Defect. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Lexus NX350, NX250, 2023-2026 RX350, and 2024-2026 TX350 vehicles. The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Risk. A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.

Remedy. Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.

Defect. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.

Risk. An instrument panel display that does not show critical information can increase the risk of a crash or injury.

Remedy. Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Defect. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 Toyota Grand Highlander, Grand Highlander Hybrid, Lexus TX350, TX500 Hybrid, and TX550 Hybrid+ vehicles. The driver and passenger side curtain air bags may not unroll as intended and could partially deploy outside of an open window. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 226, "Ejection Mitigation."

Risk. An improper deployment of the side curtain air bag can reduce occupant protection during a crash, increasing the risk of injury.

Remedy. Dealers will replace the driver and passenger curtain shield air bag assemblies, free of charge. Owner letters were mailed November 4, 2024. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 24TB09/24TA09 and Lexus' numbers are 24LB05/24LA05.

2024 Lexus TX 500h Luxury. Since I bought it new, the vehicle has intermittently failed to accelerate from a stop. I have reported it to my Lexus dealer at every 5,000-mile service. Lexus TAS case numbers:[XXX] and [XXX]. The dealer applied Lexus TSB L-SB-0026-25 (Hesitation From a Stop), a TCM reprogram. It reduced how often it happens but did not fix it. It still occurs, and is worse in Arizona summer heat with the A/C on max. It matches the TSB conditions: it happens when the energy monitor shows about 30% (3 of 10 bars) and within about 5 minutes of starting the vehicle. What happens: I am stopped at a green light waiting to turn left. When a gap opens I press the accelerator, and the vehicle coasts through the intersection at about 5 mph even with the pedal fully to the floor. I have nearly been hit by oncoming traffic while stuck in the intersection. A left turn across traffic depends on the vehicle accelerating when commanded, and this one does not. I no longer trust the vehicle to accelerate when I need it, so I have to wait for much larger gaps and take extra care on every turn across traffic. A second symptom: pulling away from a red light, the transmission suddenly downshifts and the vehicle lurches forward. The engine revs unexpectedly with no change in pedal input. My dealer has told me they have seen other TX 500h vehicles with this same problem after the TSB was applied. The contact owns a 2024 Lexus TX 500H. The contact stated that while driving at an undisclosed speed the "Hybrid System Malfunction" warning light illuminated. The contact stated that several days later, a secondary warning light illuminated for the battery failure. After the third failure, the "Hybrid System Malfunction - Contact Dealer" message was displayed. Additionally, the vehicle failed to start and was jumpstarted. The vehicle was towed to the dealer on several occasions for the hybrid battery and related electrical warning failures. The contact stated that there was excessive heat inside the vehicle. The manufacturer was notified of the failure. The failure mileage was 21,000.

The car emits very strong gas fumes into the cabin during longer drives. You can taste the gas. Has happened over 5 times. Submitted for service multiple times; on the 4th time, they were finally able to "recreate it". And the dealer confirmed the issue. No warning or check engine lights came on.

Approximately two weeks later, the air conditioner failed to blow cool air. The vehicle was taken to a local dealer, where it was determined that the A/C condenser was damaged by the debris, causing a coolant leak. The dealer stated that the condenser needed to be replaced. Odometer Fraud: The contact owns a 2024 Lexus TX 350. The contact stated that a mileage discrepancy was discovered. The vehicle was a dealer sale. At the time of purchase, the vehicle mileage was 39,000; however, during a service appointment, it was discovered that the vehicle mileage was 41,800.

I am reporting a severe, systemic vehicle performance discrepancy on my Lexus TX 500h (2.4L Turbo Hybrid powertrain). The vehicle is officially labeled with an EPA rating of 27 MPG combined. However, under normal, conservative driving conditions, the vehicle routinely achieves only 20 to 21 MPG—a consistent 20%+ shortfall from the manufacturer's certified window sticker metrics. This has drastically limited the vehicle's safe total driving range to under 300 miles per tank. The shortfall persists despite cautious driving in Eco Mode and after passing the engine break-in period. This issue matches a widespread pattern reported by numerous other owners in consumer forums, pointing to a potential defect in the vehicle’s fuel management software or hybrid battery management logic. I request that NHTSA log this tracking data to investigate whether the manufacturer misrepresented the vehicle's real-world operational capabilities or fuel-system integration.

I had my foot on the brakes waiting at a red light. The car just started jerking back and forth. It didn’t stop until I released my foot off the brake.

I am filing this complaint regarding my 2024 Lexus TX. A road rock/debris entered through the vehicle’s factory front grille and struck/punctured the A/C condenser, causing the air conditioning system to stop functioning properly. The repair cost is approximately $2,500. The factory grille has large openings that leave the condenser vulnerable to direct impact from normal road debris. Lexus does not have a protective screen or guard that can be installed to prevent this type of damage. This design flaw has affected many other Lexus TX owners and I think they need to offer up more protective measures to avoid this from happening over and over again.

