Defect. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2018-2019 4Runner, 2019-2020 Avalon, 2019 Corolla Hatchback, 2017-2019 Highlander, 2018-2020 Camry, 2020 Corolla, 2018-2019 Land Cruiser, 2017-2020 Tacoma, 2019-2020 RAV4, 2019-2020 Sequoia, 2017-2020 Sienna, 2019-2020 Tundra, 2018-2020 Lexus ES350, 2017 Lexus GS200t, 2017-2019 Lexus GS350, 2019 Lexus GS300, 2018-2020 Lexus LC500h, 2019-2020 Lexus LS500, 2018-2019 Lexus GX460, 2017 Lexus IS200t, 2019 Lexus IS300, 2019 Lexus IS350, 2018-2020 Lexus LC500, 2018 Lexus LS500, 2019 Lexus LS500h, 2018-2019 Lexus LX570, 2017 Lexus RC200t, 2019 Lexus RC300, RC350, 2017 and 2019-2020 Lexus RX350, 2018-2020 RX350L, 2019 Lexus UX200, 2018-2019 Lexus NX300 vehicles. The low-pressure fuel pump inside the fuel tank may fail.

Risk. If the fuel pump fails, the engine can stall while driving, increasing the risk of a crash.

Remedy. Toyota will notify owners, and dealers will replace the fuel pump assembly with an improved one, free of charge. Owners of specific models were notified that remedy parts were available starting January 21, 2021. Owners of other models will be notified as remedy parts become available. Remedy parts should be available for all affected vehicles by late March 2021. Owners may contact Toyota customer service at 1-888-270-9371 or Lexus customer service at 1-800-255-3987. Note: This recall is an expansion of recall 20V-012. Toyota's number for this recall is 20TA02. Lexus' number for this recall is 20LA01.

On March 4, 2020, Toyota expanded the recall to include certain 2014-2015 Toyota 4Runner and Land Cruiser, 2018 Avalon, Corolla, 2014 FJ Cruiser, 2017 Sienna and Lexus 2018 ES 350, 2018-2019 GS 300, 2013-2014 GS350, 2014-2015 GX 460, IS 350 and LX 570, 2014 IS F, 2018-2019 IS 350, LC 500H and LS 500H, 2013-2015 LS 460, 2015 NX 200T and RC350, 2017 RC 200T and RX 350. Toyota also removed the 2018-2019 Toyota 4Runner and Land Cruiser and 2018-2019 Lexus GX 460 and LX 570 and 2019 NX300 from inclusion in this recall. On March 19, 2020, Toyota expanded the recall to include 2015 Lexus GS350 vehicles. The low-pressure fuel pump inside the fuel tank may fail.

During the manufacturing process, the oil galley in the rotor for the brake booster vacuum pump assembly may have been improperly machined possibly resulting in a sudden loss of brake assist.

Risk. A sudden loss of braking assist can increase the risk of a crash.

Remedy. Toyota will notify owners, and dealers will replace the brake booster vacuum pump, free of charge. The recall began on May 9, 2018. Owners may contact Toyota customer service at 1-888-270-9371, or Lexus customer service at 1-800-255-3987. Toyota's numbers for this recall are J0K/JLD.

The contact owns a 2017 Lexus RX350. The contact stated that upon a cold start, an abnormal knocking sound was coming from the right side of the engine without warning. The contact stated that the knocking noise became louder upon shifting gear but would cease once the engine was warmed up. The contact also noted that the brake pedal was firm while depressing the brake pedal. There was an unknown brake warning message displayed. Upon investigation, the contact discovered NHTSA Campaign Number: 18V211000 (Service Brakes, Hydraulics) which he linked to the failure. The dealer was notified of the failure and informed him that the vehicle was not under recall. The dealer provided an estimate for the repair. The manufacturer was notified of the failure and provided a case number. The vehicle was not repaired. The failure mileage was approximately 25,800.

The contact owns a 2017 Lexus RX350. The contact stated that while driving at various speeds, the vehicle hesitated upon depression of the accelerator pedal, without warning. The contact had taken the vehicle to the dealer and was informed that the vacuum pump was defective and needed to be replaced. Upon investigation, the contact discovered NHTSA Campaign Number: 18V211000 (Service Brakes, Hydraulics) which she linked to the failure. The manufacturer was also notified of the failure and the contact was given a case manager. The vehicle was not repaired. The failure mileage was approximately 24,000. The contact stated that the vacuum pump was replaced at their expense and is requesting a reimbursement. The vehicle always had issues with the vacuum pump ever since the contact purchased it in 2021. The contact claims that they were told that the vacuum pump issue was normal. The 65k warranty of the vehicle had expired before the problem was dealt with.

The contact owns a 2017 Lexus RX350. The contact stated that while driving approximately 60 MPH, the glass sunroof suddenly exploded causing shards of glass to fall into the cabin of the vehicle onto the driver. The contact indicated that no object had struck the sunroof to cause the glass to shatter. No injuries were reported. The vehicle was taken to the local dealer who was unable to determine the cause of the failure. The manufacturer was not yet contacted. The failure mileage was 58,190.

