This recall has been open for 7 months and a remedy has still not been issued - this doesn’t seem reasonable for a safety concern. I have also several times experienced a failure in the “extra” 8th passenger seat (middle row middle seat) sensor - it alarms that the seat belt isn’t buckled even when that seat is not in use and not even installed at the time.
2021 Toyota Redundant Sienna complaints
Owner reports filed with NHTSA about this vehicle, newest first and reproduced as filed. They are unverified accounts: one proves nothing, and a pattern across many is the thing worth reading. Back to the full 2021 record →
When applying light brake force to slow to stop for traffic light/sign the vehicle will feel like the brakes have released and stop slowing (feels like vehicle surges forward) requiring to either really apply much greater force to pedal or to release and reapply force to brake pedal to continue to slow the vehicle. Safety concern as without warning and not consistently happening additional 20 to 50 foot braking distance added to vehicle stop. Occurrence is random and happens approximately one in ten stops. The main consistency is a lower speed and gentle type stop: start braking speed generally in the 25-35mph range, brake release feel at 10-15mph range with light to medium braking force being applied. Seems to happen most often with a light downhill gradient. Occurs rendomly and cannot be reproduced on demand for service tech. Has happened since vehicle was bought new and still occurs randomly 26000 odo.
The bottom/floor of the center console between the 2 front seats getting warm or hot everytime I drive the car for a while
When breaking, mostly during downhill, the brakes feel like they simply “let go” for an instant. Most times this is very short and It feels like a typical downshift during breaking in an ICE car. However, last time it really gave me a lurch and I almost ended up falling off a cliff face. Many reports all over the internet but nothing from Toyota.
The brakes release for a period of time when stopping. This has been ongoing since owning the vehicle. It feels like a lurch forward when my foot is actively braking.
Intermittently, when braking to slow down towards a stop, the brakes let loose, and the vehicle surges forward for a very short spurt (like less than 1 second). While continuing to apply brake pedal pressure, the brakes, then, continue to work. This is VERY unnerving, and we are afraid that, if there were to be a pedestrian in a crosswalk in front of the vehicle they may be hit as this does cause a slightly greater braking distance overall. I reported this to my Toyota service department relatively soon after I purchased the vehicle (new), but they indicated they know nothing of the issue. I now see many people reporting this in various Internet discussion threads. This is a very serious issue.
Check engine light came on - Taken to dealer, diagnosed following 2 codes : P24517F and P14502A Both of these codes are known problems to Toyota, and as such a Service buliten for the Evap Emission System Switch Valve has been issued per NHTSA. This problem should be a recall, not a known service issue - it is obviously a common problem with Toyota in the Hybrid engines.
Sometimes when each of us 2 are slowing to a stop while applying the brake, the Sienna will lunge forward. This unexpected event may cause a rear-end collision if too close. Also, it is enough to cause us to hit our head on the head rest. Can’t understand why this happening. We are sure we only have our foot on the brake and not the accelerator.
Safety of self and others at risk due to odd breaking. Feels like you have to put pedal to the floor to break! Especially if turning. I found a Toyota service bulletin when googling the issue. This is a major safety issue!
There has been several incidents when I step on the brakes and the vehicle seems to accelerate forward after what feels like an attempt to find the right gear when decelerating. This occurs infrequently but usually when decelerating along with an external factor. I was able to consistently replicate when I’m coming off the highway onto a service road then have to make quick turn at a corner. It feels as if the vehicle is trying to search for a gear and once it finds it there’s a momentarily acceleration forward.
2 issues to report 1) when under medium consistent breaking(slowing down for a light) there are instances where the breaks feel like they let up or slipping. This issue has been reported and with the car since the first year of ownership but Toyota says it’s all within spec 2) second issue is an issue with the rear captions seatbelt and has also been reported since 2022 where upon normal driving the seatbelt locks up and the car has to be stopped and seatbelt unbuckled to release tension. Also brought up multiple times with Toyota and no action. Kids sometime unbuckling while car is driving since they cannot move
When applying the brakes, the brakes sometimes feels as if though it "slips" and the brakes momentarily release causing the vehicle to feel like its suddenly accelerating. This has happened on several occasions -- on downhill and on flat roads. After the brake feels like it slips, then the brake returns back to normal.