Strong smell of gas coming from around the car and near rear end of car. Usually after a drive. The smell does not happen after every drive. I brought up the smell to my husband a number of times within the year…and finally took to JD Auto in Arizona in June but the gas smell could not be replicated and no source or defect could be visually seen. No recall for this has been reported by Lexus. dealership. No check engine light has been recorded. Worried that there is some internal issue that could cause huge problems. I do have children who have also reported smelling the gas in the back seat.

On two occasions while driving my hood has become open due to windy conditions coming from the Makua side of the highway as I’m driving on the Queen Kaahumanu Highway Today was the second incident except that I was heading south The first time that happened I was driving north and it occurred probably about a few months ago, but I dismissed it I know for a fact that I did not open the engine compartment I will download my front camera files and see if I can find a record

I have 2024 Lexus TX 350 and the winshield cracked from the smallest rock and spread across the entire winshield. Seems to be a flaw in material.

Starting in drive with the brakes depressed, when trying to accelerate with the gas pedal, the vehicle will not activate the engine from electric mode(EV). The car will not accelerate even with the gas pedal fully depressed. It has happened four times so far. Safety issue because when I’m pulling out into traffic to make a left or right turn, the car will not accelerate and therefore can get hit by oncoming traffic, especially when I am trying to merge into a lane with oncoming traffic. The car crawls and will not accelerate. My car is a hybrid vehicle. It almost feels as if the non electric engine doesn’t kick on when it is supposed to. The car will crawl for about 5-10 seconds before the engines engages and kicks on the acceleration. There are no warning lights or indication that anything is malfunctioning when this happens. I have not taken the vehicle in to the dealer yet.

I am a loyal Lexus owner since 1998 and have always recommended Lexus vehicles to my family and friends. In over 25 years of ownership, I have never experienced an issue like this. While driving under normal road conditions, my new 2024 RX 350’s AC condenser was punctured by small road debris, causing a refrigerant leak and loss of air conditioning. The condenser is located directly behind the front bumper, leaving it exposed to minor debris, which I believe is a design vulnerability. Damage at such low mileage suggests this may affect other vehicles of this model. Reduced AC performance could impact driver comfort and visibility in extreme heat, and operating the system with low refrigerant may cause further mechanical damage. I am submitting this complaint to request that NHTSA review this as a potential design defect affecting multiple vehicles. I have also contacted Lexus for warranty repair.

The vehicle's lower front fascia exhibits a design flaw, creating an unsafe gap that permits road debris to impact the condenser, leading to air conditioning system damage. This compromise in the A/C system's functionality results in an inability to adequately cool the vehicle's interior, thereby creating potentially unsafe operating conditions, particularly in high-temperature environments. It has been observed that dealerships are declining warranty coverage for these repairs, and the manufacturer has not provided any countermeasures to address this inherent design deficiency.

The plastic casing holding the driver seat cushion broke/detached itself after a couple of months of driving and even if put back in place it'll continue sliding and moving. It can interfere with my left foot while driving making it a hazard. Lexus reviewed the issue and said it was not covered by warranty.

If I remove the cover I see that a plastic attachment linking the spare wheel well to both sides of the trunk is bent. Lexus says that to replace it I need to replace the whole unit and it's also not covered by warrantee.

The vehicle was towed to Mayfield Wrecker Service, and then it was towed to the dealer. The cosmetic part was repaired on the vehicle. However, the safety issues were not repaired. The vehicle is at the residence. The insurance company was contacted. The contact related the failure of the side curtain air bags to NHTSA Campaign Number: 24V461000 (Air Bag). However, the VIN was not included. The manufacturer was not made aware of the failure. The failure mileage was approximately 18,000.

The AC stopped working and now they are charging me to replace the condenser which is located right in the front behind the grill. They said it was damaged from a rock that hit it. There are thousands of these vehicles that have this poor design where all these condensers are getting hit and having consumers pay for this damage because it is not under warranty. These should be recalled or repaired for free as it is obvious there should be some kind of screen to prevent this as the grill gives no protection.

Driving on the highway, heard a very loud pop, followed by a crackling noise above us. Investigated and found the sunroof had spontaneously shattered. On the drive home, the sunroof glass collapsed into the car. The initial incident was very disruptive, and we were lucky that the sun shade was closed, which blocked glass fragments from falling on us. There are no signs of impact from any objects or rocks. There were no cars driving in front of us, and we did not drive beneath an overpass. There were no dents on the car. Photos of the glass after shattering but before collapse dont show a clear sign of an impact point. My best guess is a manufacturing flaw exacerbated by temperature changes. It was a very sunny day, and we had the shade open with cold AC blowing for 30+ minutes. The sun became a distraction, so we closed the shade. I think the heat then built up quickly between the shade and the glass, and caused the glass to shatter.

