My 2013 Ford F-150 (VIN: [XXX]) suffered a catastrophic rear-wheel lockup while traveling at highway speeds, mimicking a sudden, unprompted hard braking event. The vehicle's speedometer dropped to zero and the dashboard went blank. A certified Ford dealership has just scanned the vehicle and pulled Diagnostic Trouble Code (DTC) P0722, confirming an Output Shaft Speed sensor/molded leadframe failure under Ford TSB 16-0008. Federal records confirm there are 0 unrepaired recalls on this VIN because Safety Recall 19S07 (NHTSA Campaign 19V-075) was completed two months ago. The software-only recall remedy completely failed to prevent this life-threatening transmission lockup. Ford Corporate (Case #[XXX]) and the dealership are refusing to replace the defective hardware due to mileage, despite the fact that a federally mandated safety recall remedy failed to secure the vehicle. "Safety Recall 19S07 was completed by an authorized Ford dealer two months ago, but following today's highway lockup incident, a different, separate Ford dealership officially scanned the vehicle and retrieved Diagnostic Trouble Code (DTC) P0722, confirming a complete recall remedy failure." Truck is experiencing sudden downshifting with loss of acceleration, and the speed odometer & MPG will both drop to zero. Recall #19S07 was issued in 2019 for Ford Motor Co. for this reason, but NHTSA site is showing that my truck is not covered under the recall (based on vehicle VIN), even though it is now experiencing the issue described. This is a major safety issue. Issue began occurring approx. six months ago, and happens about once weekly.
Traveling down the road and truck had a sudden down shift.
I was driving into town where the speed limit dropped from 60 to 30 so I tapped my brakes and the transmission slammed into low gear and the rpm's went up. Almost sounds like recall 19V075 for other ford vehicles.
The issue involves a loss of signal between the transmission range sensor and the powertrain control module, which can cause the transmission to unexpectedly downshift into second gear, potentially leading to a loss of vehicle control. I purchased this vehicle in Feb 2025 used 2013 Ford F 150 6 Gear and this happened to me. I took it to the dealership, and they said the recall had expired and charged me over $1,300.00 to repair. This vehicle was always dealer serviced with Herb Chambers of Massachusetts and they failed to inform the previous owner of the recall. This Ford F150 2013 STX should be covered under this safety issue and they did not honor.
The contact owns a 2013 Ford F-150. The contact stated while driving at an undisclosed speed, the transmission independently downshifted. The power train warning light illuminated. The contact stated that when the power train warning light illuminated, the fuel gauge, the odometer and the speed limit indicators were disabled. In addition, the contact stated that when the transmission shifted to fifth gear, it did not downshift. The vehicle was taken to the dealer where it was diagnosed with a failed OSS sensor and a failing lead frame. The contact was informed that the OSS sensor and the lead frame needed to be replaced. The vehicle was not repaired. The contact referenced an unknown recall; however, the VIN was not associated with a recall related to the failure. The manufacturer was not made aware of the failure. The failure mileage was approximately 185,000.
Internal Speed Sensor Failure for 2013 Ford F150 6R80 transmission. Vehicle stopped in traffic would not move forward in drive. Could be expansion of recent recall of this transmission from Ford with similar issues. Vehicle shows code for Output Speed Sensor circuit P0720
Contacted ford about recalls 26S28 and 19V075 and they report that my vehicle is out of the warranty period for repair. They were advised that this is under a NHTSA recall and still refuse to honor the NHTSA safety recall stating that it is outside the window for replacement. Again advised them that this is a NHTSA recall and they still refuse to honor it. My vehicle is inoperable due to this.
The contact owns a 2013 Ford F-150. The contact stated that while driving at various speeds the transmission had erroneously downshifted causing the vehicle to abruptly stop in the middle of the road. During the failure the wrench icon had displayed. The vehicle was later towed to the local dealer who diagnosed that the transmission valve body needed to be replaced. The vehicle was not yet repaired. The manufacturer was notified of the failure but no assistance was offered. The failure mileage was 120,000.