When my car is going down a steep hill and I have the brake applied, Suddenly the car will speed up and lurch forward instead of continuing to slow down. This is not how cars normally behave in the 30 years I have been driving. I could have slammed into the car in front of me. This has happened over 100 times. My vehicle has approximately 27,000 miles and I have mentioned it at the Toyota service center and it has been checked out and there are no issues that they see. No warning, lamps or messages appear at all
When I apply brakes sometimes the brake is released and I need to push the paddle hard to engage the brake again. Same thing happens sometimes on adaptive cruise control. It sometimes release the brake and I need to push the paddle to engage the brake.
This incident had happened twice in the last 30 days. I have been stopped, then starting to accelerate after a stop sign, I push on the accelerator and the vehicle slows down, as if it’s losing power for about 5 seconds. I do not release the accelerator and after the 5 seconds of what feels like no power, the van begins to accelerate like normal. This poses a severe safety risk while crossing intersections when you believe your vehicle should respond when pushing on the accelerator. I did not have any notifications pop up on my van, I was in an open intersection on both instances, with no one in front of me. Speed limit was about 5mph, coming off a complete stop. This is extremely dangerous, especially when your vehicle feels like it stalls without any reason. The gas pedal is completely non-responsive for 5 seconds.
In the 2-1/2 years than I have owned and driven this vehicle, I've had no problems. On Saturday, [XXX], driving home from a 80 mile trip, as I deaccelerated to make a left turn onto my street (speed was 7mph) I tapped the break pedal and the vehicle went bonkers! The Sienna veered right at a high rate of speed that the front end cleared the curb; the back end smashed into the curb flattening both rear tires and damaging the metal hubcaps; the vehicle speed tore off the bumper and both rear panels. I was able to steer the vehicle: missing a large 2-foot high tree stump; two other large trees; a 10-foot high solid fence at the yard back. I then drove down the sidewalk to the next intersection and was finally able to slow down. This entire time, my foot was on the break, but the vehicle would not stop. The Sienna is now at Faulkners Collision Center in Mechanicsburg with virtually no front end damage but colossal rear damage. The vehicle had 5-full maintenance checks since purchase. I was not told of any suspected problems and never received a vehicle recall. I could have been killed or may have killed others as there is a playground on the corner of this block. I BELIEVE THE COMPUTER CAUSED THE PROBLEM. THE CAR IS AVAILABLE FOR INSPECTION AS PERSONNEL AT GETTEL TOYOTA IN PUNTA GORDA CLAIMED THEY COULDN'T TOUCH THE COMPUTER AND HAVE DONE NOTHING. Purchased Sienna in August 2021. In October at about 5:30 a.m. we were in the far left lane on a PA interstate, traffic came to a stop for construction. A delivery truck in front of us rolled backward a bit. When traffic started up again, our car would not go forward, only reverse. We tried "drive"--nothing--only had reverse. Finally, with cars whizzing around us, shut the engine off. Upon restarting, all gears were restored and we continued on to Boston. We were shaken by this event. Since then, we have had numerous incidents of the car not moving when I put it (mainly) into reverse. I had to shut the engine off--restart--and it went into gear. Latest incidents--first week February 2024--I pulled into a parking space at Englewood Hospital. After visiting my husband, came out, started the car, put it into reverse--NOTHING. Shut the car off--restarted it, and it was ok. Ten minutes later, I pulled off onto the shoulder to take a call from one of his doctors and--no gears. Shut off the engine, restarted it and it was ok. I'M WORRIED I'LL STOP FOR A TRAFFIC LIGHT ON ONE OF OUR 4 OR 6 LANE STREETS AND GET HIT BECAUSE OUR CAR WON'T GO INTO GEAR.
Occasionally when easing on the brakes the van will accelerate. The engine will rev up while depressing the brake pedal, the van will start jerking and jumping trying to stop and go at the same time. This has happened on numerous occasions in parking lots at slow speeds and while driving on a busy road.
