Forward Collision Avoidance › Automatic Emergency Braking1 injured
Filed Oct 3, 2026CA

Repeated Unexpected Braking / Suspected AEB Activation My 2026 Tesla Model Y has repeatedly experienced sudden unexpected braking at the same location near my home over the past several weeks. The incidents have occurred both while Full Self-Driving (Supervised) was engaged and, importantly, while I was manually driving with FSD disengaged. During some periods, this occurs almost every morning. It has also occurred during both daytime and nighttime driving. The location currently has construction activity, and the behavior is highly location-specific. In the most serious recent incident, the vehicle suddenly braked while another vehicle was following behind us, creating a near rear-end collision. Our approximately two-year-old child was in the rear seat, and repeated sudden braking events have frightened our child on multiple occasions. We now do not feel safe driving this vehicle with our child through this location, but the road is a necessary route for entering and leaving our neighborhood. We reported the issue to Tesla and provided multiple specific timestamps. Tesla performed remote diagnostics and stated that no vehicle hardware fault was detected. Tesla told us that it believes the behavior is related to the construction/road environment at this location and that it currently has no repair or solution for the issue. My concern is that an active safety/braking system can repeatedly initiate unexpected braking during normal manual driving because of a specific road envi

On September 28, 2026, I was driving my 2026 Tesla Model Y Dual Motor northbound on Interstate 17 in Arizona near the Sedona exit area. Full Self-Driving (Supervised) was engaged. The vehicle was running software version 2026.27.11 and FSD (Supervised) version v14.3.10. There was light rain in the area and the roadway was wet. I was traveling approximately 66–67 mph in a posted 75 mph zone. While FSD was engaged, the vehicle unexpectedly initiated a lane change. During or immediately following the lane change, the vehicle encountered the wet/standing-water area, lost control, and struck the concrete barrier. I am concerned about why FSD initiated this lane change under these roadway conditions and whether the system properly detected and responded to the wet roadway. I have requested that Tesla preserve and review all available FSD logs, vehicle telemetry, steering and braking data, lane-change and path-planning information, camera data, collision data, and other information associated with this incident. I am requesting that NHTSA review this incident as a potential safety issue involving Full Self-Driving (Supervised), particularly its lane-change behavior and operation under wet roadway conditions. I am very positive this incident wouldn’t have happened if FSD wasn’t in play. This is huge safety concern I want to share to avoid any issues!

Vehicle Speed Control
Filed Sep 29, 2026WV

The speed recognition of Tesla Full Self Drive is unsafe in its present form. The speed recognition is very inaccurate. While driving on an interstate highway where the speed limit is 70MPH, it is recognizing a route sign with a much lower number as a speed limit sign and suddenly applying braking, causing me to nearly be rear ended on multiple occasions. Every time I pass this sign I have to either disengage full self drive, or I have to override it to keep from being rear ended. The same thing happens on another highway, where the exit ramp has construction going on. When you are on the main road, it recognizes the exit ramp construction speed limit and tries to reduce you from 65 to 35. This problem has been reported to the manufacturer with no response other than "it is being forwarded on to the appropriate team for review"

I am writing to raise a serious safety concern regarding my Tesla Model Y Premium AWD 7-seater which was involved in a significant rear-end collision The vehicle sustained substantial impact and has been determined to be a total loss. Given the severity of the collision, I am concerned that none of the airbags deployed. This is a serious safety concern, and I would appreciate a detailed technical explanation rather than a general statement about airbag deployment criteria. Please let me know what information, photographs, police reports, or other documentation you need from me to conduct the investigation

