Sudden acceleration while driving and at stop sign
The contact owns a 2020 Tesla Model 3. The contact stated that on three occasions while driving at various speeds, the vehicle experienced phantom braking. The contact continued driving. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 13,000.
Tesla's Full Self Driving was enabled. The car failed to stop at a stop sign, and was actually accelerating through the stop sign. This was a huge error, and I had to slam on the brakes in order to get the car to stop. It was critical that the car stop, as cross traffic did not have a stop sign (2 way stop intersection) and the speed limit for cross traffic was 45mph. Very dangerous. I have video to confirm this incident. Please contact me if you need access to the video.
On August 2, 2023 at 0400, the Tesla M3 was charging in the garage using a home charger. Smoke indicative of an electrical fire likely originated from the vehicle and eventually triggered a smoke alarm in the kitchen. The driver was unaware the source of the smoke was the Tesla, disconnected the Tesla and moved it out of the house to protect the vehicleb assuming a fire was in the house, and called firefighters who searched the home without success for a source of the fire. The driver now believes the Tesla was the source as the garage access door was cracked open to feed the charging cable from the 240v outlet in the laundry room. The nearest smoke detector from the Tesla is in the kitchen, where the driver smelled smoke from an electrical fire. At roughly 0745, the driver was about to complete a one hour commute to work in this Tesla unaware that the incident at 0400 was related to the Tesla. The driver experienced a sudden vector and velocity change in the vehicle while traveling at highway speeds. This is the driver's last memory of the accident that followed. A witness driving behind the Tesla described the vehicle traveling in clear lanes when it abruptly jerked left, then right into the side of the highway into concrete barricades. The Tesla was destroyed, airbags deployed, and the driver was rescued by first responders, firefighters, and paramedics. The driver was evacuated to the nearest hospital with serious injuries. The driver suffered broken ribs, a pneumothorax,
The car repeatedly rapidly decelerates when passing semi-trucks on the highway. This creates a problem for vehicles driving behind my vehicle as it creates sudden braking for them and it creates a bit of panic for the driver of my vehicle due to the suddenness of the braking. This problem occurs almost every time the vehicle is driving over 60mph on a freeway and is midway in passing a semi-truck trailer truck. The dealer has replaced the camera but the problem still occurs. Collision lights occur at the time of the error but no other warnings occur.
Sudden hard "phantom" braking while in adaptive cruise control. This put myself and other drivers in danger of a collision. The problems been reported to Tesla, but as of yet not fixed or inspected. The problem seems to be completely random with no advanced warning of any kind.
While driving on the freeway, my 2020 Model 3 “phantom braked” multiple times while using the cruise control. No autopilot for lane keeping assistance. There were no cars in front of me or any hazards. The pavement was pretty new. I was using this feature earlier in the trip with no issues then it started all the sudden with multiple incidents in a short amount of time.
There is no manual door opener from the rear seats. In case of a power loss doors would not open. Only way the rear passengers can get out is climb to the front seats and get out through front doors. This is a serious situation in case of accident when power loss can happen.
My car cameras and audio will randomly stop working. Come to find out I took a recording with a timestamp of 1151 pm where I am on the highway and driving. Have witnesses of me driving and the issue. My model 3 logs, as stated by service technician, say my vehicle is "Parked and not being operated". This means I am driving with potentially dangerous and absolutely unreliable software in my car. It shows the car isn't being operated as I am going 60~ on the highway in traffic. My car cameras will go completely black and the audio warnings will go out including the backup sounds which are a required feature. Have contacted Tesla since 2020 and no action except a blanket statement of "a future update"
Phantom Braking occurred almost 40 times on our recent trip from Northern NV to California and return. We left on July 16 and returned on July 21. Our route was from Minden NV to Las Vegas on Highway 95 and then onto Temecula CA on Highway 15 and 215. The return trip was on Highway 15 to CA 138 to Highway 14 to Highway 395. All travel was during daylight hours. Sometimes phantom braking was so severe, the deceleration cause us to be thrown forward towards the dashboard. We wore seat belts to prevent hitting the dashboard. This also happened on a similar trip in 2022 and was reported to Tesla and were told that nothing could be done. The current software version is 2023.20.9

Unexpected Phantom Braking was experienced in this vehicle on a sunny day at mid day on I-30 going East through Arlington. The car was operating on Traffic Aware Cruise Control. Travelling at approx 70mph the vehicle suddenly slowed very quickly below 50 mph before I interceded to resume control of the accelerator to resume my speed before I was rear ended. This occured as I was approaching an overpass in my middle lane.