This is a follow up to NHTSA ID Number: 11542558. We have determined that this issue does not merely involve the brakes releasing, but the vehicle actually accelerates during the incident. Our 2021 Toyota Siena LE AWD has an issue where, while steady pressure is being applied to the brake pedal, the brakes will randomly release and the car will actually accelerate briefly. While attempting to slow or stop the vehicle with steady pressure on the brake pedal, the braking action will momentarily give out, resulting in the vehicle lurching forward beyond the anticipated stopping/slowing distance before the braking action re-engages. In many, if not all occurrences, the vehicle actually accelerates. For example, if the incident occurs while driving up a hill, not only will the rate deacceleration decrease, but the vehicle will actually accelerate up the hill briefly before the braking re-engages. This issue occurs on a regular, but seemingly random basis, multiple times per week. This issue occurs under all driving conditions (hot or cold, dry or wet, smooth pavement or rough surfaces). It occurs at all driving speeds, including under 25mph and over 55mph. This is a dangerous condition that could cause serious injury or death. This could easily result in a collision with cars, bikes, or pedestrians as the vehicle suddenly stops slowing without warning and unexpectedly accelerates. It could also result in a serious collision from cross-traffic if the issue caused the Sienna's stopp
About 25% of the time when I come to a stop and I am breaking, the car surges forward slightly. At first, I thought it was the way I was driving or the fact that it is an EV vehicle, which I had not driven before, but overtime it seems it is just the way the car operates. Others in the car have felt it happen as well as I have come to a stop sign or stoplight. I have not bumped into another car or anything but feel that that could happen if this is not corrected.
I have this car for 1 year, and I’ve have problems when I am driving and I have use the brake the car stop and keeping going forward ,I already had a accident hitting a car in front me,I’ve see a lot complain about this problems and the Toyota is not getting fixed and keeping saying that is the driver fault,I work as a driver I need this problem get fix.
The contact owned a 2021 Toyota Sienna Hybrid. The contact received notification NHTSA Campaign Number: 23V865000 (Air Bags) however, the remedy to repair the recall was not available. The contact stated while driving from a stop light, another vehicle crashed into the driver’s front side door. The contact mentioned the other driver failed to stop at the red light which caused the crash. The air bags in the vehicle failed to deploy. The Supplemental Restraint System (SRS) Air Bag System malfunctioned and the air bag light was illuminated. The contact sustained a neck injury and hit her head. The front passenger sustained back inquires and both required medical attention. The vehicle was towed and deemed destroyed. A police report was filed. The manufacturer was not notified of the failure. The failure mileage was approximately 98,000.
The contact's client owns a 2021 Toyota Sienna. The contact stated that she was unable to place a car seat in either of the outboard third row positions due to the angle of the seat edge. There was more than twenty percent of the car seat hanging off the edge. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was unknown. The VIN was not available.
Recall: OCS Sensors May Short Circuit; 3TB15/23TA15 ; I am a owner of 2021 Sienna Hybrid (VIN [XXX] ) and was getting alert for the airbag and seat belt warning even when plugged in. I took it to a dealer in Sunnyvale California (Stevens Creek Toyota; 4202 Stevens Creek Blvd San Jose, CA 95129; 408-984-8833) and they are not recognizing the recall. I was charged a diagnostics fee of $ 899.93 and am being asked to pay $ 2000 to replace the car seat to fix the issues. Your advise is solicited. Best regards. Thanks -- [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
The contact owns a 2021 Toyota Sienna. The contact stated while driving at approximately 50 MPH, the rear end of the vehicle started to sway to the left and the right. The contact attempted to steer the vehicle however, the steering wheel was very difficult to turn. Additionally, the contact depressed the brake pedal, and the pedal was very loose and depressed to the floorboard. The contact pulled over and had the vehicle towed to a dealer. The contact stated that the dealer was not able to duplicate the failure. The vehicle was not repaired. The manufacturer was informed of the failure. The failure mileage was approximately 73,000.
