While driving on highways using adaptive cruise control, the vehicle will slow down quickly from highway speeds of 60 to 75 mph, down to 40 or 45 mph without the presence of an obstruction in front, such as a vehicle, or even a shadow, that could fool the vehicle’s camera. This puts us at risk of being rear-ended on the highway. The problem happens randomly. I have reported the issue to Tesla twice. It happened once while on the phone with a Tesla representative that had called me back regarding my reported complaint. The Tesla rep said they were aware of the issue and were working on correcting the issue via a software issue. Tesla has cancelled my requests for service appointments regarding this issue. In September of 2023 we attempted to use the cruise control to see if it had been repaired but it happened again. There were no prior warnings. The issue first appeared in September of 2021. It has happened many times since then. I first reported around Sep of 2022.

Subject: Safety Concerns Regarding Recent Model y Autopilot Update To Whom It May Concern at the National Highway Traffic Safety Administration (NHTSA), I am writing to urgently express my concerns about the latest software update for the Tesla Model Y’s autopilot system, which, in my view, significantly compromises safety. As a Model 3 owner for over one year, I have generally found the vehicle and its features to be reliable and safe. However, the recent update has introduced an overly stringent hands-on-wheel detection mechanism that is not only inconvenient but also potentially hazardous. The new update requires frequent and often forceful interaction with the steering wheel to assure the system of driver presence. This change is drastically different from my previous experience, where I received only one hands-on-wheel violation in two years. The current sensitivity of the system disrupts the smooth operation of the autopilot, leading to frequent and abrupt disengagements. I have found myself struggling to maintain the system’s activation, inadvertently causing the vehicle to exit autopilot mode multiple times. This issue goes beyond mere inconvenience; it actively detracts from driving safety. The need for constant and sometimes aggressive adjustments to satisfy the system’s requirements is distracting and can lead to erratic vehicle behavior. The irony is stark: a system designed to enhance driving safety and ease is now a source of potential danger and stress. The unp

What component or system failed or malfunctioned, and is it available for inspection upon request? Emergency Breaking in Autodrive mode. How was your safety or the safety of others put at risk? My car auto breaked hard for no apparent reason when I was driving at 75 MPH on an open highway. Has the problem been reproduced or confirmed by a dealer or independent service center? There are plenty of articles online about Phantom Breaking. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No, this happened yesterday morning on 11/18/2023. I plan to report this to Tesla today as well. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Not a single warning and this was the first time that this has happened to my vehicle. I was driving using auto drive at 75 MPH on highway at 8:45AM on 11/18/2023. All of a sudden, and for no reason, my Tesla Model Y slowed down within 3 seconds to around 45 MPH. This was completely unexpected as no other car, debris, person was near me. When I got. home that day, I read about phantom breaking with the Tesla. What happened to my car is yet another example of phantom breaking. This could have caused a major accident if there were more cars on highway at the time. Luckily no one was injured, but my daughter and I were in shock and shooken up.

Front left wheel of vehicle started pivoting under causing tire to bottom out on inner fender without any prior incident or issues. Inspection revealed one of two bolts missing on the lower lateral control link, which was later found in the driveway. Second bolt was 1/3 loosened. Car was towed to Tesla and they proceeded with repairs to include replacing both sets of lower lateral control links. After looking into this further, we found Tesla service bulletin SB-21-31-003 which issued a recall for this problem for 2019-2021 model Y and 3, however, according to Tesla our vin was not included in this recall. These bolts hold the linkage that secures the wheel to the frame, without which the wheel and tire assembly will likely separate or collapse causing major damage and possible death to any occupants that are traveling at normal highway speeds. The SB refers to a torque inspection for these bolts that “may not have been torqued to internal specifications.” The problem appears to be wider than the recall vin range as this was precisely what occurred on our model Y.