The vehicle made contact with other vehicles twice within a short period while Full Self-Driving (Supervised) was engaged, and it did not respond to my attempts to intervene. On XXX, at approximately 1:30 PM, in the Stanford Oval parking area in [XXX], I was using Tesla Full Self-Driving (Supervised) in my brand-new 2026 Tesla Model Y when the vehicle began a reversing maneuver between two parked vehicles. During the maneuver, the on-screen visualization continued to show space between my vehicle and the surrounding vehicles. The vehicle then made physical contact with another vehicle. I received no proximity or collision alert, and the display continued to show clearance after contact occurred. After contact, the vehicle continued attempting to reverse, stalled, and behaved erratically. I attempted to brake and disengage the system, but the vehicle kept glitching and moving backward. I could feel resistance while trying to control the vehicle.Shortly afterward, at [XXX], the vehicle showed substantially similar behavior and again made contact with another vehicle. My vehicle sustained damage to the rear bumper and lights. The other vehicle had pre-existing damage, and its driver told me she did not want to pursue anything or have her vehicle repaired. The vehicle is brand new and I had no prior accidents. I will be assessing myself for injuries. I reported the incident to Tesla. On September 26, 2026, the automatic rear liftgate of my 2026 Tesla Model Y struck me on the head while it was closing. I was not manually pulling, pushing, or holding the liftgate at the time. The liftgate made contact with my head, causing a cut and bleeding, and then stopped. I sought emergency medical treatment following the incident and was diagnosed with a mild concussion. I am concerned that the liftgate's obstruction detection/anti-pinch system did not detect the obstruction early enough to prevent contact with my head or did not respond appropriately before impact. The fact that the powered liftgate made physical contact with my head before stopping raises a serious safety concern regarding the liftgate's obstruction detection sensors and closing mechanism. I am reporting this incident because the automatic liftgate caused an actual physical injury requiring emergency medical evaluation. I request that NHTSA document this incident and investigate whether there is a potential safety defect involving the automatic liftgate, obstruction/anti-pinch sensors, or closing mechanism.

Component Involved: Full Self-Driving (FSD) / Uncommanded Vehicle Movement DESCRIPTION OF SAFETY DEFECT & INCIDENT: While using Tesla's Full Self-Driving (FSD) system, the vehicle navigated into a driveway sloped on an uphill incline and came to a stop. Immediately after the front passenger unbuckled their seatbelt to exit, the vehicle unexpectedly began moving backward down the slope on its own. Because my focus was entirely on preventing a collision, I could not verify if the touchscreen or gear indicator changed states; I immediately had to apply the manual brakes to stop the vehicle. This uncommanded rearward movement immediately after a passenger unbuckled poses a severe safety hazard to exiting occupants and nearby pedestrians. Emergency driver intervention was required to secure the vehicle.

Tesla Model Y, after a new update, went into an intersection while the traffic light was red. It saw it, but did not stop until it was half way into the intersection...then it slowly backed up out of the middle, until the light turned green. This happened crossing a bridge over [XXX] north of [XXX] onto [XXX]. It has crossed this intersection many times over the last six months without incident. Fortunately there was no traffic on the right trying to get a head start. There was no damage, just weird strange behavior, unlike any we have experienced with this vehicle and FSD 14.3.10. I and other Telsa drivers report Tesla FSD takes too long to enter a lane from which the car has to turn. Some drivers report “last minute” trying to get into turn lane or expressway off ramp. Twice last week my Tesla (Model Y built in June 2026) waited much too long to get into the required turn lane. On both occasions I was either in outside lane needing to turn left, or middle lane needing to turn left. One first occasion Tesla in FSD was only 0.3 miles from the turn as cars behind stacked up preparing to make the left turn. On second occasion, car in FSD was only 0.2 miles from turn, even indicating by LED light strip turning red, that a car behind was in my blind spot. Yet, the Tesla wedged itself in front of the car behind already in the turn lane forcing that driver to slam on his brakes to let Tesla in for the turn. The second occasion could have resulted in an accident or incited a road rage incident from the driver behind. I asked Grok to request Tesla engineers fix this FSD poor decision-making. Grok said he’s aware of this problem, correctly referring to it as a “lane change timing” issue, but stated he could not message Tesla engineers. At next opportunity, I sent a message via microphone in my car, though not during a “why did I take over intervention” request, because I did not intervene. Grok offered help saying I could use my turn signal to get in turn lane further in advance of the turn, or change speed mode to adjust for traffic behind, but that defeats use