When you apply a brake, the vehicle initially responds with an impetus to stop, then the brakes seem to disengage for a fraction of time and then engage again. This is all with the foot being on the brake pedal. It is perceived as if the vehicle continues to move by inertia or accelerates when you hit the brake. Occasionally, it feels like a delay in brake response time. The brakes issue is sporadic. It may occur 2-3 times a week or may not come up for a few weeks. As a result of the problem, the deceleration-to-stop distance is greater than you would expect under normal brake functioning. The issue is consistent across the primary and secondary driver of the same vehicle. The brake problem most often occurs when the car is going down the hill or when there is a short distance to stop, such as at a stop sign or red light in an urban area. The other driver reported that the problem occurred when there were bumps on the road. There are no warning lights indicating the problem with the brakes. The problem was reported to the dealer; however, the issue could not be replicated. The dealer advised us to have a Toyota representative perform the inspection as the problem would most likely not be identified because it is sporadic. As a result of the issue, I got in a car accident on a highway hitting another vehicle in the rear, when it made a sudden stop. The brake did not respond when I applied it two times in an attempt to stop, and my vehicle continued to move with my foot depressin
While slowing to gentle stops for a STOP sign or a traffic light, occasionally the brakes will feel as if released and the vehicle stops decelerating in the expected fashion. This could result in a rear end collision with another vehicle if the operator doesn't rapidly apply firmer pressure to the brake pedal immediately. Because the brake pedal is already depressed, the forward collision avoidance system is overridden and will not stop the vehicle in time. This has occurred at random intervals and is quite disconcerting. I reported this to the authorized dealer's service manage, who said they were aware of the issue with the hybrid braking system, but that Toyota did not have a "fix" at this time. While reporting this issue at the dealer, another nearby customer began to relate the same issue with respect to his hybrid Toyota vehicle. The service representative did advise me to notify Toyota USA directly.
While depressing the brakes, it will feel as though the brake "slips" for just a moment, and then re-engages. This causes the car to lurch forward, potentially colliding with a vehicle in front of you. This happens intermittently, and cannot be recreated on demand. It is, however, most noticeable when engaging the brakes downhill. The Toyota service department says that there is nothing wrong with the brakes, whatsoever. Numerous forums online would suggest otherwise. This seems to be an issue that a high number of Sienna hybrid owners are experiencing, and are being told by Toyota that the car's system says that there is nothing wrong. Consumers are being gaslit by Toyota, and are headed for serious safety issues if this is not addressed.
On 12/15/2023 I brought my 2021 Toyota Sienna, as described above, to Waites Toyota Dealership located in Watertown NY, because of a brake issue. Over the past month or so, when braking, we have experienced a jolt of power after decelerating despite keeping the brake pedal being depressed. A representative from Waites Toyota Service Department took the vehicle for a test drive and experienced the same issue and it was decided to take a further look at the brakes. The dealership contact TAS, a department within Toyota, and even though Toyota acknowledge there was an braking issue, Toyota did not have enough complaints concerning this issue, so no remedy was available. The vehicle has been converted so it can carry my daughter who is wheelchair bound, and has been for life. This is the only vehicle we have to transport her and even though we feel that this is a serious safety issue, we continue to use the vehicle as it is her only mode of transportation. The vehicle never displayed any warning lights or other symptoms, but the issue does happen almost every time you come to a stop. We feel Toyota should be forced to make this right. This vehicle cost more then $60,000 with the conversion and it should brake as it was designed to do.
1. The vehicle displayed the Rear Cross Traffic Alert (RCTA) warning light along with the Blind Spot Monitoring (BSM) warning light on the dash board. The vehicle was brought to Toyota of Ann Arbor for an inspection. After inspecting the vehicle, Toyota of Ann Arbor reported that road debris (snow, dirt, mud, etc.) was trapped between the bumper and the sensor resulting in the sensor to malfunction. The vehicle has been taken through a car wash with an underbody wash option, but due to the position of the sensor, the sensor is not able to be cleaned from a car wash. The sensor can only be cleaned by removing the bumper. But when the bumper is removed, the sensors have to be re-calibrated and the re-calibration of the sensors can only be completed by trained professionals. 2. Due to the malfunctioning sensor, the driver's safety is put at risk by not being alerted of obstructions (both objects or persons) behind the vehicle, nor oncoming cross traffic, nor detecting vehicles in blind spots. This could cause collisions or worse, fatality, due to the malfunction sensor not alerting the driver. 3. This vehicle has been able to reproduced the problem multiple times and as mentioned above, the vehicle was inspected and confirmed by the Toyota of Ann Arbor dealership. The dealership (Toyota of Ann Arbor) mentioned that there are multiple reports of this issue on other vehicles of the same make and model. 4. The vehicle or component has not been inspected by the manufacturer, police,
Van surges occasionally when applying breaks at a stop sign or traffic light.