I took vehicle to Tesla and had to pay $387 for diagnosis. I was 4,000 miles over warranty. Tesla is saying it could be a possible steering rack replacement and cost $2,500. To the National Highway Traffic Safety Administration (NHTSA), I am writing to formally file a complaint regarding a recent experience with my 2021 Tesla Model Y, which is still under warranty. While using the vehicle, I observed an abnormal sound when reversing. After seeking Tesla’s service, I was informed that the rotors on my car needed replacement. What greatly concerns me is that, despite the car being under warranty, Tesla charged me an amount of one thousand dollars for this repair. It is essential to note that this malfunction wasn’t due to any negligence or external factors on my part. I have video evidence that captures the sound the vehicle made while reversing, further emphasizing the unexpected nature of this issue. My understanding of the warranty terms led me to believe that such unforeseen defects should be covered without any additional charges. It is deeply unsettling to be presented with such a significant expense for a problem that, in my view, should have been resolved under the warranty’s purview. I am reaching out to the NHTSA to shed light on this matter, as it brings forth concerns about the potential for similar experiences faced by other Tesla Model Y owners. I request the NHTSA’s assistance in investigating this incident to ensure consumers like myself are not unduly burdened with unjustified costs. Your attention to this matter is highly appreciated. I am hopeful for a just resolution that aligns with the NHTSA’s dedication to safeguarding consum

The 12-volt battery in the vehicle unexpectedly failed without any prior warning, contrary to Tesla's assurance that a warning should appear on the dashboard screen in advance to indicate the need for replacement. In our specific case, no such warning was given, resulting in the vehicle becoming inoperative. This situation could pose a significant safety hazard if the car had stopped in the middle of the road. On September 12, 2023, my son experienced an incident where he could open the driver's side door, but the dashboard screen went completely blank. Despite attempting to reboot the system, there was no response. After exiting the car and closing the door, we observed that the window did not behave as expected in typical situations, as it did not slide down when the door was closed. This unexpected malfunction led to the window breaking without any prior indication or warning from Tesla. The absence of any alert regarding this electrical failure is concerning, especially considering the potential danger if such an incident occurs on a busy highway. Regrettably, Tesla's service center has firmly asserted that the vehicle is no longer covered under warranty and has declined to provide any assistance or resolution for this issue.

On Wednesday, our 2021 Tesla Y - which we have LOVED driving for the last 3 years - took off on its own while I was sitting in traffic - where traffic was at a standstill, and the max speed was probably 4 MPH. Construction ahead had closed a lane, so there was a massive back-up and cars were creeping along each time the traffic light ahead turned green.) As any Tesla owner will tell you, the best way to slow down is to remove your foot from the gas pedal, and the car will slow down and come to a stop on its own. That had always been the case for our vehicle until Wednesday. Shockingly, for no reason at all, the car massively accelerated. I slammed on the brakes, but the brakes did not slow or stop the car. Luckily, the car in front of me was only 2 - 3 car lengths away, so my Tesla which was rapidly gaining speed stopped only when it crashed into that car. I have relatively minor injuries, but I am sure I would have been killed and/or killed others if not for the vehicle in front of me forcing my car to stop. Today is Sunday and nobody in Las Vegas was able to help. Customer Service took over 30 minutes just to connect, and was no help. I got transferred around numerous times because they have no solution. The frustrating part was that tesla employee wasn't willing to help. I understand that they might not be able to do anything, but their attitude and the unwillingness to help was just shocking. One of them even told me that there's nothing they can do and I need to contact someone in Las Vegas. Other employee who had no knowledge of the car told me to jump start the car. He gave me an attitude when I told him I cannot open the frunk. He told me to read the manual to manually open the frunk. I replied stating I already did read the manual already and do not have the tool to pop open the side frunk. I'm just disappointed with being transferred around and the waste of time while my child was stuck inside the car. Currently, there's no manual door unlock from the outside! There's no manual frunk or trunk unlock either from the outside. All Japanese cars, including hybrid and electric has emergency door lock built into their car.
