Seat Belts1 injured
Filed Aug 5, 2026SC

My son turned around in his second-row seat of our Honda Odyssey, causing the seat belt to cinch tightly around his waist. Even after the buckle was released, the seat belt continued to tighten instead of loosening. It became so tight that he could barely breathe, and we ultimately had to cut the seat belt to free him. There was no way to manually release or loosen the belt once it had locked. This presents a serious safety hazard. Seat belts should include an emergency release or override mechanism that allows a locked belt to be loosened during an entrapment situation.

The rear coil spring snapped with no warning. My safety was at risk because the spring broke and collapsed on the suspension while driving on a tollway. The bottom of the spring that was snapped off completely which presents a safety risk to others as it becomes a heavy sharp steel projectile flying into traffic. The problem has been confirmed by a service center and it is a large issue as the parts are extremely hard to source due to the frequency of these coil springs breaking in Honda Odysseys. They can’t make them fast enough to replace. They are not supposed to break and are not part of any scheduled maintenance. The component has been inspected by a Honda service center. There were no warning lamps or symptoms. The part broke suddenly and without warning.

Vehicle: 2020 Honda Odyssey VIN: [XXX] Mileage at time of failure: 155413 Component: Engine — Connecting Rod BearingsDescription: My 2020 Honda Odyssey experienced a failure of the connecting rod bearings on 7/30/26, resulting in loud knocking noise. The failure was diagnosed by Luis Asturias at Jim Coleman Honda in Clarksville, MD. This vehicle falls within the scope of NHTSA's open Preliminary Evaluation PE25008, which covers 2018-2020 Honda Odyssey vehicles with the 3.5L J35 V6 engine for this exact failure mode. My dealer declined to cover the repair cost under warranty because no recall has yet been issued. I am filing this complaint to document my experience as part of the ongoing federal investigation. Repair cost estimate: $4573.00. The vehicle is not driveable without this repair.Thank you for your prompt attention,[XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Power Train
Filed Aug 1, 2026MD

At roughly 5,000 miles, this 2020 Honda Odyssey was brought into an authorized Honda dealership due to transmission concerns. The dealership service department improperly evaluated the 10 speed transmission, and diagnosed a 9 speed transmission system, assured me that what I was feeling was completely normal, and instructed me to only bring the vehicle back if the issue got worse. At approximately 60,000 miles, after experiencing a transmission slip, I immediately took the vehicle to an ASE Blue Seal Certified shop for a complete transmission service to ensure proper maintenance was performed by qualified technicians. At approximately 80,000 miles, while driving on a highway during a family trip, the transmission suffered a sudden, catastrophic failure, resulting in a total loss of motive power at speed. Losing all propulsion on a high-speed highway created a severe safety hazard for my family and surrounding traffic. The complete mechanical failure was subsequently confirmed by a service center, and the failed transmission assembly remains available for inspection upon request. Initial symptoms were presented directly to an authorized Honda dealer in Annapolis MD while under factory warranty and the vehicle was properly maintained by certified technicians thereafter, but the underlying defect went unaddressed until complete failure occurred on the road.

My vehicle is around 40,000 miles. The rear shocks have rusted, leaked, and collapsed. This seems to be due to a manufacturing defect. It makes driving long distances on uneven terrain very unsafe. A vehicle’s suspension should not fail this completely in such a short amount of time.

The electrical component is failing. I have brought in this vehicle 2 times for failed electrical issues over several years (around 2 years), and it only has 52,000 miles currently. The back up sensors no longer work, as well as warning lights for the following began to appear a year ago: collision mitigation, adaptive cruise control, lane keeping assist, road departure mitigation. Sometimes, our electrical side door will not close and will keep opening when trying to shut. This electrical issues has been draining our battery, and while driving, my car shut off in the middle of the street while I was waiting for a light. After a few attempts, I got my car started again. We replaced the battery, and a few days later the car would randomly turn on and I could not use an electrical key to open the car door. Only way to open it is with the physical key. The battery had to be unplugged in order to stop the car for randomly turning on. All electrical systems would turn on in the car, including the alarm and radio, even if the radio had been off when the car was last turned off. The car has been inspected by Honda mechanics, who told me the electrical component is dying, but they don't know why, the car isn't that old, and it has very few miles on it.

My SRS light went on without being in an accident. Around the same time I noticed that there was a recall reported (NHTSA Recall Number 26V-227) which states that the vehicle may deploy airbags without cause by going over a pothole etc. I took my vehicle in to the Honda dealership and they confirmed that my SRS ECU needs to be replaced bc it detected an airbag deployment but since it already deployed they will not replace it under the recall. However, the recall states that they need to update the SRS ECU to prevent this exact scenario from happening without proper cause and if it already happened the SRS ECU will need to be replaced. The worst part is, that they cleared the recall through the database for my vehicle but did not actually fix any issues and the SRS system is still not functioning. I already made a complaint with Honda corporation over two months ago but so far they have not provided a resolution.