The sliding door on the passenger side opened while I was driving the vehicle. The door started out as closed. This is unsafe because I had a 6 yr old in the middle row of seats. I have not yet contacted the dealer or service center. The vehicle has not been inspected by anyone. There was no warning prior to the failure.
The contact owned a 2021 Toyota Sienna Hybrid. The contact stated that while driving at an undisclosed speed, the check engine warning light illuminated. Soon after the malfunctioning hybrid system "pull over immediately warning message displayed. The contact stated while he was driving the vehicle to a nearby parking lot he noticed smoke coming from under the hood. The contact then parked the vehicle and removed a 4-year-old and a 1-year-old from their car seats. Flames came from the hood area and he grabbed a nearby water hose in an attempt to extinguish the fire and property owners had a fire extinguisher that was also used. The fire department arrived and extinguished the fire. A fire report was filed and there were no injuries. The vehicle was towed to the dealer and an investigation was completed where it was mentioned that the origin of the fire was the hybrid component. The results of the investigation were not released to the contact. The vehicle was declared a total loss. The manufacturer was made aware of the failure. The failure mileage was approximately 23,500.
Pulling into paved parking spot on very slight uphill grade. Tried to apply brake pedal but car kept moving forward about 6ft over curb and crashed into cement wall creating slow speed impact damage to front end. Towed on flat bed to dealers lot. Tow truck driver noted brake seemed loose and had little resistance. Car still under warranty, 28.8k mi, All warranty required maintenance has been performed to date by Dealer. No prior accidents. Annunciation on screen after event saying "Parking Support Brake Malfunction Visit Your Dealer" Message has subsequently remained. Large number of same or similar annunciation messages over last 4 yrs reported on Toyota Nation web site. Messages indicate that TSB-019-19 may apply and/or DTC C13B0. However, Service Manager at Dealer has refused to investigate this and similar diagnostics to explain safety systems failure or otherwise repair under warranty agreement, unless we first sign a release for personal injury liability with Toyota legal department. Dealer has advised it is not safe to drive car in its present condition. Please note similar complaint sent a few days ago on this web site (no response so far) but there was an error in reported VIN so this is a resend
While driving vehicle, warning message come on saying, “Parking brake malfunction. Visit your dealer.” The message stayed on. I delivered the van to the dealer two days later and there it is @ DCH Toyota in Wappingers Falls NY.
Third Row Seats does not stay stowed and while driving over a hump the seats ended up moving away from the potential stowed position and locked itself in the upright position. the noise was so loud that we thought something had banged on the car, but then we realized that the seat had just moved to the upright position. a little bit of research indicated other drivers reporting the same issue without any fix until date. The risk is if the seat decides to move and there is somethin on the seat in stowed position that can become a projectile. Also if the middle seats are reclined at the farthest position, it will cause an impact. Toyota dealership does not have any solutions for this issue and currently the third row seats are held down with a bungee cord.
The contact owns a 2021 Toyota Sienna. The contact stated that while his wife was parking, she depressed the brake pedal however, the vehicle failed to stop. A message displayed on the instrument panel “Parking brake failure see dealer”. As a result, the vehicle crashed into a wall. The air bags failed to deploy. The contact’s wife sustained injury to her neck and shoulder. Medical attention was required. A police report was not filed. The vehicle was towed to the undisclosed dealer’s service center lot. The vehicle had not yet been diagnosed or repaired. The manufacturer was notified of the failure and the contact was advised to contact the NHTSA Hotline. The failure mileage was approximately 28,800. The contact stated that their vehicle continues to display the same message about the parking brake. They are unwilling to drive their vehicle.