I was driving on a highway around 65mph, about 1:30PM on May 11, 2025. The temperature outside was about 70 degree F. I heard a very loud noise like a big firecracker exploding. Then I heard the sound of air coming from the sun roof. I opened the sun roof door and found the sun roof was shattered and had a big hole more than a foot in diameter. If I didn't close the sun roof door, I could had been injured by the exploding tempered glasses from the sun roof and lose control of my SUV. I drove to the Lexus dealership and leave my car there overnight since the service department was closed on Sunday. The Lexus service advisor will call me in the morning. I googled the topic about "Lexus sun roof exploding while driving" and found many Lexus car owners had the same problem as mine. Toyota Motors already knew about this issue since 2012 and they refused to fix the issue free of charge for Lexus car owners since they said that the car warranty doesn't covered the sun roof. I believe Toyota Motors is a irresponsible car company. If this issue is not fixed, I believe some Lexus owners might get hurt when their cars' sun roof explode while driving. Toyota Motors should fix the problem free of charge for their customers because this is a defective tempered glass sun roof.

Starting from a stop in a residential neighborhood, stepped on the gas to go and turn left and the vehicle hesitated and lacked power. It almost felt like it was trying to start from a stop in too high of a gear, but this is a 6 speed automatic vehicle. After panicking a little, backing off the accelerator pedal and stepping on it again, power came back so I could complete the turn. If there were other cars coming straight through the intersection, I would have been hit. Informed my local dealer about it and that the Toyota Grand Highlander Hybrid has a service bulletin (T-SB-0017-24) to address this very issue I encountered and that the Lexus TX500h has a similar drivetrain set up, but was told Lexus has no such service bulletin to address this issue. There were no warning lamps or indications from the vehicle that anything was wrong and it has only happened once so far, a few days ago, since I purchased the vehicle new at the beginning of December 2024.

When turning the truck left, there is a clicking sound that sounds like it’s coming from the axel. Contacted Lexus and it is a known issue. They advised it is not a safety concern but a mold problem. Getting axel replaced.

I have a Nuna Rava carseat that has malfunctioning harness straps. The straps can be pulled loose even after proper buckling and tightening. My child is at risk of injury, bodily harm, or death in the event of a car accident due to the harness straps failing to stay tight. The car seat has not been inspected.

I recently bought this vehicle and the drivers seat height does not raise approropriately. I have brought it to the dealership for them to take a look and they say that’s how it’s designed however the passenger seat height goes much higher. I called Lexus corporate who mentioned there is nothing they can do. I have driven the LX and RX models that do not have this issue. The contact stated that several messages indicating the brake system, the Advance Autosonic detection, the Intelligence alert, the ABS, the parking brake, the electric power steering, the electronic stability control systems malfunctions, and other malfunctions were displayed on the vehicle mobile app. The contact stated that the front driver's side air bag and the knee air bag failed to deploy as intended and partially deployed. The ambulance transported the contact to the hospital, where a CT scan and X-Rays were performed, and the contact was informed that an internal neck injury was found. My Lexus TX 500h has an intermittent problem. It seems to have trouble shifting between 1st gear and second gear. Sometimes when I try to drive from a stop, the car will go a tiny bit and then feels like it switches into neutral instead of the next gear, leaving me stopped in the middle of traffic or an intersection. I have been stuck pulling out of my street onto a busy street with traffic bearing down on me. I have also started rolling backwards on a hill when my foot is on the gas and the car is in drive. It seems that the car puts itself into neutral vs second gear. I have taken it into the dealer twice for this and they “cant duplicate the problem” so tell me to go home and bring it back if it happens again. It keeps happening and it’s a huge safety issue. I’m not sure where I go from here.

Recall has been since June and still no remedy. Lexus needs to offer buy backs to customers.

Then the Lexus of Orlando dealership has lied and treated me like crap because I don’t feel safe in a car that has a high ticket price. Refusing to do anything to help me get out of the car with statements such as”you’re going to have to eat the depreciation” this is sickening to know I have to deal with this, not to mention my insurance company has threatened to drop coverage on the car!

The manufacturer, Lexus, is unable to remedy an airbag safety recall (24LB05/24LA05) for my vehicle in a timely manner. Additionally, they are unwilling to provide a comparable 7-seat loaner. Our insurance has also threatened to drop coverage for the vehicle as it is unsafe to drive. In the meantime, we are unable to drive or sell the vehicle.