12v battery failed without warning. Car was parked. Was unresponsive. Could not unlock the car. Able to open trunk and passenger side door only. Drivers side would not open. Touchscreen black no power to Do anything car essentially dead with no warning. There should have been on screen display informing driver of the need to replace 12v battery. There was none. Car needed to be towed. If this has happened while we were in the car we would have been locked in. Tow operator jumped the battery to open hood. Touchscreen restarted but car still unresponsive. Multiple warnings on screen. Tow mode would not work so car needed to be pulled with the rear wheels locked onto tow truck.
Open door button doesn’t work, this is a widely reported problem online. In case of accident, fire or submersion the passenger will be at great risk for bodily harm or death
My vehicle was in a collision on the freeway and the airbags did not deploy. The car is being assessed to see if it deemed totaled. Regardless of the outcome of the status of the vehicle, given the impact of the accident, the airbags should have deployed given the impact of the collision and the impact on the car.
While driving on the freeway I5 in central California in the middle of the day there were no cars in front of me and the vehicle went into sudden PHANTOM BRAKING mode. I was driving at 75 MPH and it suddenly went into hard braking. Extremely dangerous. The vehicle behind me had to swerve around us. We could have been killed. I was using the cruise control but NOT the Enhanced Autopilot. This needs to be fixed. We escaped severe injury or death by sheer luck
Had cruise control on and “phantom braking” occurred about 10 times on a 22hr trip from California to Oklahoma. Very scary. Car went from 80 to about 40mph in seconds. How is this even possible for Tesla to have these vehicles on the road? I’ve only had the car for 2 weeks.
I was driving from indio,ca to Orinda,ca on interstate 10-210-5 north and the vehicle had at least 6 phantom braking events when the car was in cruise control and one event the auto pilot was on. There was clearly nothing front of me. This is so dangerous and there will be a bad accident if it isn’t fixed. Please have Tesla fix this.
The contact owned a 2020 Tesla Model 3. The contact stated while pulling into the driveway and depressing the brake pedal, the vehicle failed to respond, accelerated unintendedly, broke through the garage door, and crashed into the vehicle parked inside the garage. The other vehicle moved away and damaged the garage. There was no warning light illuminated. The air bags did not deploy. The contact shifted into reverse (R), pulled out of the driveway and shifted into drive (D). The brake pedal was depressed but the vehicle failed to respond. The contact stated that he hit the curb to stop the vehicle, and the vehicle stopped. The contact stated that all the air bags then deployed. The vehicle failed to unlock. The contact rolled down the windows and cried for help until the neighbors arrived and opened the vehicle from the outside. The contact sustained bruises on both arms, and the contact's wife sustained bruises on the right shoulder and on the left leg. Both the contact and his wife received medical assistance. There was no police report filed. The vehicle was towed to a third party it was sold to. The other damaged vehicle was totaled. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was 9,600.
On July 19,2022, when I was driving on driveway, at less than 5 miles, I tried to stop to drop my wife and applied brake. Instead of brake, due to computer mal function, it accelerated & hit the garage door, broke the garage door, door fell on the car parked in the garage, damaged that car,and tesla automatically reversed. while reversing brake wasn't working too so I tried to put in drive & brake still was not working so to stop the car for safety, I hit the curb in my front yard All airbags were deloyed, inuring both of us. All doors were locked & we couldn't open any of them. We could open windshield. We shouted HELP and my neighbor came & open the door from outside so we could get out otherwise we both could be dead inside the car
Recurrent phantom braking while using autopilot. Sudden deceleration that does not seem to correlate to any specific traffic or weather condition.
