On July 15, 2024 this vehicle at 50 mph on cruise control suddenly flashed white warning words indicating problems with the cruise control and immediately thereafter started flashing “hybrid malfunction” and to return to service dealer. At the time of the hybrid malfunction warning which was right after the quick cruise control warning, the engine operation, specifically the gas pedal no longer delivered power to vehicle, either electric or gas. Additional warning was to pull off to the side and stop and turn off vehicle. This occurred and vehicle was pulled to the side put into park and turned off. Waited approximately 3 to 5 minutes and restarted the engine which started in gas with no electric although there was electrical charge on the battery. Was able to continue to our final destination approximately 15 minutes later and no more than 10 miles driven. Hybrid malfunction was still visible on the dash to include the warning stating that vehicle needed to go to the service department. Approximately an hour later, vehicle was turned on again with the same warning about the hybrid malfunction, however, engine started in electric. Drove vehicle to dealership and stated what happened. Dealership the next day stated that this was a phantom code known by Mazda, but no problems associated with the code. Vehicle picked up per service advisors request two days later. Mazda dealership stated that headquarters had eyes on this particular vehicle. A day later after the pick up Mazda d

4 weeks ago my wife was driving on a busy highway and lost propulsion. Hybrid System malfunction message. She was able to pull over before causing an accident and put the car in gas only mode. She never shut the car off and drove on to her destination. I met her and picked up the car and took it to the dealer. Car is new and has 5000 miles on it. Car has been at dealer for 4 weeks with no answers and several unanswered phone calls to customer service at Mazda Corporate. Been told that is is a software problem with no fix and no time frame of when there will be a fix. I do have a case number with Mazda. Today after threatening calls to invoke "Lemon Law" in my state Mazda has suddenly said that the car has a faulty inverter and that the part is available and will be replaced. My feeling is that they are just throwing parts at a problem. This car is unsafe and NHSTA should put pressure on Mazda to either resolve issue or buy back cars. The majority of the complaints listed here are the same. Please take action!!

On April 3rd, around 5:20 PM, while driving with my daughter on a suburban street, my Mazda hybrid experienced sudden brake failure as I approached a stop sign. Attempts to engage the brakes failed, but they unexpectedly resumed, allowing the car to stop safely. This incident triggered multiple dashboard warnings including "Engine Malfunction," "Hybrid System Malfunction," and "i-Activesense System Malfunction." The hybrid battery’s range indicator also fluctuated erratically between 1 and 4 miles. The primary malfunction involved the vehicle’s braking system, with concurrent issues in the engine and hybrid systems as indicated by the alerts. The vehicle and all related components, along with the computer fault codes, are available for inspection and further testing at the Mazda dealership. The brake failure significantly endangered the safety of both myself and my daughter, as we nearly entered an active intersection uncontrollably, which could have led to a severe collision, posing a grave risk to both occupants and pedestrians. Despite extensive diagnostics and approximately 33 miles of test driving under various conditions, the dealership was unable to replicate the brake failure, raising serious concerns about the braking system’s reliability and predictability. The dealership's technicians, in cooperation with Mazda corporate engineers, have inspected the vehicle and analyzed the diagnostic data and fault codes. As of April 19th, the car remains with the dealership for

Driving up snowy pass on I-90 approaching the top of Snoqualmie Pass. Vehicle stalled and warning lights flashed critical warning and loss of power. The vehicle stopped in the middle of the highway and would not start or go into drive. After a few minutes, because of speeding and adverse conditions, another driver collided with my vehicle from behind. Another following vehicle lost control and ran off the road. Nobody was injured. Mazda has not provided any information to what happened with the vehicle. We added about 12k miles in the 5 months of driving it, so we had lots of testing with it, and never ran into this serious of an issue with this or any other vehicle.

When using adaptive cruise control on multiple lane highways/freeways the vehicle detects other cars/trucks that are in other lanes as being in your lane. (Shows on the dash display as in your lane). This fools the vehicle into believing it needs to make an emergency brake as a vehicle is in your lane and a crash is imminent when no such maneuver is necessary. This could cause a rear end crash or loss of control of the vehicle when it brakes unexpectedly with no real world cause. This has been an issue since vehicle is owned (July 2023) it is also widespread with this specific model [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

While connected to a level 2 charger, overheating caused both the charger and charging port of the vehicle to melt together such that it was very difficult to disconnect, and both were then unusable – needing replacement. (photos attached) We feel that there should be some safeguard in the vehicle to prevent this situation as it is a fire hazard. There were no warning lights, sounds, nor signals from the vehicle to warn of the problem. We were present when this occurred, so we could disconnect before a potential fire may have occurred. When we called for service, we had to wait two weeks for an appointment, and then were told that the car was not safe to drive until repaired. This was not considered a warranty item even though the car had fewer than 2800 miles and only owned by us for fewer than 7 months. We were charged $5000 for the charging port and cable assembly and $800 labor. (Copies of service receipts attached.) We contacted Mazda regarding our concern and were given Case #00182286. We received a reply by phone telling us that Mazda bears no responsibility for this (they feel ii was the fault of the charger) and that it is an insurance matter. In fact, auto insurance does NOT cover this. Our concern is that it very well may be a safety issue as people usually charge in their garages or near other cars and buildings in public. In either situation, a fire could be a serious hazard for many people.