While slowing down at normal speeds, my 2013 Ford F‑150 will suddenly and violently downshift into a much lower gear (appears to be 1st gear). When this happens, the rear wheels lock up briefly, causing the truck to skid and jerk forward. This creates a serious loss of control and a risk of collision. The issue is intermittent but recurring. It has happened multiple times under normal driving conditions. No warning lights remain on afterward, but traction control sometimes flashes during the event. I reported this to a Ford dealer, but they refused to repair it. This appears to match known issues with the 6R80 transmission and leadframe failures on similar F‑150 models. This is a major safety hazard. If this occurs in traffic or on wet roads, it could easily cause a crash. I am requesting that NHTSA investigate and require Ford to address this defect.
Truck unexpectedly downshifts into first gear and refuses to leave first. Wrench icon appears, speedometer and odometer stop working, truck enters "limp mode". OEM reading DTCs P0720, P0722.
Transmission is locking itself into either 3rd or 5th gear and will not shift. sometimes it will lock itself in 1st gear and not shift.
Transmission down shifting when driving 50 mph or faster
While driving my 2013 F-150 the vehicle suddenly downshifts into 1st gear while driving in the city and remains there. At faster speeds the truck will downshift to 5th gear and remain there regardless of deceleration. This has been accompanied by a check engine light. The truck had the code P-0722. When this happens it is extremely unsafe and unpredictable. This causes an immediate and abrupt reduction in speed and a near-loss of vehicle control. This defect poses an unreasonable risk of a high-speed accident, especially when towing or on wet surfaces. A diagnostic scan at a Ford dealership confirmed the failure of the molded lead frame. The vehicle exhibited diagnostic trouble code P0722 which is the specific indicators for the Output Shaft Speed (OSS) sensor and lead frame failure. These are the exact symptoms and codes identified in NHTSA Recall 26V237. Ford recently issued Recall 26S28 (NHTSA 26V237) on April 14, 2026, for this exact safety defect in later-model F-150s with the same 6R80 transmission. My 2013 vehicle is suffering from the identical root cause—degradation of electrical connections in the lead frame—yet Ford is currently refusing to provide a safety remedy at no cost, citing the age of the vehicle. This is a recurring safety-critical defect that mirrors the current 1.4-million-vehicle recall population. Rear camera is intermittent. Many times as im utilizing it the camera will fail leaving me a blank screen or distorted. This is a safety issue because it occurs during a vehicle movement. A warning sign appears after the fact to have it serviced by a dealer.
The vehicle shifts down, driving at 70K, and it throws into 1st gear. Very scary and dangerous, I was lucky no one was behind me. Left some nice tire marks on the road.
The contact owns a 2013 Ford F-150. The contact stated that while driving at an undisclosed speed, the transmission was slipping and hesitated to shift into gear. The vehicle was taken to the dealer, who repaired the vehicle under NHTSA Campaign Number: 19V075000 (Power Train); however, the failure persisted after the recall repair. The contact stated that while driving at 30 MPH, the vehicle experienced a loss of drive power. The speedometer went to zero without the vehicle decelerating. The transmission warning light was illuminated. The contact pulled over and turned off the vehicle. The vehicle was restarted, and the transmission warning light had disappeared. The vehicle was taken back to the dealer, who determined that the molded lead frame needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was 167,401.
Diagnostic trouble code (DTC) P0720 stands for “Output Speed Sensor Circuit Malfunction.” It is stored when the ECM/PCM/TCM perceives a problem with the output speed sensor or its circuit. If, for example, the engine speed and input shaft speed are at a certain RPM but the output shaft speed is erratic or not what the computer expects, P0720 will be stored. There may also be a P0500 code stored on some platforms. T
On April 28, 2026 while driving at highway speeds the vehicle shifted into first gear nearly causing an accident. We have contacted the dealership and Ford Motor Company. Both are indicating a repair needs to be done in excess of $1300. Both the dealership and Ford are confirming this is the responsibility of the vehicle owner. There is a known defect related to the safe operating condition of this vehicle. A recall should happen and all repairs should be covered by Ford motor company. For this malfunction to happen can cause an accident harming people in the vehicle as well as other drivers on the road.
