My vehicle’s auto-high beam system does not properly recognize other traffic. It specifically fails to substantially dim the high beams when approaching oncoming vehicles or when behind other traffic on roadways. I have reported this to the manufacturer and attempted to have the dealership fix this on multiple occasions. The dealerships insist that the vehicle dims the required number of pixels in the headlights to not blind other drivers. However, this does not appear to be effective as other drivers that I have followed behind reported that they were being blinded by my highbeams while I was behind them. There is a notable difference between the highbeams and the low beams which the car fails to substantially adjust for when using this required feature to utilize the self driving/auto pilot feature.

I would like to report a significant incident involving the auto-corrective steering in my vehicle. While driving on the highway, the steering wheel experienced a strong jolt, which I believe was caused by the auto-corrective steering system. This resulted in the vehicle spinning three times and ultimately coming to rest on the grassy center divider. Fortunately, no damage occurred at that time, and I initially attributed the incident to road conditions. I continued driving for approximately 30 minutes in rainy weather without further incident. However, a few hours later, during my return journey, the same forceful jolt to the steering wheel recurred about 20 minutes into the drive. On this occasion, the car spun 1.5 times and subsequently collided backward with the concrete center divider. It is fortunate that no other vehicles were involved, given the heavy traffic and rainy conditions at the time. During both of the incidents, I had both hands on the steering wheel and was not able to correct steering due to the strong force from the steering wheel jolt. I did have a Police Report filed, but have to get number when I return home.

Vehicle appears to be giving off toxic fumes causing medical symptoms like headaches for multiple drivers over last 2 months since vehicle was purchased used, fumes have a sweet smell but source of fumes was unable to be identified by Tesla service center. Fumes appears to be coming from front vents of vehicle. Suspecting there is a coolant leak but source of leak is unknown. Tesla had indicated that coolant reservoir was slightly below expected levels. Tesla replaced cabin air filters but that did not resolve the issue, cabin air filters had already been replaced prior to tesla replacing the filters. Tesla service centers indicated they do not have the equipment needed to test what chemicals are in the fumes and have no idea what they could be.

Seats
Filed Nov 15, 2025NV

The driver’s seat material is deteriorating prematurely on the left bolster area. The damage affects the structural support of the seat edge, which causes the driver to shift position unexpectedly during turns, braking, and lane changes. This instability can delay reaction time and reduces secure seating posture. The deterioration is progressive, and if it continues, the driver may not remain firmly supported, which creates a safety risk under normal driving conditions. This also suggests a potential materials or manufacturing defect for this model year.

I am writing to report critical safety issues I’ve experienced while using Tesla’s Full Self-Driving (FSD) software version 14.1.3. The recent update has introduced several dangerous behaviors that were not present in previous versions. One issue occurs frequently on neighborhood roads: the vehicle performs sudden emergency braking for no valid reason. For example, during one drive, it abruptly stopped because a few leaves blew across the road. There were no pedestrians, vehicles, or real obstacles present. These false-positive braking events are alarming and could easily cause a rear-end collision. An even more concerning issue arises during highway driving. The car now repeatedly swerves partially into adjacent lanes—without signaling—and then abruptly returns to its original lane. This happens constantly, even in light traffic, and it did not occur in previous versions of the software. This erratic behavior is incredibly dangerous and could easily cause sideswipe accidents or create confusion for surrounding drivers. It is clear that version 14.1.3 is a regression from earlier versions. The system’s decisions are more erratic, less confident, and frequently unsafe. I no longer trust the FSD to operate reliably in environments where it previously performed well. I urge your agency to investigate this software update as it presents a real and growing hazard on public roads.