On 12/09/23, my 2024 Mazda CX-90 PHEV, purchased on 11/11/23, just got out of the driveway to make a left turn around 500 feet from the driveway when there was resistance in turning the steering wheel left, causing vehicle to swerve to the right and hitting the fire hydrant on the side of the road located just around the corner. Both airbags deployed causing extensive damage to the passenger front side of the car. The automatic emergency braking did not engage to avoid collision with the fire hydrant. One of the passengers suffered avascular necrosis of the hip as a result of the accident. The vehicle was towed to a collision center, then towed to the Mazda dealership, but according to the dealership, no inspection report was available in their system as of 03/19/24. The insurance company declared the vehicle a total loss. There were no warning lamps, messages, or other symptoms of the problem prior to the failure.

Power Train
Filed Mar 18, 2024MD

At 3200 miles the vehicle had a major powertrain malfunction causing the vehicle to throw multiple error codes - Hybrid System Malfunction; Charging System Malfunction; Electrical Leakage Detected. Was instructed to pull over to a safe area immediately. Was unclear how long car would continue to be drivable. Once car stopped it became completely immobile. Repeated restart attempts would not let vehicle start ICE engine or shift into gear. Had to be towed via dollies to nearest dealer for diagnosis and repair. Dealer found a failed PTC heater unit as source of problem and replaced it (Mazda part- KMV6-61-FHX). Vehicle is once again drivable, but the dealer has no idea what would cause the unit to fail and how to avoid future problems. Dealer has heard about this with many vehicles and repaired several similarly. We are fortunate to have found a safe place to pull off the road before powertrain shutdown, but if not we would have been stuck on the road with no ability to move the vehicle and occupants to safety. I hope NHTSA can investigate whether a recall of that part or the software that controls it is needed. NHTSA should also look into the failure mode of the vehicle to see if a different approach is safer and more appropriate.

When I pressed gas pedal, it would not accelerate. Because I was in the far right lane, I had just about enough time to pull over onto the shoulder before the car stopped completely. The car was in EV mode (using all electric power) when this incident occurred with 20 miles of electric range still left. CX90 PHEV Premium Plus new purchase 12/2023. 1/24/2024: "Hybrid System Malfunction, Have the vehicle Inspected " On a busy freeway with big rigs and other cars during rush hour traffic (5:25pm), going about 70 MPH. Only 1700miles on car so far. 50-60 degrees Fahrenheit outside. 2mi left on charge, full tank of gas. Car completely unable to accelerate all of the sudden on busy highway once this light came on. Was able to pull off to shoulder, albeit challenging as unable to accelerate and navigate through fast cars in right 2 lanes. Was able to restart car, and go at limp speed but unable to get fast enough to merge back. Called tow truck. Car towed to Mazda dealership. Then unable to get off flatbed of the tow truck as would not go into reverse. Tow guy called me and the staff at Mazda called their Service Manager, had to reset that code. (Something glitch kicked in and locked the car from being able to shift gears). Mazda Service was responsive, worked on and got diagnostics the next day. Stated that it was a computer system glitch. So they restarted the whole system. light is gone and car drives during their testing. They can't explain it. So computer and electronic glitch and system misread leads to a MECHANICAL malfunction that stops the car to be able to be accelerated on freeway. I am a mom of 3 young kids, and lucky I am alive.

The car has an "Engine Malfunction" emergency light that goes on. I brought the car into the dealer right away - the car had $1k miles and is brand new, so it's quite scary to get this message. They cleared the error and told me not to worry about it, but were unable to provide a solution. It recurred immediately (within 1-2 days). I reached out to the service department and they told me to ignore it (that there is no fix in progress). My car is unable to change the Mi Drive mode (so it can only stay in Hybrid mode). This is unsafe and goes against the intended use of the car. For example, on a very cold day like today, I wish to warm the car in sport mode (aka not hybrid or electric) and I was unable to change the mode.

I am reporting a safety problem with the adaptive cruise control feature of my 2023 Mazda CX-90. The adaptive cruise control system does not detect the lanes correctly and applies the brakes unnecessarily, even when there are no cars in front of me. This happens especially on curves and when there are trucks on other lanes. This is very dangerous and I have been close to be rear-ended several times. I do not feel safe driving this car with this feature on. I haven't contacted my dealer, but it seems to be a well known issue for many owners. This vehicle at highway speeds will suddenly brake when intuitive cruise control is engaged. It mainly does this when the hwy is not straight and there is a vehicle ahead in a different lane or if there is a concrete median on either side. It’s not safe to be going 70-78mph and have your vehicle brake and bring speed down to 50-60. I’ve had intuitive cruise control on 3 other vehicles and none of them did this. Vehicles following become upset when vehicle brakes with no vehicle ahead of us. Also, it’s a danger of being rear ended or causing a road range incident when someone behind you thinks you are brake checking them.

When radar cruise control is on, the car sometimes brakes hard totally unexpectedly when it’s about to overtake to a slower-moving vehicle in the adjacent right lane, and despite the fact that the CX-90 does not have any vehicle directly in front of it in its lane. This happens quite regularly at highway, when I am guetting within 100-200 ft of a semi located on the adjacent right lane, even when the road is at right. I almost got rear ended at 75mph by another vehicle because of that, as they did not expect my car to brake while overtaking a semi at that speed with no car in front of me. Additionally, the mirror housings on both sides of the vehicle seem to be tightly attached to the doors. The vibration is strictly isolated to the glass inside the mirror housing on driver side.