The vehicle itself will not follow safety lane control. Full self driving was in fact it crosses double white lines and toll roads and will merge into other lanes that has led to unsafe situation excessive ticketing, especially in the [XXX] area. This has happened several times, where it’ll go over the double white line ignore the laws and it will stay in the left lane, causing traffic and disruption. Today it did it several times and jerked into the toll lane when it was a double white line and caused a traffic issue instead of working through it. It signal quickly and then just moved over which was not OK and it was against the rules. Yep — this follows the NHTSA guidelines in your screenshots and avoids your name, VIN, phone number, or other personal information. You can paste this directly: While Full Self-Driving (FSD) was engaged, I was stopped in the left-turn lane at the intersection of [XXX] and the roadway that changes between [XXX] and [XXX]. The traffic signal was red, and I was waiting to make a left turn. When the protected left-turn signal turned green, the vehicle did not begin the turn. I pressed the accelerator to prompt the vehicle to proceed, as I have previously done when FSD hesitates at a green light. Instead of following the left-turn lane and turning left, the vehicle accelerated straight ahead toward another vehicle in front of me. The other driver had to swerve to avoid being hit. I immediately took manual control, steered away from the other vehicle, and completed the left turn myself. A collision was narrowly avoided. There was no impact and no injuries. The FSD behavior put both vehicles at risk because the vehicle attempted to proceed straight from the left-turn lane rather than following the protected left-turn signal. I am filing a safety complaint about the lane departure avoidance system in my 2026 Tesla Model Y (VIN [XXX]) software version 2026.26.6.5, purchased XXX, odometer [mileage]). The system makes abrupt, forceful steering corrections during normal driving. I have to resist these unexpected steering inputs repeatedly, and this has caused growing pain in my left hand, including carpal tunnel symptoms. The system cannot be fully disabled. Even when I turn it off, it turns itself back on every time the car is restarted, so I cannot prevent these interventions. I believe steering force that the driver cannot reliably turn off creates a risk of loss of control, particularly in traffic or on narrow roads. I reported this to Tesla on XXX, and Tesla stated that nothing can be done. I ask that NHTSA review this complaint and evaluate whether this system's steering force and its inability to stay disabled are a safety-related defect. I can provide further information or medical documentation on request. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

While I was driving on the highway in rainy weather, my electric vehicle suddenly made a very loud blast or popping sound and immediately stopped working. The vehicle lost power, and all electrical functions became inoperative while I was in the middle of the highway. This created a serious safety hazard because I was unable to continue driving or operate the vehicle normally. There was no visible damage to the exterior of the vehicle. The tires were intact and appeared to be in normal condition, with no tire damage or other external damage that could explain the sudden loss of power. The vehicle stopped unexpectedly while driving in wet conditions, without any apparent external impact or physical damage.

I am reporting a potential safety defect involving the forward-facing cameras on my Tesla Model Y. The vehicle relies on these cameras for critical driver-assistance functions, including Tesla’s FSD system. However, the camera area is prone to accumulating dust, dirt, moisture, and other contamination during normal driving. When this happens, camera performance can be degraded and driver-assistance functions can become restricted or unavailable. More importantly, the current design makes proper cleaning unnecessarily difficult. To access and clean the camera area properly, the camera cover/housing must be removed. This is not a reasonable or practical routine-maintenance procedure for a component that is essential to the vehicle’s safety systems. It creates a recurring risk that camera contamination will remain uncorrected. This appears to be a design issue, not normal vehicle maintenance. A safety-critical camera system should be designed to remain reasonably protected from contamination and allow the owner to clean the camera area easily without disassembling the camera enclosure. I request that NHTSA investigate whether this design constitutes a safety defect and whether Tesla should provide a permanent design solution or recall/service campaign for affected vehicles. I can provide photographs and documentation showing the contamination and the cleaning procedure required.

While using Tesla Full Self-Driving (Supervised), my vehicle approached an intersection and came to a complete stop at a red traffic light. The vehicle remained fully stopped while the traffic signal was still red. Without any accelerator input from me, the vehicle unexpectedly began moving forward toward the intersection while the traffic light was still red. I did not press or touch the accelerator at any point during this event. As soon as I realized that the vehicle was moving forward against the red light, I immediately applied the brake to stop the vehicle and then immediately reversed to move back to a safe position. My intervention was necessary to prevent the vehicle from entering the intersection and potentially colliding with cross traffic. There was no collision and no physical injury. However, the unexpected movement created an immediate and serious risk of a collision. My pregnant wife was in the passenger seat at the time of the incident. Full Self-Driving (Supervised) was engaged throughout the event. I have preserved the original Tesla Dashcam footage documenting the incident, including the red traffic signal, the vehicle coming to a complete stop, the unexpected forward movement while the signal remained red, and my immediate braking and subsequent reversal. Every time the driver takes over from Full Self-Driving (Supervised), a mandatory "Why did you intervene?" feedback dialog appears on the center touchscreen. The dialog cannot be dismissed: it does not time out, and the driver must either tap one of the on-screen options (Preference / Discomfort / Navigation / Critical) or record a voice note using the steering-wheel microphone button before it goes away. This forces the driver to look at and interact with the touchscreen immediately after a takeover, exactly when full attention should be on the road. Earlier software versions dismissed this prompt automatically after a few seconds; recent updates made it mandatory and it even blocked other screen controls. It is a driver-distraction safety hazard caused by the UI design. The prompt appears on every single FSD disengagement, i.e. daily. Software version 2026.26.6.5.