The contact owns a 2013 Ford F-150. The contact stated that while driving at an undisclosed speed, the rear wheels seized, and the vehicle went into LIMP Mode. The dealer was made aware of the failure, and a mobile technician performed the recall repair under NHTSA Campaign Number: 19V075000 (Power Train) at the residence; however, the failure persisted. The contact stated that while driving at an undisclosed speed, the RPMs were high, and the rear wheels seized briefly. The transmission warning light was illuminated. The contact retrieved DTC: P0720 (Output Speed Sensor Circuit Malfunction). The dealer was notified of the recurring failure but provided no further assistance. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 135,814.
I was driving along, truck was in about 5 gear and then suddenly dropped into 1st gear. And sometimes you start out and truck will not change from 1st gear to second. You have to stop and turn off motor to get it to drive correctly!
The brakes aren’t working properly due to loss of vacuum. This happens anytime I’m stopping at a red light or parking which can result in me hitting someone or another vehicle. This has been confirmed by Firestone Compete Auto Care but they were unable to fix it. Ford says this TSB applies to the same year but not to the plant this car was manufactured. There were no warning messages.
My 2013 Ford F-150 5.0 pulled any evil on me. On 4-23-2026 driving home from work at approximately 3:55 pm with speed of approximately 40 to 45 mph a very hard hit of something as if I had been struck from the rear and immediately at the same time both of the rear wheels locked up and where skidding on the roadway. This lasted for maybe just under 30 seconds. Fortunately, no one was behind me. The truck did make that hard bump sound again but nothing else occurred. With less than 3 miles from home I was able to drive slowly and get home. I am afraid to drive it anywhere now!
The transmission software update only made it worse. The transmission is deciding to shift into 5th gear wrench comes on the no speedometer no information in the information center airbag light comes on radio gets quieter or I have experienced the first shift sudden downshift at 65 mph back tires are hopping on the tar and now everyday it's stuck in first gear for a while I have to shut off the truck and it might reset or I have to turn it off for more than 5 minutes and then it might reset or go in third to start with no speedometer no information and the information center airbag light on this is a safety hazard and it should be an open free recall to replace the bad components inside the transmission from 2013 on up before somebody dies because of a sudden downshift
Truck downshifted to first gear from about 35 mph with no warning, and would not shift back, until it was shut off. I was in middle of road. No warning lights came on prior to this happening. The dealer confirmed a recall for this problem but my truck is a 2013 and recall currently is for 2014 forward. I never received a recall notice on my truck and the estimate to have it fixed is about $1700. Dealer said recall on my truck expired, in 2023. I am not safe driving this vehicle. I have had this truck since October of 2018, and bought it at Toothman Ford Grafton, WV. I never received a recall.
The contact owns a 2013 Ford F-150. The contact stated that while driving at 35 MPH, the vehicle automatically downshifted to first gear and remained stuck in first gear. The vehicle was pulled over on a side street and waited for 30 minutes before the transmission returned to normal operation. The failure reoccurred and became progressively worse. Additionally, the contact stated that while driving at highway speeds, the speedometer indicated 0 MPH, and the vehicle went into LIMP Mode. The vehicle was turned off, restarted, and returned to normal functionality. The dealer was not notified of the failure. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 80,000.
Car would jerk and transmission was failing
Transmission downshift from 6 to 1 at high speed without warning. Loosing control of the vehicle at highway speed that easily could result in a crash with fatalities.... No inspections performed, manufacturer refused to fix/replace the defective parts due age and mileage.... not fair for a manufacturer defect that put a high accident risk that could result fatal to the driver or others. Unknown about warning; first time happened about a year ago.
Transmission upshifts randomly when driving into 1st or 2nd gear. A Wrench icon comes on the dash indicating there is an issue that made it go into " limp mode". When using a diagnostic tool it throws a P0720 code for the output shaft speed sensor. Was checking to see if there has been any recall on these models too for the same issue?