While driving ~40 mph with no Autopilot or cruise control active, my 2024 Model Y suddenly braked itself from 40 → 0 mph for no reason. The road ahead was clear. I did not touch the brake. My 3-year-old was in the car. We were nearly rear-ended and could have been seriously injured. When I went to Tesla Service, the manager insisted I must pay a $270 diagnostic fee unless they “find a problem,” while also saying the incident was likely caused by “shadows.” This makes me believe they will simply say “no issue found” and charge me despite this being a severe safety malfunction. When I reached out to Tesla Customer Support, they stated it was a known issue. This poses a serious risk of rear-end collisions, injuries, or fatality, and Tesla is not addressing it.

Vehicle Speed Control
Filed Oct 30, 2025CA

Tesla removed the speed limit offset feature in FSD version 14.1.4 and now only uses speed profiles called Sloth, Chill, Standard, Hurry, and Mad Max, and this has made my car extremely unsafe to drive. The problem is that these profiles make the car go either way too slow or way too fast and I cannot adjust it to match actual traffic conditions anymore. To make matters worse, the speed limit detection is broken and frequently shows the wrong speed limit for the road I am on. When I use Sloth, the car never goes over the detected speed limit no matter what, so if the system thinks I am in a 35 mph zone when I am actually on a 55 mph highway, my car crawls along at 35 while traffic is flying past at 60-70 mph. I have people tailgating me, honking, flashing their lights, swerving around me dangerously, and I have almost been rear-ended multiple times because my car is going so much slower than everyone else. Without the ability to set a maximum speed offset, I am stuck going whatever speed the broken detection system thinks is right. If I switch to Standard, Hurry or Mad Max to try to keep up with traffic, sometimes the car goes way too fast for the actual conditions or blows through school zones and residential areas at unsafe speeds. I cannot fine-tune the speed anymore to drive safely with the flow of traffic. I am forced to either be a rolling roadblock and risk getting hit from behind, or use an aggressive profile that might go dangerously fast. This happens every single t

Recently Tesla released it's latest version of FSD, v14.1.4. One of the key features is the revamping of the driving modes: sloth, chill, standard, and mad max. Tesla also removed the ability to manually increase or decrease the speed of the car. This is a problem. Let me illustrate it with a real-life example. Yesterday I was driving of a 40 mph road and the car correctly set the speed. I was using the standard mode so the car allowed an overage of around 10% so the actual speed was 45 mph. In our area this is typical for most drivers. As we entered a village, the posted speed limit was changed to 30 mph. The panel was perfectly visible and readable. The car did not register it and continued at 45 mph. I changed to mode to "sloth" and the car decreased its speed to 40 mph but I was still 10 mph above the legal speed limit. I came across a second 30 mph panel and again the car "ignored" it. Because I did not have the ability to manually reduce the speed of the car, I had to disengage FSD and drive the car by myself. Tesla claims FSD must be supervised. Fine. But removing the ability to manually change the speed of the car should be a requirement not a design preference.

The incident occurred at approximately As I attempted to dive in slowly, the accelerator suddenly malfunctioned and my car creeped in slowly and rapidly increased power without my intention. Despite immediate attempts to brake and regain control, the vehicle continued to accelerate and subsequently collided with a gate, resulting in significant front-end damage. I firmly believe this was due to a mechanical or electronic failure in the vehicle’s accelerator or related control systems. The malfunction posed a serious safety hazard and could have resulted in injuries, and this has affected me mentally. I am requesting a full investigation into this malfunction, including an inspection of the vehicle’s electronic logs, sensors, and accelerator system to determine the cause of this unintended acceleration. I would appreciate your prompt response and guidance on the next steps to resolve this matter, including vehicle inspection, repair, and coverage options.