On XXX at about 6:14 AM, my 2026 Tesla Model Y drove into a flooded railroad underpass on [XXX] near [XXX], with Full Self-Driving (Supervised) engaged and a navigation route active. It was raining heavily (Newark Airport measured 1.25 in from 5:51 to 6:51 AM), the light was poor around sunrise, and the underpass was dark. The water depth was not apparent to me. At 6:12 AM the National Weather Service had issued a Flash Flood Warning naming [XXX] and listing underpasses. Tesla's data log (my privacy export) records FSD active from 6:12:02, my third FSD session that morning. With FSD active, speed rose from 7 to 27.4 mph between 6:14:04 and 6:14:31, with no accelerator input recorded. Speed then fell sharply (up to about 0.3 g), which I believe is when the car hit the water. FSD stayed active about 8 seconds (200 ft) more; the state changed to ABORTING at 6:14:39, at about 6 mph. I saw no warning, and the export shows none during the approach. We could not drive out. Water rose to about window level and entered the cabin. From 6:17 the log shows restraint system, AEB, ABS, stability and traction control alerts. I lowered a window at about 6:19. At 6:25 the log shows both front door releases activated repeatedly; neither door opened. We climbed out the window onto the roof. Our videos show us there at 6:31 with the front of the hood and headlights under water, and the Elizabeth Fire Department rescued us by boat at about 6:51. The car is a total loss. Inner barrel of factory OEM rear passenger wheel developed circumferential crack with no known impact. On 09/17/2026 I discovered a crack running along the inner lip/barrel of the factory rear passenger wheel on my 2026 Tesla Model Y with approximately 21,000 miles. The vehicle is 5 months old from new. There was no pothole strike, curb hit, or accident that I am aware of. The tire (Continental ProContact RX) shows no sidewall bulge, cut, pinch, or blister, and the outer wheel face shows no curb damage. The crack was found during visual inspection. The wheel was never repaired, refinished, or modified from factory condition. Tesla Service inspected and denied warranty coverage, stating a small bend was observed near the crack and attributing it to road hazard. No corresponding tire damage was documented. Repair quote provided was $846 for wheel replacement. This is a safety concern because a cracked wheel barrel can cause sudden air loss or wheel failure at highway speed, leading to loss of vehicle control. I have retained photos of the cracked wheel. Searching online, this seems to be a common issue with Tesla's as it is discussed in many blogs and social media websites.

Full self driving (FSD) has taken away my ability to set a maximum speed. Car is frequently wrong about the speed limit, especially in constructions zones where the speed limit changes more often. Car goes way too fast, then thinks the speed limit dropped and slows way down and then speeds up again. In addition, the 4 speed profiles they gave us instead of a simple max speed setting are useless. Standard mode usually works best but will sometimes do more than 15 mph over what it thinks the speed limit is.

My data export from the manufacturer (200 ms resolution) confirms the timestamp and shows the driver-monitoring channel never entered either of its warning states during the drive, though both appear elsewhere in my data. First brake application was 5.56 seconds after; first accelerator input 15.5 seconds after; no steering wheel button events near the event. This creates visual and cognitive distraction at the exact moment I have assumed manual control and need to focus on driving. Hello, I am requesting assistance with an unresolved noise issue coming from the rear seat belt area of my Tesla. I have been experiencing this noise for quite some time. I brought the vehicle to a Tesla Service Center in April, and at that time, the advisor advised me that they did not yet have a solution for the issue. I was told that because the concern involved the seat belt area, they needed to be careful with the repair and that I would be contacted once a proper solution was identified. I never received that follow-up. I am now being told that the noise is “normal,” which is unacceptable. This is a brand-new vehicle, and I paid a significant amount of money for it. I should not have to accept an ongoing noise that appears to be related to a manufacturing or assembly issue, especially after I was previously told that the concern was not normal and that Tesla would work toward a repair. I am requesting that this matter be formally escalated to management and reviewed again. I would like: A documented case number Review by a Service Manager or Regional Tesla representative Another inspection and road test of the vehicle The rear seat belt noise documented clearly on the repair order A clear explanation of the repair plan or next steps Please assist me in getting this issue resolved as soon as possible. I would appreciate a prompt response and confirmation of who will be handling the escalation. Thank you.