The contact owns a 2017 Lexus RX350. The contact stated that while at a complete stop, the vehicle would intermittently hesitate upon depression of the accelerator pedal without warning. The contact had taken the vehicle to the dealer where he was informed that the cause of the failure was related to defective fuel in fuel tank. Upon investigation, the contact discovered NHTSA Campaign Number: 20V682000 (Fuel System, Gasoline) which he linked to the failure. The manufacturer was not notified of the failure. The vehicle was not repaired. The failure mileage was unknown.

The contact owns a 2017 Lexus RX-350. The contact stated that while driving at an undisclosed speed, a strong chemical odor was detected coming from the cabin filter. The contact experienced severe headaches and a numbing, sour tongue sensation after the failure. The contact had replaced over 12 cabin filters and had the vehicle detailed where the interior cabin was shampooed and cleaned however, the failure recurred. The dealer was made aware of the failure but informed the contact that the vehicle was tampered with and to go through his insurance company for the repair costs. The manufacturer was not made aware of the failure. The vehicle was not repaired. The failure mileage was approximately 19,000.

I WHEN TO A CAR WASH FACILITY AND THE ICS SENSOR WHEN CRAZY!!! (FLASHING THE WARNING LIGHT). NOW MY CAR GIVING ME A ICS MALFUNCTION. THE AUTOMATIC BRAKING DON'T WORK AND THE PARKING ASSISTANCE NEITHER. I BELIEVE THIS IS A MANUFACTURING PROBLEM DUE SAME CAR MODEL AND YEAR EXPERIENCE THE SAME BUT IT HAS DIFFERENT VIN SO THERE IS NOT A RECALL FOR MY VIN.

WHEN FLOORING THE ACCELERATOR IT HESITATES AND BUCKS

THE AIRBAGS DID NOT DEPLOY. THERE WAS NO RECALL EITHER. I DON'T KNOW IF THERE IS AN ISSUE OR NOT BUT I WOULD HATE TO SEE SOMEONE GET HURT BECAUSE AIRBAGS DID NOT DEPLOY SEE PHOTO OF DAMAGE. HAVE PICTURES BUT SIZE PROBLEM DIDN'T KNOW HOW TO REDUCE SIZE

DRIVING ON HIGHWAY, MY VEHICLE STALLED WHEN I HIT GAS PEDAL, THEN IT RESUMED NORMAL SPEED. THIS HAS HAPPEN MANY TIMES, BUT SEEMS TO BE MORE NOTICEABLE NOW. I DON'T FEEL SAFE DRIVING THIS VEHICLE

VEHICLE EXPERIENCED ROUGH TRANSMISSION MOSTLY BETWEEN GEAR NUMBER 2 AND 3 AFTER TAKE OFF. VEHICLE STALLED AROUND RPM 1200 - 1500 DURING NORMAL DRIVE AT HIGHWAY, THEN SUDDENLY SHIFTED FORWARD WITH EXCESSIVE FORCE. ENCOUNTERED VERY DANGEROUS SITUATIONS ON HIGHWAY ON MY RECENT TRIP. I ALMOST HIT THE VEHICLE AT THE FRONT FOR A FEW TIMES DUE TO CONSISTENT ROUGH SHIFT PATTERN. I HAVE TAKEN THIS VEHICLE TO LEXUS DEALERSHIP SEVERAL TIMES TO ADDRESS ISSUE IN THE PAST. UPDATED ECU SOFTWARE DIDN'T RESOLVE THE PROBLEM, AND ALWAYS KEEP COMING BACK. I BELIEVE THIS ISSUE COULD HAVE BEEN RELATED TO RECENT FUEL PUMP RECALL, BUT THE RECALL DIDN'T INCLUDE THE 2017 RX 350 MODELS. PLEASE INVESTIGATE FOR SAFETY ISSUE. THANK YOU.

I WAS INVOLVED IN THE INTERSECTION MOTOR VEHICLE ACCIDENT. WHEN I WAS T-BONED BY OTHER VEHICLE (MY VEHICLE WAS GOING 30-45 MILES/HOUR, AND MOVING) AND THE OTHER VEHICLE (WHO CAUSED AN ACCIDENT) WAS ALSO GOING 30-45 MILES/ HOUR. WHEN THERE WAS AN IMPACT TO MY RIGHT SIDE OF THE VEHICLE. MY REAR PASSENGER SIDE AIRBAG WAS NOT DEPLOYED. DAMAGES DONE TO THE CAR IS MY RIGHT SIDE PASSENGER SIDE (FRONT AND REAR) WAS DAMAGED AND UNABLE TO OPEN OR CLOSE. *TR

MY CAR (INSURED) REAR ENDED ANOTHER PARTY AT LESS THAN 10 MILES PER HOUR. THE PRE-COLLISION SYSTEM THAT IS PART OF MY CAR NEVER BECAME ACTIVE IN MY CAR AND SO I REAR ENDED THE CAR. LEXUS INSPECTED MY RX-350 AND HAVE AN UNACCEPTABLE REASON THAT IT WILL NOT WORK, UNLESS THERE IS A NOTICEABLE SPEED DIFFERENCE BETWEEN MY CAR AND THE CAR AHEAD OF ME. THE CAR WAS THE PROBLEM AND IT DID NOT WORK AND SO I REAR ENDED THE PREVIOUS CAR. MY LIGHTS WERE ALSO BROKEN AND HAD TO BE REPLACED. SOME ELECTRICAL WIRING REWORK WAS ALSO DONE.