There was no collision or damage of any sort that led to this malfunction. The following systems no longer work: Adaptive Cruise control Collision mitigation system Lane keeping Assist Road departure mitigation system Blind spot info system Tire pressure monitor system Transmission system All of these failures started at exactly the same time. This severely compromises the ability to use this vehicle safely as these systems are all designed to prevent various safety hazards.

When vehicle is completely stopped at a traffic light there have been numerous occasions in which all electrical components fail and the vehicle turns off and fails to restart. This only occurs when the auto idle start is engaged. A few times I’ve had to completely jump the vehicle. I have replaced the battery and also had the car examined at the dealership. They stated they had previously updated the system, had heard of other instances of the same issue but since it wasn’t under recall and they couldn’t replicate the issue they couldn’t do anything. This has proved to be extremely dangerous because at rest, without being able to restart when the light turns green my family and I are at risk of being severely rear ended at high speeds.

The vehicle is a 2020 Honda Odyssey. A recurring and dangerous safety failure occurs involving the Auto Idle-Stop system. With the system enabled and the vehicle at a complete stop, the engine shuts off as designed. However, upon depressing the brake pedal to resume driving, the engine severely struggles to restart and completely fails. Simultaneously, the vehicle immediately and automatically shifts itself into PARK, and the entire dashboard display illuminates with all available warning icons and messages. This malfunction creates an immediate, high-risk safety hazard. When the incident occurs, I am trapped, locked in PARK, in live traffic lanes, sometimes at busy intersections or during left-hand turns. It is impossible to shift back into drive or quickly restart the engine. I am forced to remain stationary in traffic for multiple minutes until the system allows me to cycle the ignition completely OFF and then back ON to attempt a full restart and shift back into Drive. This leaves my vehicle, my passengers, and other drivers vulnerable to rear-end collisions.

2020 Honda Odyssey EX-L, 49k miles, no prior collisions. 1. Faulty system: SRS airbag module, defective factory aluminum clip (official Honda recall defect). EDR data verified this component failure by dealer technicians. 2. Safety risk: Driver airbag spontaneously deployed at 41MPH with zero impact. Seven occupants suffered soft tissue injuries and PTSD; sudden deployment created severe risk of secondary crash. 3. Dealer confirmation: Bay Ridge Honda (Brooklyn NY) diagnosed and confirmed the airbag clip defect via EDR readout and physical teardown, filed service records for this issue. 4. Inspectors reviewed vehicle: Honda dealer techs, EDR forensic specialists, and GEICO adjuster all inspected the car and confirmed no pre-crash damage. 5. Pre-fault warnings: No SRS warning lights, error codes or abnormal electrical symptoms appeared before the airbag deployed unexpectedly.

I am the original owner of a 2020 Honda Odyssey Elite. On July 7, 2026, at approximately 76,127 miles, five safety-system warning messages suddenly appeared simultaneously upon startup: Adaptive Cruise Control Problem, Auto High Beam Problem, Road Departure Mitigation System Problem, Collision Mitigation System Problem, and Lane Keeping Assist System Problem. Restarting the vehicle several times did not clear the warnings. After the vehicle sat for approximately 30 minutes, the warnings disappeared. On July 8, after driving approximately 400 miles home, I parked the vehicle outside in approximately 110-degree heat. About two hours later, upon startup, the same five warnings reappeared simultaneously, along with the Brake System warning light. An authorized Honda dealer confirmed the multiple warnings and performed a DTC scan. Code U3000-49 was stored, indicating an internal failure of the multipurpose camera located behind the rearview mirror. Further testing confirmed that camera replacement was required. The dealer replaced and recalibrated the multipurpose camera at 76,567 miles. The repair cost was $1,631.80 for parts and $717.50 for labor, totaling $2,349.30. I am concerned that failure of a single internal camera component simultaneously disabled multiple driver-assistance and collision-avoidance safety systems. I believe NHTSA should investigate whether multipurpose camera failures associated with DTC U3000-49 represent a recurring safety defect in 2020 Honda Odyssey v

The Honda dealership quoted me $1850 to fix it and they said it was a well-known error. There were NO issues with my car prior to this problem. I keep up with all routine maintenance, oil changes, timing belt and water pump replacements, etc. Not only are these safety features inoperable, but these warning lights are very distracting to the driver, but worse than that- I would be unaware of other system failures since I have so many warnings displayed.