Whenever I disengage full self driving the car forces me to tell it why I did it via an on screen selection. It does not go away after a period of time. I used to be able to dismiss it by hitting the voice button twice but they've removed that loop hole. It's very distracting and forces me to interact with the screen while driving. The selection screen also blocks the blind spot cameras. It comes up even when disengaging to park. Someone is going to wreck messing around with this forced distraction.

Vehicle: 2026 Tesla Model Y System involved: Full Self-Driving (Supervised) Date of incident: September 2, 2026 Approximate time: 5:20 PM Location: I-10 area near Tucson, Arizona Crash: No Injury: No Near-miss / safety-critical event: Yes I am reporting a serious safety incident involving the Full Self-Driving (Supervised) system in my 2026 Tesla Model Y. On September 2, 2026, at approximately 5:20 PM near Tucson, Arizona, I was using Full Self-Driving (Supervised) while traveling toward I-10. The vehicle was expected to enter I-10 but did not enter the highway as intended. Instead, the vehicle proceeded onto a service road, then entered an abandoned parking lot. Rather than safely correcting the route it started towards highway ramp against the normal direction of traffic. I immediately took manual control, stopped in an empty emergency area between the ramp and the service road and activated my emergency hazard lights. I remained stopped while approaching traffic slowed after observing my vehicle. Once it was safe to proceed, I manually and slowly repositioned the vehicle in the correct direction and entered I-10 using the ramp. There was no collision and no injury. However, I believe this event created a serious and potentially fatal risk because the vehicle’s automated driving system directed the vehicle toward a highway ramp in the wrong direction, creating the possibility of a wrong-way conflict with incoming traffic. I am also notifying Tesla for evaluation. I wonder if

Ever since our 2026 Tesla Model Y was purchased in June 2026, we routinely receive a "Center Brake Light Condition Detected" error message. We have brought the vehicle in for service, and have been advised that the issue is not hardware-related and will be fixed through a "future software update". We have been waiting nearly 3 months for a remedy but Tesla Service has been unable to provide a status report on the remedy. A number of other Tesla owners have commented on Reddit that they have the same issue with their vehicles.

It is a safety complaint about the automated parking/FSD reverse maneuver. My brand-new Tesla Model Y (VIN: [[XXX]]), which I purchased XXX and which had only 819 miles on the odometer at time of the incident. Following the over-the-air software update to version 14.3.7, the vehicle's behavior changed. It began automatically selecting a reverse parking maneuver into my garage. Over the days following the update, the vehicle successfully completed this maneuver approximately 7–10 times without incident, which established a reasonable expectation that the system was handling it competently. On XXX at 03:34 PM, while executing this same reverse parking maneuver under my supervision, the vehicle struck my garage, causing damage to the rear bumper and quarter panel area. The prior version of software stopped when not able to correctly park in garage. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The contact owns a 2026 Tesla Model Y. The contact stated that while the vehicle was in Full Self-Driving (FSD) Mode, while reversing into a parking space, the vehicle initially stopped while approaching a pillar on the driver's side; however, the vehicle then proceeded to park and scraped the driver’s side of the vehicle against the pillar. The contact was concerned about the vehicle failing to stop while approaching an object. The contact stated that the failure might have been reported to the manufacturer through the steering wheel voice recorder. The failure mileage was approximately 10,000.

On approximately August 15th,2026, I experienced a serious near miss while driving my 2026 Tesla Model Y at approximately 70 mph on an interstate. I activated my turn signal and began moving toward another lane while another vehicle was in or approaching my blind spot. I did not receive an audible blind spot warning early enough to alert me before beginning the lane change. The audible warning occurred only after I was already moving toward the occupied lane and the situation had become dangerous. My previous vehicle, a 2015 Hyundai Santa Fe, provided an audible warning when the turn signal was activated and another vehicle was detected in or near the blind spot. That earlier warning gave an opportunity to cancel a lane change before a dangerous situation developed. I believe the Tesla Model Y should provide an optional early audible blind spot warning when the turn signal is activated and another vehicle is detected in or rapidly approaching the blind spot. This would supplement the visual warnings and could help prevent high-speed lane-change collisions without requiring the driver to look toward a display. I respectfully request that NHTSA evaluate whether the timing and design of audible blind spot warnings in the Tesla Model Y provide drivers with adequate advance warning and whether an earlier audible alert could improve safety and save lives.

NHTSA publishes every complaint as the owner wrote it and investigates only a fraction. Spelling, capitalisation and judgement are the filer’s own. What carries weight is repetition: the same component failing the same way across many filings, and whether the manufacturer eventually opened a recall for it.