When driving on a longer trip, I have repeated sudden deceleration and sometimes harsh braking when using cruise control. This has also happened in normal daytime hours with no traffic around me on rural interstates. On my last trip, I had to keep my foot on the accelerator to keep the car at a constant speed. It is unnerving to me and very scary for passengers. It has made it impossible to use cruise when I have passengers. Tesla has said there is no hardware issue and have declined to service the car.
The car had unintended acceleration. I was driving and suddenly it sped up really fast and I needed to slam on the brakes. This was on a residential street so it was really dangerous. This happened once before, on a highway. Both times this happened, I was touching the wipers controls. I have no idea if it’s connected or coincidence.
Rear windshield developed a stress crack. No incident was involved, it just suddenly appeared. It ran about 1/3 of the rear window. I was concerned the glass would fall in on infant passenger seated in a car seat in the back seat. This appears to be a common glass problem for Tesla.
The automatic windshield wipers came on and would not shut off. The car was clean, the day was sunny, and the camera lenses were free of dust or anything else that could cover them. While driving along, the wipers started and would not shut off. I went into the main monitor/screen that has the wiper settings but I was not able to activate the wiper/off button, so the wipers kept working. Pressing on that button would not activate it. I was in autopilot at the time, i.e., cruise control + lane keep assistance. I have FSD/full self driving, but have never used it since it is still in beta testing. I’m using Tesla software # 2023.7.10 I checked the various Tesla websites, and you can turn off the wipers only if you disable the ADAS features. After I turned off the ADAS, I was able to activate the wiper/off button. But this is a terrible situation, the ADAS needs to be activated, but if I activate, it wipers will begin working whenever they feel like it, regardless of the weather outside. This is clearly a software problem with the Tesla ADASN needs to be corrected. I don’t want to be driving along with auto pilot on, and then the wipers will simply turn on. Any help you could provide would be greatly appreciated to correct this dangerous situation. There are countless hundreds of complaints like this on the various Tesla websites about spontaneously functioning wipers when the weather is bright and sunny.
The main computer failed right after the warranty expires at 51,721 miles. My computer just failed and I have not done a single thing to damage the main computer. I was upgrading to 2023.12.5 and the computer just crapped out. I’ve been driving around with a blacked out screen and no access to backup camera, A/C, and unable to tell my speed, or when the turning signal is on. I tried to explain to the service center that this is not my fault for the computer failing and that they need to take care of the service free of charge. The car is still drivable and I have been fighting with Tesla support and the service center people to get them to understand that I am not liable for their defective equipment. It seems the only way they are willing to take responsibility is after a recall is issued.
Hard Phantom braking incident when using cruise control with no vehicles ahead or next to me.
I have an alert that says "Front passenger safety restraint system fault". I took the car in to Tesla Service in late April. Was told it would be about $270 to replace the sensor in the seat that detects it someone is sitting in it. They told me that when the parts people went in, that wasn't the issue and that to fix the sensor they thought it was they would have to tear the whole seat apart and that takes too long so instead I need to buy a whole new seat at the cost of $3000. I didn't have time to deal with it further at that point and the seat was back ordered anyway. I asked if the issue would affect the operation of the airbags or seat belts thus causing a safety concern and was told that it would not. That the car was still safe. I took the car back without repair. I have since noticed that when a passenger is in the seat, the symbol that states that the airbag is not engaged is still on. So it would appear that the airbag is not working properly. I called Tesla today to inquire and ask for clarification if this is a safety concern and if the airbag is not working. They told me a manager would call me back. They said there is not a Tesla wide recall- even though it would appear that this issue is causing the airbag not to work. From research online it seems like other customers have experienced the same or similar issues.