They told me to take to dealership and they need to do a system update and should fix no additional charge. I took it to the Honda dealership service department on 7/6/26 and was told it is the multipurpose camera has an internal failure and it would cost me $2,443.00 to repair it, plus I had to pay them a diagnostic fee. I purchased my 2020 Honda Odyssey Brand new in California. On 6/27/2026 upon starting the vehicle up in the morning there was a myriad of warning messages that came up on the cluster for the first time out of the blue. The rear coil spring (driver side) on my 2020 Honda Odyssey failed and broke while I was driving approximately 45 mph on a city street on XXX in [XXX]. The component that failed is the rear suspension coil spring. I do not have the broken spring in my possession, as it was replaced by the dealer. Due to the coil spring failure, the rear shock absorber on the driver side was also damaged and requires replacement. This sudden failure caused the rear of the vehicle to drop/sag on the driver side, which affected vehicle stability and handling. It put my safety and the safety of other road users at risk due to a loud noise and an abrupt change in ride height. This made the vehicle pull and feel unstable. I was able to maintain control and safely pull over, but the incident was alarming and could have led to loss of control or a crash under different conditions (e.g., higher speed or heavier traffic). A few days prior to the failure (approximately XXX), I noticed a weird/unusual sound from the rear of the vehicle. At the time, I did not associate it with a suspension issue, and there were no warning lamps, messages, or other clear symptoms. The problem has not been reproduced. The vehicle and component were inspected by a Honda dealer on XXX. The dealer confirmed that the driver-side rear coil spring had broken near the bottom and replaced it. They also noted that the rear shock absorber was damaged as a result and needs to be replaced. No other inspections (manufacturer representa

The auto Start/Stop feature doesn't work as needed. It will randomly lurch, turn the car off make me put it in park and then have me restart it. Sometimes the car does not start right away and I am stuck on the road until it decided when and if it will restart. Myself and my family of 5 are not stuck in the road scared of getting smashed into by other drivers unaware that we are stopped or anticipating for us not to be stopped in the middle of the road. I really dont want my family to go to the hospital or die because of this nonsense. Sometimes it doesn't fully shut off but will give error warnings like collision mitigation assistance even though there is nothing for it to mitigate. There are no cars around me and it shouldn't have done this. This has been happening for some time and only happens haphazardly meaning I can't always have the dealer identify or diagnose the issue. Then when I brought it to the dealer, they told me it was because of a power liftgate module error. Which was not true and didn't solve the issue.

I own a 2020 Honda Odyssey with approximately 73,000 miles that has experienced recurring infotainment and electrical system failures. The vehicle has undergone two prior repairs related to the Fakra connector issue, but the problem continues to occur. During the failure, the infotainment screen becomes inoperable, audio functions are affected, and the rearview backup camera becomes unavailable or unusable. The issue appears to be intermittent and is often triggered during or after rain. In addition to the infotainment failures, I have experienced key fob recognition issues requiring the key to be placed near the start button to start the vehicle. There is also static through the speakers, particularly when turning the steering wheel. The vehicle has been taken to a Honda dealership multiple times for this concern, and dealership personnel have witnessed the malfunction. Despite two previous repair attempts, the issue persists. Because the failure affects the operation of the rearview camera system, which is a critical safety feature, I am concerned about the safety of the vehicle and whether the prior repairs adequately addressed the underlying defect. I am submitting this complaint because numerous other Honda owners have reported similar recurring failures involving the infotainment system, Fakra connector repairs, backup camera operation, and related electrical concerns. I am filing a complaint regarding recurring electrical and infotainment failures involving my 2020 Honda Odyssey EX-L. The vehicle experiences loud static and speaker distortion, infotainment system instability, static noises when turning the steering wheel, and intermittent key fob recognition issues requiring the key to sometimes be held near the start button. The issue appears to happen more often during or after rain or moisture exposure. The dealership has already repaired/replaced the FAKRA connector or related infotainment connector multiple times, yet the issue continues returning. The dealership recently witnessed the malfunction themselves and acknowledged the problem is still present. I am currently waiting for further direction from Honda technical support. This appears to be a widespread issue. There are many Reddit discussions and Honda owner Facebook groups where owners describe identical symptoms and repeated failed repairs. Some owners report having the same FAKRA connector replaced as many as six times with the issue still returning afterward. The repeated failure of the same repair raises concerns that the current remedy is ineffective and that there may be a larger electrical, wiring, grounding, or moisture intrusion issue involved. Our vehicle has stalled at stop lights several times over the past 6 years. The auto stop/start feature seems to fail. The vehicle will shut off but when you repeat the brake it will not restart and the vehicle will die completely. You have to put it in park, wait a few seconds and try and restart. Additionally it has completely died at gas station after turning it off to refuel. When you go to start the vehicle it will be completely dead, zero power. We had to have it jump started. The battery has been replaced twice and was testing normal at the dealership. Not a battery issue. Honda acts like this is unheard of but our own research indicates it is a common well known problem with new Odyssey models.

NHTSA Complaint Narrative – 2020 Honda Odyssey VIN: [XXX] Beginning in 2021, the vehicle experienced repeated starter and idle stop system failures, including: •delayed starting •failure to restart after idle stop •multiple warning lights •intermittent no start conditions •repeated battery failures The earliest documented complaint (Culver City Honda, March 25, 2021) states: “When using the auto start/stop feature the vehicle will have multiple warning lights. Sometimes will struggle to turn back on.” These symptoms continued for several years despite multiple dealer visits, battery replacements, electrical diagnostics, and software updates. In late 2025, the starter failed completely and required replacement at a cost of $1,890.25. The failure of the idle stop restart system poses a safety risk, as the vehicle may fail to restart in traffic after stopping at an intersection. Honda has refused to provide any written response regarding the issue. I am submitting this complaint so NHTSA is aware of the repeated failures and the potential safety implications.