After Tesla Berkeley performed an alignment on my 2024 Model Y on July 22, 2025, the car suffered a complete systems failure while driving on the freeway. Within minutes of leaving the service center, multiple safety systems shut down at once — steering assist, emergency braking, lane keeping, stability control, etc... all turned off, and the car went into manual mode. The vehicle became extremely difficult to control at highway speeds, forcing me to steer and brake manually without assist. Because the Service Center was closed that evening, I couldn’t contact anyone, so I drove back first thing the next morning (July 23) to report the issue. I was initially told the next available appointment was two weeks later, but I had to beg to be seen the same day because the car was unsafe to drive. Tesla redid the alignment and told me the car was fine. After reviewing the alignment reports, both the July 22 and July 23 data are mechanically impossible. The reports show rear camber changes from −1.8° / −2.3° to −1.5° / −1.6°, then to −2.0° / −1.8°, even though rear camber on a stock Model Y is not adjustable. This means the alignment equipment or calibration was inaccurate, and therefore the toe settings were also unreliable. The car was almost certainly released with incorrect rear toe, which explains the immediate handling instability and safety system shutdowns. A later independent alignment on October 13, 2025 (America’s Tire, Roseville) confirmed the correct geometry (−1.9° / −2

I was driving in McKinney, TX on [XXX] taking a friend to his destination. FSD (full self driving) was turned on and enabled for this drive. Normally the car is extremely good at noticing its surroundings, including school zones amidst other things. It is getting better all the time with software updates. However, I turned right into a school zone where the car did not notice that there were other cars going to pick up children. The school zone sign looked like it was abnormally above the ground and so there is a possibility FSD didn’t read it. Unfortunately a police officer noticed me and by the time I did the car was going 16 miles over in a 20 mph school zone. No warnings or anything was shown on the screen to indicate that I had come into a school zone, the speed limit was showing at 40 mph and not 20. Car did not indicate to me that I was not paying attention to the road or doing anything dangerous; it was just going with what it perceived to be flow of traffic. I am filing this complaint to report a serious safety defect in the Tesla Model Y. On [date], my wife suffered a rear tire blowout at freeway speeds in our Tesla Model Y. Despite regularly checking tire tread depth and relying on Tesla’s built-in tire monitoring systems, the blowout occurred because the inside edges of the rear tires had worn down to exposed cords, while the outside tread still appeared to have ample depth. Tesla Service attempted to explain this away as “normal wear and tear” and a need for alignment. However, this is misleading. The Model Y’s rear suspension has built-in negative camber that cannot be adjusted from the factory. Alignments can only adjust toe, not camber, so this design characteristic inevitably causes accelerated inside-edge wear regardless of alignment. More critically, this risk is not disclosed to consumers: It is not explained during the sales process. It is not documented in the Owner’s Manual. Tesla’s sensors only track pressure, not tread depth, meaning there is no warning system to alert the driver. The average consumer has no way of knowing they must crawl under the car to inspect the inside edges of the tires. To an owner, the tires look safe until catastrophic failure occurs. In addition to the safety hazard, this hidden defect creates a financial burden. Because of the blowout, I was forced to spend $1,200 on new tires unexpectedly. This premature and unforeseen cost compounds the danger by placing families under financial stres

There were no vehicles in front of my car. The car suddenly lunged forward as the brakes were automatically applied. Simultaneously, the steering wheel began to jerk back and forth causing the car to veer back and forth. Vehicles around me began braking as well to avoid rear ending my car and to avert hitting my car as it veered back and forth. The steering wheel used to be centered in the middle. when you turn right, its resistance would turn it left back to the center point. or when you turn left, its resistance would turn it back right towards the center point. so after each turn such as u-turn, one would grab the steering wheel lightly so it can slide back to the center position. i will call it the "no resistance" point for that centered steering wheel position. now the problem is, that no resistance point has moved a few degrees right and it brings the wheel with it. i now have to turn the steering wheel left (with the resistance that it wants to go back to right) to make the steering wheel centered and go forward straight. car goes forward straight when the steering wheel is centered so it's not an alignment problem. but in order for the steering wheel to be centered, i have to turn it slightly left because the no resistance point has shifted a few degrees right. now this is dangerous because the car can now take you off the lane easily. car is equipped with lane keeping assistance and i find the system helpful but the system is not activated some times under some circumstances. I visited a dealer and they claimed the wheel is centered that there is no need for any repairs.