Our 2021 Toyota Siena LE AWD has an issue with the brakes randomly releasing briefly when slowing to a stop. While slowing to a stop with steady pressure on the brake pedal, the braking action will momentarily give out, resulting in the vehicle lurching forward beyond the anticipated stopping/slowing distance before the braking action re-engages. This could easily result in a collision with cars, bikes, or pedestrians as the vehicle suddenly stops slowing without warning and unexpectedly increases stopping distance. It could result in a serious collision if the issue caused the Sienna's stopping distance to go past a stop sign or red light into an intersection. The problem is intermittent and difficult to reproduce, but has occurred repeatedly over the 2+ years we've owned the Sienna. Going over any kind of bump, crack, pothole, etc. seems to increase the likelihood of the occurrence, but it also happens on dry, flat pavement. No warning lamps, error messages, or other related symptoms from the vehicle either before or after occurrences of the problem.
When braking, I experience a jolt of power after decelerating, despite keeping their foot on the brake.
My young daughter got the seatbelt caught around her waist. The seatbelt was unable to give any slack and was locked up. It could retract a little bit but that would only squeeze around my daughter’s waist tighter. The seatbelt would not pull forward. We eventually had to cut the seatbelt to release her. It caused red marks around her waist and had we tried to retract it any further could have suffocated her. We took the van to the dealer to replace the seatbelt and they had said they heard of this happening before but it wasn’t a malfunction but just a weird design quirk. This incident has not been investigated.
The breaks give out to a point where you run the risk of running into someone or something. We've visited Toyota 3 times now complaining of this issue yet they just state it's the regenerative breaks and how they operate. Although the breaking never got to this level of safety concern until we reached around 30,000 miles. I filed a complaint with Toyota that resulted in nothing being done. A couple examples would be sitting at a stop light and the breaks release almost causing us to run into the car in front of us. Pulling into daycare trying to stop while pulling into the parking spot and the breaks release. The last one is while going down hills the breaks seem to let go and the vehicle speeds up. It scared my wife so much that we actually sold the vehicle as Toyota deemed it safe. We loved the vehicle but can't risk the safety of our family.
Since the day I bought the van new from a Toyota dealer, the brakes periodically (roughly once a week) will feel as if I've momentarily let off the brake and tapped the accelerator. In other words, the decelerating van suddenly has a quick surge forward or lunge of acceleration. It seems quite random when this happens. I'm usually applying a gentle to medium amount of braking and the van is traveling somewhere in the 5-35 mph range. I've yet to have any close calls, but it is a scary sensation (as if the brakes have momentarily given out) and I have to quickly react by pressing down harder on the brake pedal. Because it occurs quite randomly, I haven't taken the vehicle to the dealer yet as it is highly unlikely I'll be able to replicate the issue for them. I searched online and saw that hundreds of other Sienna 2021-23 drivers are reporting the same issue. To repeat: the problem has not been reproduced or confirmed by a dealer, nor inspected by the manufacturer, police, insurance rep or others. No warning lights come on when this issue happens. I currently have 36,000 miles on the vehicle. The issue has happened almost weekly since I first bought the vehicle.
When the sliding doors are left open for any purpose (cleaning, loading, unloading, sitting in a car, etc.) after approximately 60-minutes the sliding doors are disabled and cannot be closed. The key won't work. The button will not work. The overhead will not work. This poses a threat and danger to driver and passengers in possible and many times probable scenarios. If sitting the car at a park and there is a bee or bug swarm, you would be stung to death by the time you manually closed the doors that will only allow a few inches at a time. If there was a sudden weather change or flash flood. If there was an assailant approaching while you are sitting with your family for a bit with the doors open. Any number of things. I took it to Toyota and they wanted to charge me $150 plus for an assessment, to which I declined because this is their problem, not mine. My request is to have this fixed so the key will at least override the door shut off before someone gets hurt because of this. This has happened twice. I will enter the date it happened on the second time. It is Beechmont Toyota in Anderson Ohio that refused to service me.