We recently started having issues with the transmission locking into a gear and not changing or downshifting. We received the Code: P0721 which refers to the transmission control module or output shaft sensor is having an issue.
Faulty Molded Leadframe To my knowledge I was never notified of the known safety issues with this part and its recall/warranty (Customer Satisfaction Program 19N01). This is still an ongoing issue and just had to have my vehicle in for service that could of been avoided.
Bought that as used at low milage with good service history indicated with pcm twice made me believe to trust as reliability and regular check ups. carfax is not really honest, as well as I learned that powertrain itself has nothing to mechanism but actually programing meaning actual PCM itself has troubleshoots and not at its stability. It caused no speed on dash board sometimes even though got served but aftermath it gets more worse. I experienced collapsed in middle of high speed traffic to full stop. But The odd that MPG got great better than only during trouble times like first time ever seen over 15 MPG that reached between 16.4 to 18.7 for over a week. Now backward to 14 MPG. On the trouble occurances were R080 downshift to first gear notch caused by faulty outpost shift speed loss sensor. Also code of 19s 07 needs reprogram the powertrain control module has been common and I don't believe wiring is the issue thus the sensor is but reprogram itself is factor although it caused various issue like noise installing from park notch to drive notch, cannot increase or stay constant stead on speed, sometimes caused vibration that had no equality the pcm does not make correct specific of the fault. I checked with auto zone to do diagnosis code on mine was much unspecfic and inaccurate fact in which was not fact thus the manufacturer fails to program the correct troubleshoots. Pcm is very bad. I had to stay under 65 mph either stay at 60 mph sometimes and if i increase the truc

Transmission shifts down to 2nd while traveling without warning. It was taken to local dealership but nothing found wrong at that time. It is available for inspection. The down shift was immediate and severe without warning, which was startling. We were traveling at regular speed when this occurred. It has been reported and was told there was no recalls, however, I now see recalls for F150s beginning year 2014. My truck is a 2013 but the recall for the 2014s is exactly what occurred with my 2013 F150.
I have reported the same exact issue that Ford recalled April 14,2026 with my vehicle. I almost had an accident by it changing from automatic to the gears & it comes to a sudden stop when it do it. I guess someone or me gonna have to lose a life before some look at my issue. It showing a recall was on my truck in 2019 Before I brought the truck. The recall HAS NOT BEEN DONE ON MY TRUCK. PLEASE HELP B4 someone get hurt. Thank you
2013 F 150 5.0 TRANSMISSION LEAD MODULE LOOSING DATA TO COMPUTER AND TRANS GOES TO 3RD GEAR WHILE DRIVING AT HIGHWAY SPEED THIS HAPPENED MULTIPLE TIMES IN THE LAST FEW DAYS the 6r80 transmission and has code P0720 AND IS GOING IN FOR REPAIRS 4 21 2026 AS IM AFRAID TO DRIVE IT .IT SEEMS TO BE A COMMON PROBLEM WHEN TALKING TO THE TRANSMISSION SHOP AND COULD COST $900 OR MORE IF THE TRANS IS NO HARMED BY THIS SUDDEN MALFUNCTION

Subject: Safety Complaint – Transmission Lead Frame Failure (2013 Ford F-150) I am submitting a safety complaint regarding my 2013 Ford F-150 (VIN: [XXX] ), which has experienced a confirmed failure of the transmission lead frame connector (output speed sensor), a known defect previously acknowledged by Ford Motor Company. This vehicle has only 28,000 original miles and has been properly maintained. Despite the extremely low mileage, the transmission began exhibiting unsafe and unpredictable behavior, including harsh shifting and the risk of sudden downshifting while driving. A Ford dealership has diagnosed the issue as a failure of the transmission lead frame connector. This component is critical to proper transmission operation, and its failure directly impacts the vehicle’s ability to shift safely and maintain control. Ford Motor Company previously issued a Customer Satisfaction Program extending warranty coverage for this specific defect. However, that coverage was limited by time rather than mileage or actual condition of the vehicle. Although my vehicle is outside the time-based coverage window, the extremely low mileage clearly indicates that this is not a normal wear-and-tear issue. Instead, it represents a premature failure of a known defective component. This defect presents a significant safety concern. The failure can cause: - Sudden and unexpected downshifting into lower gears - Loss of speed or erratic vehicle response - Increased risk of rear-end collisions or