Windshield wipers routinely start "auto-wiping", even while turned off. I've reported this to Tesla many times and nothing is done. Plenty of evidence online that others have this same problem. I live in Phoenix, Arizona so when I say my windshield is dry, I mean it's DRY. Leaving out how distracting it is to have your wipers refusing to quit wiping, it's also wearing out the wiper blades so they're not efficient when actually needed. I've already replaced them once a year ago with, maybe 10,000 miles on vehicle.
When using “Auto Pilot” (Teslas adaptive cruise control) the vehicle experiences violent phantom braking. The system slams on the brakes in some cases going from 65 to 20 MPH almost instantly. This happens on a fairly regular basis and is a massive safety issue. We have nearly been rear ended on several occasions.
When using cruise control from Las Vegas to Reno Nevada and also going back the car will brake extremely hard over 80 times one way which almost caused alone 4 major accidents with stopping abruptly for no reason causing the cars behind you to almost rear end you. This is tremendously dangerous and it s happening so often you can even use the cruise control. This does happen on other trips as well such as Las Vegas to Phoenix. When it slams the brakes randomly all your items get thrown to the front as well. This needs fixed before someone gets killed
The contact owns a 2020 Tesla Model 3. The contact stated that while driving 25-35 MPH, all four windows slightly went down independently, wiggled and made clicking sounds. There was no warning light illuminated. The contact stated that the sound had become unbearable. Additionally, the contact stated that the front driver's side and passenger's side doors and the rear driver's side door failed to close securely. The door chime reminder engaged. The contact also stated the vehicle would shift into park (P) and would jerk when attempting to drive due to the failure. The contact stated that the doors required to be slammed in order to close securely. Additionally, the contact stated that the vehicle shifted into park (P) and stopped abruptly while driving. The contact stated that the failure also occurred while driving downhill and while at a traffic light. The vehicle was taken to a Tesla Service Center several times, and the windows and doors were readjusted; however, the failure persisted. The vehicle was taken back to the Tesla Service Center, where the window trim panel and the door trim panel were readjusted; however, the failure increased. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 14,000.
I was driving on a highway using the adaptive cruise control. Several times the car braked for no reason at all. There were no cars in front of us, it was phantom breaking for no apparent reason. It was a long road trip of 12 hours over 2 days and I stopped using cruise control when anyone is behind me out of fear of causing an accident. The issue happened at least 15 times over the 2 days. It is a safety issue in my opinion, as other cars following you may rear end you when the car brakes hard for no reason. I've owned the car for a while, and it seems to have gotten much worse than it was before. There are no warnings before it does the phantom breaking. I have not had it inspected by anyone else. I believe it is a software issue that needs to be fixed by Tesla, as there are no error messages, and all my cameras appear to be functional.
While driving in FSD, autosteer, traffic aware cruise control, and with all turned off, I have experienced 5-10 episodes of “phantom braking” on the highway at high speeds over the past 48 hours. I have driven in 4 states total so far; in sunny, cloudy, straight road ahead conditions, sometimes with cars around and sometimes on totally empty stretches of road. The first episode took my car from 80mph to 60mph extremely suddenly, with no warning, lurching me and the things in my car forward. I was shocked as I was unfamiliar with this issue. Luckily the car behind me at the time was about 1/8 mile away. However, this could have very easily caused someone to rear end me. This repeated over and over all day yesterday, sometimes the braking less severe than others. Now if using cruise control I need to hold my foot over the accelerator instead of the brake, which feels wrong and also unsafe, but it’s terrifying and I disengage cruise control if anyone gets too close behind me to be safe.
Driving from Reno to Las Vegas on hwy 95. I was using the cruise control and the car repeatedly slammed on the breaks when there was nothing on the road to cause it. Breaking was applied dropping the speed from 5 miles an hour up to 35 miles per hour. The excessive breaking could have caused a crash if a semi was behind me. Every time it happened I noticed a phantom object showing up on the very limit of the traffic display screen. If there was any other vehicle picked up by the sensors the cruise control never malfunctioned. It braked at least 10 times during the trip. I talked to 10 other Tesla drivers and 9 told me they were having the same issue. The one driver that did not never used his cruise control.