The brake sometimes does fade. Hard to explain but a sudden feeling like the car isn't stopping when slowing down for a light or someone crossing or needing to brake. This is potentially very dangerous since today I almost ran a red light because the brake let go briefly. This is a wide known issue and a lot of people are complaining about it, but dealers aren't able to replicate it because it happens and goes. My 2021 Sienna was brand new, and I first had it and thought it was supposed to be like that. The issue disappeared for months before coming back again 4 times this week alone. The car is very well maintained and only taken to dealer. This is an article from a forum of many people also experiencing it: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Periodically, when I press on the brake to slow down for a red light or stop sign or to make a right hand turn, my van, instead of slowing down, surges forward as if it is accelerating. It feels as if my vehicle jumps forward. I am able to stop the vehicle by slamming on my brakes. I believe my safety and the safety of others is at risk each time this occurs as I worry it will happen when I need to stop suddenly for a pedestrian or for another vehicle. The problem has not been reproduced. The issue occurs sporadically. It may be weeks before it reoccurs. I went for a ride along with a service manager but was unable to reproduce the issue. It has not been inspected except at the dealership. No warning lamps appeared. This issue has been occurring since June 21, 2021. I have kept a log since then.
The contact owns a 2021 Toyota Sienna. The contact stated that while his wife was fueling the vehicle, his 5-year-old son pressed the automatic rear driver's side sliding door switch and the door opened, bumped, and scraped the fuel door. The contact's wife immediately stopped fueling the vehicle. The contact stated that the vehicle was designed with an automatic door lock that should engage to prevent the rear driver's side sliding door from opening and bumping into the fuel door while fueling the vehicle; however, the automatic door lock failed to engage. The contact stated that he was concerned that the failure had caused metal fragments intrusion in the fuel tank. The vehicle was taken to the dealer, where the cause for the failure could not be determined. The vehicle was not diagnosed nor repaired. The manufacturer was notified of the failure and referred the contact to the dealer for assistance. The failure mileage was approximately 49,000.
Since we have owned the vehicle (April 2021), we've had three separate incidences where, when pulling out into traffic from a stop, and the accelerator (gas pedal) is pushed, the vehicle begins to move, but then power is immediately lost, leaving you stuck for a moment. Each time has been temporary, and when the pedal is released and depressed again, the vehicle begins moving. However, this sudden loss of acceleration and few second delay in movement of the vehicle now in the path of oncoming traffic can lead to an accident. The most recent occasion happened on July 24, 2023. We've reported the problem to the dealer two times, the most recent today, July 28, 2023, with no resolution.
Braking system disengages briefly when coming to a stop. Have been told its the transition from EV mode to gas but it poses a safety concern. The problem was replicated by Carmax service and they have sent me to toyota dealership.
Went to stop, so I pressed on the brake pedal. as I slowed down, the brakes released and the vehicle surged forward suddenly as if the accelerator pedal had been pressed. i panic stomped the brake pedal and the vehicle started braking again with a sudden hard stop. The pedal had to be lifted and depressed again for the brakes to reactivate. during this short time, the vehicle surged forward and accelerated. No, I was not pressing the accelerator pedal. Yes, the vehicle is available for inspection. The vehicle surged forward suddenly while braking. I was not pressing the accelerator pedal. There was no one in front of me, but had there been, I am unsure if the vehicle would have stopped in time, or if the AEB would have even worked, seeing as I was pressing the pedal and the brakes were not working. toyota has said that nothing is wrong, and that I am feeling the "transition" between regen braking and friction braking, except, the vehicle GAINED speed, as if power was being applied to the electric motors instead of regen resistance drag. The brake pedal no longer worked, and the fake "tachometer" went from in the "CHG" section of the tach to the "neutral" position, despite me pressing the brake pedal. I did not have time to see if it went up, as I had to lift up and stomp on the pedal again to get it to actually stop at the stop sign. I have been unable to reproduce this issue on demand, as I cannot quite figure out the conditions that trigger it, as it seems completely at ran
2021 Toyota Sienna Hybrid. The following has been true from the beginning of ownership of new Sienna. When braking for a turn or stop, as speed drops below about 10-15 MPH, there can be a sudden power/speed surge and lurching forward that lasts for 1-2 seconds. This feels as if there are no brakes at all for those 1-2 seconds Dealer says that that's just the way that hybrids are, that once your speed drops below when the EV system had been regenerating electrical energy. But it is scary because, if for example a child jumped in front of the car at that moment, there may not be adequate braking. The dealer says there is no software fix to smooth the transition from regenerative braking to "normal" braking. WHY ISN'T THERE??