Shortly after i purchased this car it began to put itself into park while the car was moving slowly and even when it was in car wash mode going through a brush carwash that requires cars to be in neutral. i reached out to tesla support to investigate and they explained this is a safety feature that engages if the driver leaves the seat while the car is not in park. i explained to them that in both cases i had been sitting firmly in the seat and there should have been no reason for the car to put itself in park and asked them how i turn this auto feature off. they said there was no way to turn it off. i explained to them that ALL automation features should be optional, they argued with me. came to find out some sensor in my driver's seat was malfunctioning. they took the car to replace the seat/sensor, but i reiterated that all these auto features should be optional and a consumer should be able to turn them off. my car was rendered unsafe to drive due to the malfunctioning sensor, i should have had a way to turn off the feature, i asked them to open a ticket with their engineers to add settings to turn off this automation feature. fast forward to 6/25/25, as i am pulling my car into a parking space at a store the car SLAMS itself in park, so hard that i thought maybe i had hit something hard with one of my wheels. i followed back up with Tesla support and am being told there is still no way for me to turn off this automation feature. no automation feature should EVER be hard

I am writing to formally raise a complaint regarding a serious malfunction that occurred in my Tesla Model Y (2024) following a recent software update pushed by Tesla. A couple of days ago, my vehicle received a new over-the-air software update. After installation, my car began displaying two error messages, and key safety features, including detection of nearby vehicles and warning systems, ceased functioning. This creates a significant safety risk when driving, as the car no longer alerts me to surrounding traffic or visualizes other vehicles on screen. Upon contacting Tesla customer service, I was instructed to schedule a service appointment, which I did for May 21, 2025, at 2:15 PM. During the appointment, the Tesla service advisor confirmed that the issue began only after the software update. However, I was told I would need to pay $3,000 out-of-pocket to repair the issue because my car is no longer under warranty. I raised the point that the malfunction was caused directly by Tesla’s software update. Despite this, I was informed that Tesla refuses to repair it at their cost, even after acknowledging the connection between the update and the malfunction. Moreover, the technician informed me that this issue could escalate and create safety hazards in 4–6 months, yet no resolution or recall has been offered. Given the acknowledgment that: The problem was introduced by Tesla's software, The malfunction affects critical safety systems, and There is imminent risk to safety, I

The vehicle was operating in Autopilot mode as it approached a right turn but was in the incorrect lane. The vehicle accelerated in an apparent attempt to merge into the correct lane and initiate the turn, but it approached the turn at an unsafe speed. I pressed the brake pedal, which disengaged Autopilot as expected, but the vehicle failed to slow down or stop. I attempted to manually complete the turn, but although I turned the steering wheel, the wheels did not respond. The vehicle continued forward at speed, struck a curb on the opposite side of the road, and then collided with a parked vehicle. The impact with the parked vehicle ultimately brought the car to a stop. Multiple error messages briefly appeared on the screen during the incident before disappearing.

Unintented Accelaration May 15th, 2025 - at about 2 pm the car was being parked in a small local shopping area parking lot by my wife. as she was pulling into the lot, the car suddenly accelarated and surged forward and she had to slam on the brakes to avoid any collision and getting out of control. after about 30 minutes, as she was pulling out of the parking in reverse, a similar experience, the car accelerated/surged backwards and she again had to slam the brakes to avoid any collision with other cars. this time, she also felt a loud thud under the rear portion of the car, no damage noticed. have sent a note to tesla service for it to be evaluated

The automatic rain-sensing wipers on my 2024 Tesla Model Y fail to activate during rain from delivery date, leaving the windshield uncleared and forcing manual activation. To be clear, this feature does not work at. This happens consistently and creates a significant safety hazard due to impaired visibility while driving. Tesla uses a camera-based system rather than a dedicated rain sensor, and despite service visits and software updates, the issue persists. I believe this presents an undue safety risk that should be investigated.

What carries weight is repetition: the same component failing the same way across many filings, and whether the manufacturer eventually opened a recall for it.