Failure of component are the curtain shield airbags. The safety of my customers are at risk when car is being rented out due to this concern the Lexus TX has not been in use, for safety concerns. This issue is a known safety risk which was known to the public on June 20th, 2024.

When I shift my car to get it in gear the knob comes unscrewed and have difficulty getting the car back to drive. This happens especially when putting it in neutral for a car wash. The knob is not securely tightened and if you try to tighten it is not in the correct position. It happens frequently and has almost caused an accident several times.

Driver and passenger Curtain airbags may not deploy as intended. The vehicle would not have been purchased and driven around had this been known at the time of purchase. Lexus has no remedy and this is completely Unacceptable.

Lexus has failed to provide a timely repair for this simple recall issue, and has not provided any remedy or free loaner car. Their dealers rep has said to just "drive with the windows up, and you will be fine".

THERE IS NO FIX AVAILABLE YET AND MY CAR IS UNSAFE TO DRIVE. THIS IS UNACCEPTABLE TO ME AS A CONSUMER. ADDITIONALLY I CAN NOT TRADE IN MY CAR TO PURCHASE A SAFER VEHICLE.

Severe safety issue! Pulled the vehicle into the driveway after a drive and put the vehicle in park and turned the car off. With key still in the vehicle, exited vehicle, closed driver door and went to open drivers-side rear passenger door where 2 year old baby was in car seat. Before being able to open the rear door, the car went dead and locked down all the doors (key is still inside the car). Could not get into car through any of the doors, including rear hatch. Tried the manual back up door handles (since this vehicle has electronic door releases), but none of them worked. All the while, baby is crying in the car and it's more than 85F outside. In panic, call Lexus roadside assistance. They advise that help may take 90 minutes and with baby in vehicle, we should call 911. After call to 911, fire and police arrive. Fireman tries all manual backup door handles (which are there in case battery dies and electronic release don't work) then takes crowbar to the door (bent door frame and A pillar, and other damage to car). Door still not open! Door is later opened by service tech and we successfully get baby out. Had another incident where after drive, put vehicle in park and turned off vehicle. The vehicle power immediately went dead (no lights, no ignition, nothing) and I couldn't exit car even when using the manual door handles. Had to sit and wait in car for a while until power came back to get out. This is extremely dangerous because someone could get stuck inside the car i

Unknown - my car is in the dealership since June 25, 2024. No remedy yet

The recall was not communicated to me as consumer, when i saw online I was notified and then when called dealer i purchased car i was told ‘we only sell call not part of manufacture process call corporate” and after calling lexus corporate i was told i could get loaner car since i dont feel safe but the dealer on the call with corporate said “if not front air bag its not big deal” for a saftey recall. I told corporate since this is recorded call they have proof of the dealer service associate saying this to me and corporate said just please hang up and we will notify regional offices. I have yet to hear back, i dont think i should be told this recall isnt big deal if they have put hold on selling this car to public nor should be told its okay to drive. I would be given either loaner but dealer does not want to provide this or 55 a day but there is no lexus anywhere you can get for 55 dollars a day. There is not set remedy nor notification to the customer. This is beyond frustrating and do not feel safe by this car and dealer is telling me this recall is not big deal which is very inaccurate and misleading.

On 3 different occurrences. Regarding rear-left gate foot sensor. Vehicle rear lift-gate as closed when feet are planted on ground while unloading groceries from the vehicle. This claim has also been confirm with dealership as Diagnostics Technician was able to duplicate issue. First incident hit spouse on the head. Second time hit myself. 3rd was able to dodge lift-gate closing down. Treasure Coast Lexus Diagnostics technician advised to disable feature and when needed to enable. That doesn't resolve the safety issue as the occurrence of lift-gate closing could still occur when feature is enabled. New vehicle purchase 5/21/2024. Its at dealership waiting for repairs.

The contact owns a 2024 Lexus TX350. The contact stated that while driving at various speeds, several messages related to the various electrical features were displayed on the instrument panel. The Lane Departure, Forward Collision Avoidance, and other various features were displayed on the instrument panel. The vehicle had been taken to the dealer for various repairs and updates were performed on the vehicle; however, the failure returned after each repair. The manufacturer was notified of the failure and a claim was filed. The vehicle was not repaired. The failure mileage was approximately 1,600.

The car gives out from a stopped position, when hitting the accelerator. As if the car stalls without turning off. It’s happened at least 3 times. Dealership is telling me nothing is wrong with the car.

There is significant vibration coming from the driver-side side mirror. Very noticeable on the freeway. As a result, the mirror appears blurry and at certain times of the day, light is reflecting back into my eyes which is dangerous. The other side mirror and rear view are ok. Something seems to be wrong with the driver side.