Recall 21V-00D is well over a year old and Tesla has not remedied it in a timely manner.
I was T boned at an intersection by a red light runner. My car got hit on the driver side and front to back where the initial impact occurred by the left front wheel. None of the airbags deployed. My speed was probably around 30 miles and the other cars speed I don’t know. The other car was a 2021 Toyota suv. I don’t recall what model but all of the airbags deployed in the Toyota. I got thrown against the driver door and sustained an injury to my left rib cage.
The contact owns a 2020 Tesla Model 3. The contact received notification of NHTSA Campaign Number 21V00D000 (Electrical System, Visibility, Back Over Prevention) however, the part to do the recall repair was unavailable. The local dealer was contacted, and it was confirmed that the part was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure. Parts distribution disconnect.
I got a "power reduced dir_a126" error code that limited my speed to 55 mph. This is dangerous on the highway as 55 mph is significantly under 70 mph and cars were almost rear ending me.
April 10 & 11. Drive from Minneapolis to Washington D.C. Several phantom braking incidents—two severe. If someone would have been behind me on the Ohio Turnpike, there would have been a major crash. This issue first cropped up on my vehicle in late fall of 2020. WHAT CAN BE DONE TO FORCE TESLA (really MUSK) to install radar sensors or LiDAR? Is there a class action lawsuit that has been started? I have a car I cannot trust to use cruise control. I do not use, or ever want, lane assist or FSD. I just want a reliable and SAFE cruise control system. I get nowhere with Tesla on this. At this point, Tesla should buy back my car—and likely thousands of others or retrofit. I know that others have been injured and no doubt someone is going to get killed…something must be done…this is major SAFETY issue. Thanks.
Left headlight blinker and windshield wiper not working. Happened twice once in 2022 and now. Repaired once in 2022 and when happens is dangerous because can’t signal and can’t use wipers in the rain
I was on a 3000 mile road trip over multiple interstates (including I-35, I-90, and I-80), and while the adaptive cruise control was engaged, the car would abruptly and aggressively brake when no other cars or obstacles were in the area. This would often be a 30mph deceleration before I could disengage cruise control and take over. This happened repeatedly, in multiple states, and in different weather conditions. At times, it was happening every 10 minutes. The system would sometimes, but not usually, make an alarm noise. This problem has become noticeably worse in later software updates. No accidents occurred because I would not use cruise control if other cars were in the area, and I eventually stopped using it altogether due to how frequently it was braking.
Vehicle suddenly braked while traveling 70mph on freeway with traffic aware cruise control engaged. This incident happened 3 times during the same 40 mile trip. No other vehicles were within 50 feet of the vehicle in either direction. A rear end collision could have happened if another vehicle were behind me. No warning sounds occurred or warning indication on the computer screen. I purchased this vehicle used from Tesla 3 days ago. Tesla service has not been notified of issue.
Since recent software update The adaptive cruise control now randomly applies the brakes while driving on a straight road with no obstacles or vehicles in front. Today driving i80 through navada This happen about 5 times within a few minutes of engaging cruise control. This is a new issue I’d not seen until now. I’d had maybe 2 or 3 incidences of phantom Braking over the last couple of years. Today I had it happen multiple times through the day.
Since recently updating the software I now get a lot of phantom braking when employing adaptive cruise control. This hardly happened in the past. Recently it’s happening every time I engage cruise control Today we drove from San Francisco to Utah on the i80 The car randomly braked 5 times Each time the road was flat, straight with no traffic in front or obstacles or road cones. After engaging cruise control the car would randomly brake aggressively with a few minutes. I would turn off cruise control then try again a few minutes later. The same throng would happen Speeds ranged from 65-75 mph

What carries weight is repetition: the same component failing the same way across many filings, and whether the manufacturer eventually opened a recall for it.