The contact owns a 2013 Ford F-150. The contact stated that upon shifting to Reverse(R), the vehicle hesitated to respond, made an abnormal loud sound as if it had been rear-ended, and the vehicle shifted into gear violently, causing the driver to sustain neck pain. Additionally, the contact stated that while driving and depressing the brake pedal to come to a stop, the vehicle unintendedly downshifted, and upon depressing the accelerator pedal to resume driving, the vehicle delayed responding. The contact stated that the failure had been recurring intermittently. The power train warning light was illuminated intermittently. The vehicle was taken to the dealer to be repaired under NHTSA Campaign Number: 19V075000(Power Train). The dealer diagnosed the vehicle and informed the contact that the transmission lead frame had failed and needed to be replaced prior to the recall repair. The contact declined to pay for the repair. The vehicle was not repaired due to the cost. The manufacturer was not contacted. The failure mileage was approximately 113,000.

The contact owns a 2013 Ford F-150. The contact stated that the transmission failed to shift into gear and the vehicle was pulled over to the side of the road, turned off, and restarted after a while, and the transmission shifted properly. The contact used a scanner to reset the system, and the transmission shifted properly while driving approximately 50 - 60 MPH. The speedometer rose to 100 MPH, then 0 MPH unexpectedly, with the power train warning light illuminated. The vehicle was taken to the dealer and was diagnosed with transmission failure. The dealer completed a system update. The failure persisted and became progressively worse. The vehicle was returned to the dealer, who determined that the leadframe was defective, causing the transmission failure. The dealer informed the contact that the repair was not covered under warranty because the vehicle had exceeded 6,000 miles. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 75,000.
The contact owns a 2013 Ford F-150. The contact stated that while driving at an undisclosed speed on the highway, the vehicle became disabled. A diagnostic scan tool indicated a cam phaser failure. The vehicle was able to be restarted; however, the engine was loud and was shaking. The vehicle was taken to an independent mechanic and diagnosed with timing chain failure. The vehicle was not repaired. The manufacturer was notified of the failure. The approximate failure mileage was 143,100.
While driving, the transmission deaccelerated into first gear while going 65 mph.
My front windshield is leaking from the top of the window. I can see little trails of dried drips on the inside of my windshield. I feel it could possibly be a safety issue of being loose. It has not been confirmed by a dealer, service center, police, insurance company, or anyone yet. Just found it about a month ago. Trying to see if a recall before I contact a glass company to repair.
I was driving down the road when my power steering went out in the middle of a sharp turn. As the power steering was going out the screen on the dash showed “Service power steering now” and beeped three times, giving me only about a second to read the message and react to losing power steering. The steering wheel jerked right and I almost drove off the road. I have since taken it to a ford dealership where they informed me that the entire rack and pinion needs to be replaced.
The contact owns a 2013 Ford F-150. The contact stated that while driving approximately 40 MPH, the transmission shifted improperly and became stuck in third gear. The vehicle went into “Service Mode” and the powertrain warning light illuminated. The contact stated that the speedometer became inoperable and remained at zero. The vehicle was pulled over to the side of the road and remained stuck in third gear. The vehicle was then shifted into park(P), and there was a loud sound coming from the engine compartment. The vehicle was turned off and restarted, and the failures ceased. The vehicle was then taken to the residence. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was 120,000.

NHTSA publishes every complaint as the owner wrote it and investigates only a fraction. Spelling, capitalisation and judgement are the filer’s own. What carries weight is repetition: the same component failing the same way across many filings, and whether the manufacturer eventually opened a recall for it.