VEHICLE ROLL OVER DRIVER AFTER BEING PLACED IN PARK AT THE RESIDENCE CAUSING FATAL INJURIES. DRIVER REVERSED OUT OF GARAGE AND EXITED VEHICLE. INSPECTION AT THE SCENE BY THE SHERIFF'S OFFICE REVEALED THE VEHICLE WAS IN PARK BACKED AGAINST SOME TREES. THIS VEHICLE IS EQUIPPED WITH A AUTOMATIC TRANSMISSION AND A AUTOMATIC EMERGENCY BRAKE. THE GEAR SHIFT LEVER CAN BE PLACED BETWEEN PARK AND REVERSE AND NOT MOVE. IF THE GEAR SHIFT LEVER IS MOVED SLOWLY FROM DRIVE TO PARK OR REVERSE TO PARK THE AUTOMATIC EMERGENCY BRAKE DOES NOT ENGAGE THIS CONDITION IS VERY DANGEROUS.

ALL THE FRONTAL AIR BAGS DEPLOYED (INCLUDING CURTAIN AIR BAGS). THE VEHICLE WAS TOWED AND DEEMED DESTROYED ACCORDING TO THE INSURANCE COMPANY. THE CONTACT WAS TRANSPORTED BY AMBULANCE TO THE HOSPITAL, TWO FRACTURED RIBS AND 2 FINGERS, TREATED AND RELEASED. THE POLICE WAS PRESENT AND REPORT NUMBER: 20192380195 WAS FILED. BLIND SIDE MONITOR ON RIGHT HAND SIDE DOES NOT ALERT WHEN PASSING TRUCKS AND CARS AT HIGHWAY SPEEDS. AT LOWER SPEEDS IT WORKS PROPERLY. WHEN PASSING VEHICLES ON THE LEFT THE LEFT SIDE WARNING LIGHT COMES ON PROPERLY.

PULLED INTO HOTEL PARKING LOT AT 11:05AM (PERFECT WEATHER - PARKING LOT EMPTY) AND ,AFTER TURNING INTO THE PARKING SPOT AT A SLOW SPEED, THE CAR SUDDENLY ACCELERATED AND HIT THE SIDE OF THE HOTEL. THE BUILDING WASN'T DAMAGED, BUT THE CAR HAD SERIOUS FRONTAL DAMAGE. THE HOTEL MANAGER DIDN'T CALL THE POLICE SO THERE ISN'T A POLICE REPORT. THE AIR BAGS DID NOT GO OFF. I FEEL THERE WERE SEVERAL POSSIBLE LEXUS PROBLEMS WITH THIS INCIDENT: THE AIR BAGS DID NOT GO OFF EVEN THOUGH THERE WAS OVER $17,000 IN REPAIRS (EVEN THE ENGINE HAD TO BE REMOVED),; THE AUTOMATIC BRAKING SYSTEM DIDN'T ENGAGE; AND THE SUDDEN ACCELERATION OF THE CAR FROM A SLOW ENTRY INTO THE PARKING SPACE. CURRENTLY THE CAR IS AT A LEXUS DEALER WAITING FOR THE LEXUS HIRED INSPECTOR TO EXAMINE IT. WE HAVE PICTURES AND A COMPLETE LIST OF REPAIRS IF REQUESTED. SOMETIMES WHEN I TRY TO START THE CAR, THE BRAKE PEDAL WILL NOT DEPRESS, I GET A MESSAGE THAT SAYS “BRAKE POWER LOW”, AND THE CAR WON’T START. THERE IS A RECALL ON 2017 RXS FOR AN IMPROPERLY INSTALLED BRAKE BOOSTER, BUT IT DOESN’T COVER THIS VIN. FROM READING ABOUT SIMILAR COMPLAINTS FROM OTHER RX OWNERS THAT ARE ABLE TO SOLVE THE PROBLEM BY REPLACING THE BRAKE BOOSTER, I THINK THAT IS THE PROBLEM. BECAUSE OF THAT, I THINK THE RECALL DOES NOT COVER ALL VEHICLES AFFECTED AND SHOULD BE EXPANDED.

THE EMERGENCY ASSISTANCE BRAKE SYSTEM WAS ACTIVATED AND IT REDUCED SPEED WHEN VEHICLE WAS STOPPED AHEAD OF ME BUT WOULD NOT RESPOND TO MY BRAKING. THE CONTACT'S INSURANCE COMPANY WAS MADE AWARE OF THE CRASH AND REQUESTED THE CONTACT'S APPROVAL FOR THE VEHICLE TO BE ASSESSED FOR DAMAGES BY A LEXUS DEALER.
